{"title":"Long-Term Morphological Changes of Crosslinked Gelatin Stent.","authors":"Yuen Keat Gan, Hong Kee Ng, William H Morgan, Dao-Yi Yu","doi":"10.1111/ceo.14545","DOIUrl":"https://doi.org/10.1111/ceo.14545","url":null,"abstract":"<p><strong>Background: </strong>This study investigates the long-term morphological outcomes of crosslinked gelatin stents over a 10-year period.</p><p><strong>Methods: </strong>Participants from the FDA phase 1 and 2 trials for the XEN 140 stent were recalled for follow-up examinations between 9 and 12 years after their initial implantation. The stent area was evaluated using anterior segment optical coherence tomography (AS-OCT) in conjunction with clinical assessments. We focused on measuring specific alterations in stent dimensions, such as external surface pitting or 'nibbling', complete wall breaches and luminal obstruction.</p><p><strong>Results: </strong>We evaluated 11 eyes from 9 patients, with an average follow-up period of 11.2 years (SD ± 0.7). The mean preoperative intraocular pressure (IOP) was 22.5 mmHg (SD ± 4.9), which decreased to 14.7 mmHg (SD ± 1.2) a decade after surgery. Notably, four eyes no longer required antiglaucoma medication. Out of the 11 cases, only 8 yielded image quality suitable for analysis, all of which exhibited stent wall nibbling. The average nibbling depth was 70.17 μm (SD ± 36) in the intrascleral area, 79.5 μm (SD ± 18) in the proximal conjunctival region and 42.25 μm (SD ± 5) in the distal conjunctival region. Nibbling was most frequent in the proximal conjunctival area with 26 instances (p = 0.0006), followed by the intrascleral area, 21, and the distal conjunctival section, 9. Hyperreflective lumens were noted in five cases, with one complete stent wall discontinuity.</p><p><strong>Conclusion: </strong>In cases where imaging provided clear visibility, the stents exhibited signs of morphometric changes after 11 years. {"title":"Predictive Factors for Uveitis Refractory to Treatment in Initial-Onset Acute Vogt-Koyanagi-Harada Disease.","authors":"Thomas J Gin, Sujan A Surendran, Sophie L Rogers, Lyndell L Lim","doi":"10.1111/ceo.14538","DOIUrl":"https://doi.org/10.1111/ceo.14538","url":null,"abstract":"<p><strong>Background: </strong>To identify predictive factors of uveitis refractory to treatment in initial-onset acute Vogt-Koyanagi-Harada disease.</p><p><strong>Methods: </strong>This was a retrospective chart review of patients with initial-onset acute Vogt-Koyanagi-Harada disease presenting to the Royal Victorian Eye and Ear Hospital, Melbourne, Australia between July 2001 and March 2023 inclusive. Factors predictive of uveitis refractory to treatment were determined using logistic regression models with, and without, adjustment for initial use of intravenous methylprednisolone.</p><p><strong>Results: </strong>Thirty-eight patients with initial-onset acute Vogt-Koyanagi-Harada were included, 27 of whom received initial intravenous corticosteroids (71%) and 23 met the criteria for uveitis refractory to treatment (61%). Comparing those who received intravenous corticosteroids to those who did not, the incidence of complications, time-to-quiescence, time-to-relapse, time-to-low dose corticosteroid or corticosteroid-sparing control of inflammation and time-to-remission were not statistically different (all p > 0.164). Factors at onset of treatment that were predictive for uveitis refractory to treatment included greater anterior chamber inflammation (p = 0.008), greater vitreous inflammation (p = 0.015), the absence of bacillary layer detachments on macular optical coherence tomography (p = 0.010) and commencement of systemic steroid therapy 1 week or longer after ocular symptom onset (p = 0.013). {"title":"Genetic Association of Primary Angle-Closure Glaucoma and Disease Progression.","authors":"Yu Jing Liang, Anni Ling, Poemen P Chan, Jason C Yam, Chi Pui Pang, Clement C Tham, Li Jia Chen","doi":"10.1111/ceo.14539","DOIUrl":"https://doi.org/10.1111/ceo.14539","url":null,"abstract":"<p><strong>Background: </strong>To investigate single-nucleotide polymorphisms (SNPs) reported in the largest up-to-date systematic review and meta-analysis on primary angle-closure disease (PACD), on their associations with primary angle-closure glaucoma (PACG) and disease progression.</p><p><strong>Methods: </strong>This study involved a case-control design for PACG risk and a case-only design for PACG progression risk, including 628 PACG patients and 564 controls for disease association and 386 PACG patients with up to 10-year follow-up for PACG progression analysis. Associations of 17 SNPs in 15 genes with PACG were analysed using logistic regression. Sex-stratified association analysis was performed, followed by the Breslow-Day test. Genetic risk for PACG progression was evaluated using logistic regression. Bonferroni correction of p values was adopted for multiple comparisons.</p><p><strong>Results: </strong>LOXL1 rs3825942 (G153D; p = 0.0026; OR = 0.65) was significantly associated with PACG, while ABCC5 rs1401999 showed a nominal association (p = 0.023; OR = 1.32). ABCA1 rs2422493 was significantly associated with PACG in females (p = 0.0016; OR = 0.70) but not in males (p = 0.95; OR = 0.99); and the Breslow-Day Test (p = 0.046) suggested a sex-specific association in females. Information about their eye health is scarce, particularly for those detained offshore. This is the first study to explore the eye health and services available to this population, helping inform future service planning.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Methods</h3>\u0000 \u0000 <p>A mixed-methods design was employed, incorporating a multicentre audit of an ophthalmology service in Nauru and Manus Island detention centres (2015–2016) and a comparative analysis of this service against established standards of public eye care in mainland Australia. Outcomes included prevalence and causes of vision impairment, management required, and service performance using the Australian Health Performance Framework.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>Eighty patients from 14 countries were included (3–57 years, 84% male). There were high rates of bilateral and unilateral vision impairment (19% and 25%, respectively) and bilateral and unilateral blindness (1% and 12%). Bilateral vision impairment was mostly avoidable (80%), with the commonest causes being refractive error and cataract. Ocular trauma accounted for 67% of unilateral blindness. Compared to mainland services, offshore eye care was sub-standard across all performance domains, including governance, information, workforce, safety, and effectiveness.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusions</h3>\u0000 \u0000 <p>Refugees and asylum seekers held in Australia's offshore detention centres had a high burden of eye disease and inadequate access to services.
