Currently, there is no universally accepted and effective treatment protocol in clinical practice. This study aims to investigate a stepwise treatment approach for chronic myringitis under otoendoscopy.</p><p><strong>Methods: </strong>A retrospective analysis was performed on the clinical data of 118 patients with chronic myringitis who were treated at the Otology Medical Center of the First People's Hospital of Foshan between March 2021 and September 2024. Based on the severity and extent of the disease, patients were categorized into four grades (I-IV), and personalized tiered treatments were administered accordingly.</p><p><strong>Results: </strong>Among the patients, 21 cases were classified as Grade I, using a crochet hook to physical scraping and powder spraying; 56 Grade II cases were treated with argenti nitrate or clotrimazole solution for chemical cauterization; 14 Grade III cases underwent Jesse bipolar cauterization; and 27 Grade IV cases received ear endoscopic tympanoplasty. Notably, two Grade I and eleven Grade II patients who did not demonstrate significant improvement after physical scraping, powder spraying, or chemical cauterization subsequently recovered following treatment with the Jesse bipolar technique.</p><p><strong>Conclusion: </strong>This study proposes a novel stepwise treatment strategy based on clinical grading of chronic myringitis under otoendoscopy. Both body mass index (BMI) and waist circumference (WC) are associated with increased CRS prevalence, severity, and potentially poorer treatment outcomes due to sustained inflammation and altered immune responses. This study investigates how BMI and WC influence CRS severity and response to therapy.</p><p><strong>Methods: </strong>This two-year observational study included adult CRS patients, assessing BMI, WC, Rhinosinusitis Disability Index (RSDI), Sinonasal Outcome Test-22 (SNOT-22), and Lund-Kennedy endoscopic scores, before and after standard medical/ surgical treatment (3 months).</p><p><strong>Results: </strong>Of the 43 CRS patients, most showed significant symptoms and endoscopic score improvements over three months, regardless of BMI or waist circumference. While RSDI and Lund-Kennedy scores showed some group-wise differences, overall outcomes were not significantly influenced by BMI or WC categories.</p><p><strong>Conclusion: </strong>Obesity measures, as indicated by BMI, do not influence baseline CRS symptom severity but are associated with greater endoscopic improvement following treatment. Sialendoscopy was performed on the most affected gland (11 parotid, 8 submandibular glands), with 60 mg of methylprednisolone in a 5 ml solution of 0.9% NaCl saline irrigated into the ductal system. Serum cortisol and methylprednisolone concentrations were measured before irrigation, at 2 and 24 h (h) post-irrigation. Patients were examined for post-interventional symptoms and signs 24 h later.</p><p><strong>Results: </strong>Serum cortisol concentrations increased at 2 h (p < 0.05), followed by a decrease (p < 0.01) at 24 h. Decrease was recorded in serum cortisol from 0 h to 24 h (17.2 ± 5.9 vs. 9.2 ± 8.0 µg/dl, respectively, p < 0.01); 37% of patients fulfilled the criteria for HPA axis suppression (serum cortisol ≤ 1.8 µg/dl). A similar pattern for serum cortisol was observed for the parotid and submandibular glands separately, with more pronounced decrease in the parotid. Serum methylprednisolone increased at 2 h, followed by a decrease to minimal concentrations at 24 h. Pain during methylprednisolone irrigation was reported in 57.9% of patients. Blood pressure and glucose concentrations remained unaffected, and no other symptoms were reported.</p><p><strong>Conclusion: </strong>Methylprednisolone irrigation during sialendoscopy caused biochemical HPA axis suppression in 37% of patients after 24 h. However, the procedure is considered safe, as there were no reported cases of clinical hypocortisolemia. The decrease in serum cortisol at 24 h was observed mainly after parotid gland irrigation. Despite surgical management, the disease presents a high recurrence rate, which motivates the search for adjuvant therapies such as Bevacizumab. This study evaluates the effectiveness of Bevacizumab in reducing recurrence rates and prolonging intervals between surgeries.</p><p><strong>Methods: </strong>A systematic review was performed in PubMed, BVS and Cochrane database, including studies published between 2014 and 2024. Randomized clinical trials and observational studies evaluating Bevacizumab in pediatric and adult patients with PLR were included. The quality of the evidence was assessed with GRADE. Four systematic reviews were included as references for discussion; however, they were excluded because they were not primary studies.</p><p><strong>Results: </strong>Ten studies were included with a total of 173 patients, 69 of whom received Bevacizumab. The age range of treated patients was 0 months to 86 years. Bevacizumab doses ranged from 5-15 mg/kg based on weight, 25mg/ml based on concentration and a single event 400 mg single dose. Before treatment, the recurrence rate ranged from 1-26 surgeries/year and the interval between surgeries ranged from 1 week to 4.34 +- 1.446 months. After treatment, recurrence decreased to 0 to 4 surgeries/year and the intervals extended from 3 months to 14.81 +-12.421 months. The most common side effects, especially with systemic administration, included respiratory complications (glottic stenosis, dysphonia, dyspnea, stridor, aphonia), systemic disturbances (hypertension, mild proteinuria, headache, dysgeusia) and hematological effects (thrombocytopenia). One case of malignant transformation of the lesion was also reported. \"follow-up time ranged from 3 months to 5 years.</p><p><strong>Conclusions: </strong>Bevacizumab is a promising adjuvant therapy for the management of PLR, with significant benefits in reducing recurrence rates and surgical intervals. However, the quality of the available evidence is medium to low, and randomized controlled trials are required to validate its safety and efficacy. {"title":"Assessing the application potential of different species as obstructive sleep apnea models: a comparative analysis based on cranial CT anatomy.","authors":"Yiwei Feng, Weisong Cai, Xiaoping Ming, Chen Xiong","doi":"10.1007/s00405-025-09711-3","DOIUrl":"https://doi.org/10.1007/s00405-025-09711-3","url":null,"abstract":"<p><strong>Purpose: </strong>The goal of this study was to compare the cranial anatomical characteristics of different animals to provide a scientific basis for the selection of obstructive sleep apnoea (OSA) animal models.</p><p><strong>Methods: </strong>We analysed the computed tomography (CT) scan images of humans, Macaca, rabbits, mice, rats, and tree shrews; employed 3D reconstruction and measurement techniques to assess the morphological characteristics of the mandible, hyoid bone, and thyroid cartilage; normalized volumes of the tongue and soft palate, fat distribution, and volume and morphology of the airway in each species; and compared their similarity to the corresponding human structures.</p><p><strong>Results: </strong>Macaca has the greatest similarity to humans in terms of skeletal structure, airway morphology, and fat distribution. With respect to skeletal structure and soft palate research, the anatomical features of tree shrews make them highly competitive candidates. With complex anatomy, conventional surgery is challenging. Endoscopy-assisted transoral approach (EATA) minimizes trauma and scarring; however, criticism regarding infeasibility with large tumors led to the introduction of transoral robotic surgery (TORS), which gives a 3D view of the field, reduces operative time, and is effective for larger tumors but faces technical and operational issues. This review compares the outcomes and complications of TORS and EATA.</p><p><strong>Methods: </strong>We searched four databases for relevant articles up to January 2025. Two independent reviewers extracted data from the selected studies, including baseline information, operative time, blood loss, and hospitalization time.</p><p><strong>Results: </strong>We reviewed 32 studies, of which 27 provided sufficient data for analysis. These studies involved 348 patients treated for PPS tumors using either EATA or TORS. Operative times were similar for both methods, with EATA averaging 92.31 min and TORS averaging 93.93 min (p = 0.9385). However, compared to EATA, TORS was associated with reduced blood loss (16.98 ml, 95% CI: 9.38-24.59; p < 0.0001) and a shorter hospitalization duration (3.39 days, 95% CI: 2.51-4.27; p = 0.0278). Common complications associated with endoscopy included Horner's syndrome and wound dehiscence, whereas TORS rarely reported severe complications. Clinical evaluation included ophthalmological examination by fundoscopy and optical coherence tomography. Optic nerve sheath diameter (ONSD) measurement was performed using ultrasound scanning. Neuroimaging was performed by the acquisition of heavily T2 weighted MR images of the brain and skull base. Patients were categorised into definite, probable and no IIH using the Vellore diagnostic criteria.</p><p><strong>Results: </strong>Fifty-five patients (45 (81.8%) being women) with a mean age of 43.4+/-9.3 years were recruited to the study. Besides CSF rhinorrhoea, headache was the commonest symptom (70.9%). Eleven (20%) patients had papilledema. Empty sella (Yuh et al. grade IV and V) (74.5%) was the commonest MR finding suggestive of IIH. The overall prevalence of IIH in patients with SCSFR was 85.8%. Of these, 35 (63.7%) had definite IIH, 12 (21.8%) had probable IIH and 8 (14.6%) had no IIH. Older patients were more likely to have definite IIH (mean age, 45.5 ± 9 years) than probable IIH (mean age, 39 ± 8.5 years) with an odds ratio of 1.095 (95% CI, 1.001-1.199)(p = 0.048). Among patients with definite IIH, Meckel's cave enlargement (49.1%) was more frequently seen than posterior globe flattening (21.8%).</p><p><strong>Conclusion: </strong>Patients with SCSFR have a high likelihood of having IIH. However, data on the prevalence of WRMD among ENT physicians in Germany are lacking.</p><p><strong>Methods: </strong>A nationwide online survey was conducted among members of the German Society for ENT to assess demographic data, work-related factors, and musculoskeletal complaints. Data were analyzed using descriptive statistics.</p><p><strong>Results: </strong>A total of 751 ENT physicians participated (53% female; mean age 51 years). Lifetime prevalence of neck complaints was 93%, with 82% reporting symptoms in the past 12 months. Shoulders, upper and lower back were also frequently affected. More than a half (53%) reported limitations in professional activities, and 22% had taken sick leave due to these complaints. The prevalence of symptoms increased with years of professional experience. Female physicians reported higher rates of neck and upper back complaints compared to males.</p><p><strong>Conclusion: </strong>This study reveals a high prevalence and significant burden of WRMD among German ENT physicians, particularly in the neck and upper back regions.
