{"title":"An investigation of the health value and self-care capabilities of the elderly in urban-rural fringe area nursing homes and the related influencing factors","authors":"Ju-Ying Luo , Ling-Lin Kong , Zhi-Xia Zhang , Yan-Sheng Ye , Cui-Yu Bao , Li-Juan Tu , Ya-Zhen Yang","doi":"10.1016/j.cnre.2016.06.015","DOIUrl":"10.1016/j.cnre.2016.06.015","url":null,"abstract":"<div><h3>Objective</h3><p>To investigate the health value and self-care capabilities of the elderly living in urban-rural fringe area nursing homes and the factors that influence these variables.</p></div><div><h3>Methods</h3><p>A cluster sampling method was used to select 280 elderly individuals from seven urban-rural fringe communities in Xianning to complete a survey regarding their health value and self-care capabilities.</p></div><div><h3>Results</h3><p>The total health value and self-care capability scores of the elderly were 7.45 ± 1.45 and 100.25 ± 22.56, respectively. Both of these scores significantly differed by age, education level, marital status, and income (<em>P</em> < 0.05, <em>P</em> < 0.01). Self-care capability was correlated with health value (<em>r</em> = 0.521). A multivariate linear regression analysis showed that health value, marital status, and age predicted self-care capability.</p></div><div><h3>Conclusions</h3><p>Elderly people living in the urban-rural fringe area with higher health values also had higher self-care capabilities. {"title":"The effectiveness of a high-fidelity teaching simulation based on an NLN/Jeffries simulation in the nursing education theoretical framework and its influencing factors","authors":"Fen-Fen Zhu, Li-Rong Wu","doi":"10.1016/j.cnre.2016.06.016","DOIUrl":"10.1016/j.cnre.2016.06.016","url":null,"abstract":"<div><h3>Objective</h3><p>To investigate the effectiveness of a high-fidelity teaching simulation based on an NLN/Jeffries simulation in the nursing education theoretical framework and its influencing factors.</p></div><div><h3>Methods</h3><p>A high-fidelity teaching simulation on clinical nursing practices using intelligent human analogues was conducted with 200 students, and The Simulation Design Scale, and the Student Satisfaction and Self-Confidence in Learning Scale developed by the National League for Nursing were used to evaluate the training effectiveness and its influencing factors.</p></div><div><h3>Results</h3><p>For the high-fidelity teaching simulation, students gave scores of 4.36 ± 0.54 points for satisfaction and 4.33 ± 0.46 points for Self-Confidence. The students highly rated the five dimensions of teaching design, i.e., teaching objectives/information, assistance/support for students, problem solving, guided feedback, and fidelity. The teaching design was closely correlated with the satisfaction of the high-fidelity teaching simulation and self-efficacy, and the dimensions of teaching objectives/information and assistance/support for students were particularly strong predictors of teaching effectiveness.</p></div><div><h3>Conclusions</h3><p>A high-fidelity teaching simulation based on Jeffries' theoretical framework improved student satisfaction with the simulation and their Self-Confidence. Accordingly, the dialysate temperature was set as 37 °C, T+0.5 °C, T, and T−0.5 °C. The changes in blood pressure, heart rate, mean arterial pressure and dialysis-induced adverse reactions at the 4 different dialysate temperatures were consistently monitored.</p></div><div><h3>Results</h3><p>Patients who received hemodialysis with 37 °C and T+0.5 °C dialysate demonstrated an unstable blood pressure and a higher incidence of adverse reactions. Patients who received hemodialysis with T and T−0.5 °C dialysate showed a relatively stable blood pressure, heart rate, and mean arterial pressure during dialysis. In particular, dialysate at T−0.5 °C resulted in the most stable blood pressure, the fewest adverse reactions and the best self-assessed comfort scores (<em>P</em> < 0.01).</p></div><div><h3>Conclusions</h3><p>The dialysate temperature during hemodialysis for patients with hypertension should be set to a temperature based on patients' preoperative body temperate T or 0.5 °C below T. PICC catheter insertion was performed using the Seldinger approach and was delivered from the basilic vein to the superior vena cava in a B-ultrasound guided manner. Moreover, the point raised during the PICC process would be treated immediately.</p></div><div><h3>Results</h3><p>All of the PICCs were inserted successfully. The problems encountered are shown in the inserted central catheter (PICC) process, such as failures of venipuncture, the problem of inserting guide wire, the catheter was difficult to be delivered etc.</p></div><div><h3>Conclusions</h3><p>The B-ultrasound guided approach enhanced the average success regarding the PICC insertion, particularly for the subset with a poor condition in the elbow superficial vein. Six weeks after BOO modeling was established, BC was evaluated through bladder testing. Bladder tissues were then fixed and embedded in paraffin.
