We report the case of a 77-year-old woman with known renal cell carcinoma treated ten years ago, and recently presented with cholangitis and a liver lesion. 18FDG PET-CT showed a mild hypermetabolic lesion at segment V. 68Ga-FAPI PET/CT was performed for further assessment. 68Ga-FAPI PET/CT demonstrates an intensely tracer avid lesion at hepatic segment V. These findings are likely due to primary cholangiocarcinoma. Over past few decades, the obesity epidemic has lead to an increased incidence of obesityrelated nephropathy, characterised by glomerular hyperfiltration, hypertrophy, and progressive glomerulosclerosis. This review highlights the need for multidisciplinary action to prevent and manage obesityrelated nephropathy. We reiterate the current necessity of routine kidney function screening in people with obesity. We also propose an integrated concept of Renal Barocrinology. Data was collected using the online RedCap questionnaire. Data was analysed using SPSS 26.</p><p><strong>Results: </strong>Of the 412 participants, 242(58.7%) were females. The overall median age was 21 years (interquartile range: 20-23 years). This communication lists the facets of metabolic care in a simple (Seven Rs) manner, connecting them with the various levels of prevention. All individuals should be counselled about regulation of lifestyle and mindstyle (primordial prevention). Those at risk of metabolic disease should reduce/redistribute their energy intake, and rescue overburdened storage system by using energy through exercise (primary prevention). Persons with established disease will need physio-friendly therapy to replenish and restore the physiological buffering of the body (secondary prevention). In persons with target organ damage, resilience of organ- systems can be enhanced, and existing damage repaired, by using modern drugs (tertiary prevention). Quaternary prevention and quinary prevention must be practiced. {"title":"Evaluating postoperative complications in radical cystectomy and ileal conduit for bladder cancer using the clavien-dindo grading system.","authors":"Shuah Ullah, Laraib Gul, Shariq Bin Yousuf, Bazgha Hassan, Tanzeel-Ur-Rahman Gazder, Asad Shahzad Hassan","doi":"10.47391/JPMA.11614","DOIUrl":"10.47391/JPMA.11614","url":null,"abstract":"<p><strong>Objective: </strong>To determine the frequency of postoperative complications after radical cystectomy with ileal conduit surgery in patients diagnosed with urinary bladder cancer.</p><p><strong>Methods: </strong>This prospective observational study was conducted at the Department of Urology, Sindh Institute of Urology and Transplantation (SIUT), Karachi, from 30TH December 2019 to 30TH June 2020, following approval from the SIUT Ethics Review Board. The study comprised patients aged 25-70 years diagnosed with urinary bladder cancer who underwent elective radical cystectomy with ileal conduit. Complications were compared with respect to preoperative characteristics, such as disease stage, chemotherapy, radiotherapy, and intraoperative variables, like operation time, predicted blood loss, and the requirement for blood transfusion, as well as postoperative therapies. All postoperative complications occurring within a 60-day period were classified using the Clavien-Dindo grading system. Data was analysed using SPSS 22.</p><p><strong>Results: </strong>Of the 34 patients, 22(64.7%) were males and 12(35.3%) were females. The overall mean age was 51.00±12.84 years. Grade 0 complications were observed in 14(41.18%) patients, grade 1 13(28.24%), grade 2 3(8.82%), grade 3a 2(5.88%), and grade 4a 2(5.88%). We present the case of a 16-year-old female with NF2, seen on April 29, 2023, at Nishtar Hospital, Multan, exhibiting symptoms such as swollen left eye, chronic headache, decreased vision, bilateral hearing loss, tinnitus, and gait issues. Diagnostic evaluation, including Magnetic Resonance Imaging (MRI), confirmed NF2 diagnosis based on National Institutes of Health (NIH) and Manchester criteria. Surgical resection is the primary treatment, but the patient refused and opted for herbal treatment due to personal and socioeconomic reasons. What makes this case report interesting is the presence of severe refractory hypokalaemia. He was admitted to the Medical D unit of Ayub Teaching Hospital, Abbottabad, on January 14, 2024. His serum potassium remained low at 2.4mmol/L, (n=3.5-5.1 mmol/L) despite aggressive potassium replacement. Urinary electrolyte analysis showed renal potassium wasting, likely due to activation of the renin-angiotensin-aldosterone system (RAAS). Elevated serum renin levels supported this mechanism. In addition, the patient also had acute kidney injury and electrolyte abnormalities potentially caused by excessive lysozyme secretion by monocytes, as described in a prior case report. Refractory hypokalaemia in AML M4 is rare and has a poor prognosis. Early recognition and management of hypokalaemia is crucial to prevent complications and improve patient outcomes.
