{"title":"The Imaging Appearance of <i>EWSR1::PATZ1</i> Gene Fusion Central Nervous System Tumors.","authors":"Cedric Vanmarcke, Lukas Marcelis, Isabelle Vanden Bempt, Raf Sciot, Johannes Devos","doi":"10.5334/jbsr.3431","DOIUrl":"10.5334/jbsr.3431","url":null,"abstract":"<p><p><i>Ewing Sarcoma Breakpoint Region 1</i> and <i>POZ/BTB And AT Hook Containing Zinc Finger 1</i> (<i>EWSR1::PATZ1</i>) gene fusion central nervous system (CNS) tumors are increasingly recognized as a potential distinct entity, with only limited reported cases. The imaging characteristics of these tumors have not been well established. In this study, we provide a detailed radiological description of a case in a 24‑year‑old man and conduct a literature review to identify common imaging features. A total of seven cases, including our own, were evaluated. Histopathological diagnoses included two ependymomas, an infantile glioblastoma, an astroblastoma, a ganglioglioma, and two gliomas not otherwise specified. Common imaging patterns included avid contrast enhancement, intratumoral cysts, intraventricular location or extension leading to hydrocephalus, and sharp delineation. Additional frequently observed features included calcifications and hemorrhagic foci. In conclusion, although the histopathological appearance of <i>EWSR1::PATZ1</i> gene fusion CNS tumors is diverse, there are consistent imaging features. Cerebral magnetic resonance imaging (MRI) revealed marked cytotoxic edema in both cerebellar hemispheres and the brainstem, indicative of opioid‑induced neurotoxicity. Endovascular mechanical angioplasty may be performed if symptomatic CV is refractory to noninvasive medical management. Compliant and noncompliant balloons and manually adjustable mesh may be used in this indication. We describe our initial experience with the Comaneci (Rapid Medical, Yokneam, Israel) in cerebral vasospasm treatment following aSAH. <i>Methods:</i> All patients included in the prospective observational SAVEBRAIN PWI (NCT05276934 on clinicaltrial.gov) study who underwent cerebral angioplasty using the Comaneci device for the treatment of medically refractory and symptomatic CV after aSAH were identified. Patient demographic information, procedural details, and outcomes were obtained from electronic medical records. <i>Results:</i> Between February 2022 and June 2023, seven consecutive patients underwent CV treatment with the Comaneci. Angioplasty of 37 arterial segments (supraclinoid internal carotid artery, A1, A2, and A3 segments of the anterior cerebral artery and M1 and M2 segments of the middle cerebral artery) was attempted, and 35/37 (95%) were performed. The vessel diameter improved significantly following angioplasty (+64%), while brain hypoperfusion decreased (-45% of the mean <i>T</i> <sub>Max</sub>).
