{"title":"Systematic evaluation of the clinical nursing pathway with the GRADE approach applied to functional exercise in patients with hip replacements before and after surgery","authors":"Lin Yang , Xin-Man Wang , Xiao-Lin Zuo , Shang-Qun Gong , Fan-Jie Meng","doi":"10.1016/j.cnre.2016.11.008","DOIUrl":"10.1016/j.cnre.2016.11.008","url":null,"abstract":"<div><h3>Objective</h3><p>To evaluate the effect of the clinical nursing pathway applied to functional exercise in patients with hip replacements before and after the operation.</p></div><div><h3>Methods</h3><p>The China National Knowledge Infrastructure (CNKI), WanFang Data, Chinese science and technology journal database (VIP), PubMed, Web of Science, EMBASE, CBM and the Cochrane Library (2015-5) were searched for randomized controlled trials (RCTs) on clinical nursing pathways for functional exercise in patients with hip replacements before and after surgery from June 2015 to January 2010. The references included in the literature were also retrieved. To meet the literature standard, 2 reviewers independently selected and extracted data according to the inclusion criteria and assessed the risks of bias. RevMan 5.3 software was used in this meta-analysis. The quality of evidence was evaluated using grade profiler3.6 software, the level recommended for grading.</p></div><div><h3>Results</h3><p>A total of 15 RCTs and 1248 patients were included. The meta-analysis showed that, in the clinical nursing path group, the Harris score of hip function [SMD = 3.35, 95%<em>CI</em> (2.53, 4.16), <em>P</em> < 0.00001] and incidence of thrombosis embolism [RR = 0.28, 95%<em>CI</em> (0.15, 0.53), <em>P</em> < 0.0001], pulmonary infection [RR = 0.33, 95%<em>CI</em> (0.14, 0.82), <em>P</em> = 0.02], urinary retention [RR = 0.22, 95%<em>CI</em> (0.09, 0.52), <em>P</em> = 0.0005], constipation [RR = 0.20, 95%<em>CI</em> (0.10, 0.40), <em>P</em> < 0.00001], patients' satisfaction for nursing care [RR = 1.26, 95%<em>CI</em> (1.17, 1.36), <em>P</em> < 0.00001] and shortened hospitalization times [SMD = −1.91, 95%<em>CI</em> (−2.39, −1.43), <em>P</em> < 0.0001]were statistically significantly better than those in the control group. However, in reducing joint dislocations [RR = 0.25, 95%<em>CI</em> (0.05, 1.15), <em>P</em> = 0.08], pressure ulcers [RR = 0.25, 95%<em>CI</em> (0.03, 2.19), <em>P</em> = 0.21], and incidence of complications [RR = 0.42, 95%<em>CI</em> (0.15, 1.12), <em>P</em> = 0.08], there was no statistically significant difference between the two groups. Funnel plot analysis of the average length of stay showed that there might be some publication bias in the literature. The GRADE evaluation results showed that the level of Harris scores for hip function was moderate and the incidence of thrombosis, urinary retention and satisfaction of patients regarding nursing were low, and the rest of the factors analyzed were very low.</p></div><div><h3>Conclusions</h3><p>The effect of the clinical nursing pathway applied to functional exercises in patients with hip replacements before and after surgery was significantly better than that of routine nursing. However, it was restricted by the evaluation grade of the research results and the standardization and uniformity of the research. {"title":"Construction of competency evaluation measures for operating room nurses","authors":"Yu-Mei Wang , Li-Juan Xiong , Yi Ma , Xing-Lian Gao , Wen-Fang Fu","doi":"10.1016/j.cnre.2016.07.001","DOIUrl":"10.1016/j.cnre.2016.07.001","url":null,"abstract":"<div><h3>Objective</h3><p>To develop a scientific and reasonable competency evaluation index system for operating room nurses (ORN) in order to provide a tool for selecting, training and evaluating operating room nurses in China.</p></div><div><h3>Methods</h3><p>A multiple-method approach including in-depth interviews and the Delphi and Analytic Hierarchy Process (AHP) was used. Eighteen interviews were undertaken to explore the components of ORN competencies, and then 30 operating room experts were consulted to gain consensus. Finally, the AHP was adopted to determine the relative weights of competency elements, thus increasing the accuracy of the results.</p></div><div><h3>Results</h3><p>Results of the interviews suggested that there were 32 items of ORN core competency. Results of the DHP (Delphi and AHP) revealed 22 ORN core competencies in four dimensions, such as specialized knowledge, professional ability, personality and self-motivation. The weights of these four dimensions were 0.477, 0.138, 0.256, and 0.128, respectively.</p></div><div><h3>Conclusions</h3><p>The construction of an ORN core competency evaluation index system in this study contained not only explicit competence but also implicit competence, which reflected the requirements for operating room nurses comprehensively in China. Disaster nursing combines medical science, sociology, psychology, and other disciplines; thus, it has become a comprehensive interdisciplinary branch of nursing. Disaster nursing has just recently been introduced to our country and is still in the stage of exploration. Nurses comprise the largest rescue force on disaster relief teams; therefore, disaster nursing competency is influential on the disaster response and recovery of victims after a disaster. Thus, this paper defines the concepts and elements of disaster nursing, including disaster nursing skill requirements and architectural framework. A three-round Delphi study was subsequently conducted to revise and validate the curriculum.</p></div><div><h3>Results</h3><p>Twenty-six experts participated in the evaluation and validation process. The authority coefficient was 0.88, and the Kendall coordination coefficient was statistically significant (<em>χ</em><sup><em>2</em></sup> test, <em>P</em> < 0.01). The final training objective contained 9 items, covering knowledge, skills and humanistic quality. The resulting data were compiled and analyzed using Excel 2003 and SPSS 13.0, and tests of reliability and validity were conducted.</p></div><div><h3>Results</h3><p>The indicator system yielded an overall Cronbach's ɑ value of 0.948, a test-retest reliability of 0.793, 3-dimensional Cronbach's ɑ values of 0.870–0.930, and a content validity of 0.92. Exploratory factor analysis was then performed to extract three common factors, which generated a cumulative contribution rate of 54.04%. The search terms were “autistic children” or “children with autism” and “music therapy” or “music treatment.” Studies of randomized controlled trials (RCTs) were included, and each publication included was assessed for quality. A meta-analysis was conducted using RevMan 5.1.</p></div><div><h3>Results</h3><p>Publications were selected based on the inclusion and exclusion criteria. Six research articles describing RCTs were included; the total sample size was 300 patients. The results of meta-analysis showed that music therapy improved mood [Risk ratio (<em>RR</em>) = 3.02, 95% confidence interval (<em>CI</em>) = 1.93–4.11, <em>Z</em> = 5.45, <em>P</em> < 0.000 01] and behavior (<em>RR</em> = 7.36, 95% <em>CI</em> = 4.28–10.44, <em>Z</em> = 4.69, <em>P</em> = 0.000 01) in children with autism.
