This study investigated whether CI users fitted with fine structure (FS) strategies can discriminate low-frequency (LF) pitch changes via RP and whether RP performers show better speech recognition than PP performers.</p><p><strong>Methods: </strong>Thirty postlingually deafened adult CI users (15 unilateral, 15 bilateral) participated in this study. LF pitch discrimination linked to temporal fine structure (TFS) sensitivity was assessed with the Disharmonic Intonation test (A§E psychoacoustic test suite), while speech perception was evaluated with phonetically balanced words and everyday sentences (both in quiet and noise), and the Italian Matrix test (in adaptive mode).</p><p><strong>Results: </strong>Just noticeable differences (JNDs) in RP performers were significantly better compared to PP JNDs (p < 0.001, r = 0.80). No significant differences were found between unilateral and bilateral CI users for LF pitch perception (p > 0.05). Within-group comparisons (better ear versus bilateral listening) revealed that better ear JNDs were not significantly different from the bilateral performance (p > 0.05), whereas significant bilateral benefit was observed for word recognition score (WRS) + 10 (p = 0.016, r = 1.00), WRS + 5 (p = 0.021, r = 1.00), and Matrix performance (p = 0.033, r = 0.80). This scale was applied to 203 patients who presented to our clinic with nasal obstruction and to a control group consisting of 135 people without nasal obstruction, and the significance of the scale was tested. Validity, reliability, and factor analyses were performed on the prototype scale, the number of questions and subgroups were determined, and the final version of the scale was created. The final version of the scale was reanalyzed for reliability and administered twice at 3-week intervals to 125 patients with nasal obstruction who were scheduled for septoplasty in our clinic.</p><p><strong>Results: </strong>The scale consisted of 20 items in total and four factors: \"functional problems,\" \"sleep problems,\" \"social problems,\" and \"emotional problems.\" Reliability analysis showed that the Cronbach's alpha coefficient of the scale was 0.91. In the item analyses, item-total and item-remainder correlations were significant (p < 0.001), and discriminant analysis showed that all item and factor scores were discriminative. However, the surgical technique has not been adequately validated. Circumferential resection of the stenotic airway segment was done with primary end-to-end anastomosis. All surgeries were performed by the authors of this work with the same standardized surgical techniques. To decrease the anastomotic tension, suprahyoid release was performed for all patients and trachea-hyoid detensioning stitches were placed. Surgiflo was applied over the line of the anastomosis to enhance healing. Surgical and functional outcomes were evaluated.</p><p><strong>Results: </strong>Types of anastomosis were cricotracheal anastomosis (n = 18), thyrotracheal anastomosis (n = 24), and tracheo-tracheal anastomosis (n = 11) according to the remaining proximal and distal stumps. The overall decannulation rate was 92.5% (49 out of 53 patients). No major intraoperative complications were reported. Postoperative complications were reported in 13 patients (24.5%), in the form of restenosis (n = 7), granulation tissue formation at the site of anastomosis (n = 5), surgical emphysema / minor air leak through drains (n = 4), unilateral vocal fold paralysis (n = 2), wound seroma (n = 1). Regarding functional outcomes, dyspnea was considerably alleviated both at rest and during exercise, and most patients had satisfactory voice and swallowing related functions. The majority of patients reported adequate QOL.</p><p><strong>Conclusion: </strong>Revision tracheal/cricotracheal resection anastomosis presents significant surgical challenges. {"title":"Double dehiscence (Superior semicircular canal and tegmen tympani) in the epicampaniform period (Arboli type).","authors":"A I Cisneros-Gimeno, A García-Barrios, S Baena-Pinilla, J Obón-Nogués, R Gómez-Miranda, J Whyte-Orozco, M Botella-López","doi":"10.1007/s00405-025-09445-2","DOIUrl":"https://doi.org/10.1007/s00405-025-09445-2","url":null,"abstract":"<p><strong>Purpose: </strong>Although the superior semicircular canal dehiscence syndrome was described at the end of the 20th century, we want to check if it is a pathology that has existed since ancient times, through the anthropological study of bone remains.</p><p><strong>Methods: </strong>We have carried out an anthropological and radiological study (CT scan) of 8 skulls found in caves, as secondary burials of the Arbolí type epicampaniform culture (1800 - 1700 BC) on the Iberian Peninsula.</p><p><strong>Results: </strong>The 8 skulls (16 temporal bones) show a grade 4 degree of pneumatisation or hyperpneumatization. One of these skulls, belonging to a male subject of around 25-30 years of age, shows a double dehiscence (superior semicircular canal and tegmen tympani) on the right side, and a possible congenital muscular torticollis on the same side.</p><p><strong>Conclusion: </strong>Superior semicircular canal dehiscence syndrome already existed in an inhabitant from 1800 - 1700 BC (Iberian Peninsula). Identifying maxillary sinus pathologies before surgical interventions is crucial for ensuring successful treatment outcomes. Cone beam computed tomography (CBCT) is commonly employed for maxillary sinus evaluations due to its high resolution and lower radiation exposure. This study aims to assess the accuracy of artificial intelligence (AI) algorithms in detecting maxillary sinus pathologies from CBCT scans.</p><p><strong>Methods: </strong>A dataset comprising 1000 maxillary sinuses (MS) from 500 patients was analyzed using CBCT. Sinuses were categorized based on the presence or absence of pathology, followed by segmentation of the maxillary sinus. Manual segmentation masks were generated using the semiautomatic software ITK-SNAP, which served as a reference for comparison. A convolutional neural network (CNN)-based machine learning model was then implemented to automatically segment maxillary sinus pathologies from CBCT images. To evaluate segmentation accuracy, metrics such as the Dice similarity coefficient (DSC) and intersection over union (IoU) were utilized by comparing AI-generated results with human-generated segmentations.</p><p><strong>Results: </strong>The automated segmentation model achieved a Dice score of 0.923, a recall of 0.979, an IoU of 0.887, an F1 score of 0.970, and a precision of 0.963.</p><p><strong>Conclusion: </strong>This study successfully developed an AI-driven approach for segmenting maxillary sinus pathologies in CBCT images. This study evaluates the impact of different intratympanic injection techniques on patient outcomes.</p><p><strong>Methods: </strong>A retrospective analysis was conducted on patients diagnosed with idiopathic SSNHL (January 2008-May 2023). Patients were grouped by application technique: (1) transtympanic injection under local anesthesia (TI), (2) CO<sub>2</sub> laser paracentesis under general anesthesia (LP), and (3) VT application under general anesthesia (VT). Primary outcomes included hearing improvement and tympanic membrane perforation closure time.</p><p><strong>Results: </strong>The study included 99 patients: 28 with TI, 34 with VT, and 37 with LP. Groups showed no significant differences in sex, affected side, therapy initiation interval, pretreatment type, preoperative BCPTA4, subjective improvement, or treatment-related pain (all p > 0.1). Hearing thresholds improved significantly in the LP and VT groups with large (d = 0.832, p < 0.001) and medium (d = 0.513, p = 0.01) effect sizes, respectively, but not in the TI group (d = 0.294, p = 0.137). Tympanic membrane closure time differed significantly among groups (p < 0.001).</p><p><strong>Conclusions: </strong>LP and VT significantly improved hearing thresholds in SSNHL patients.
