The changes in Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ) scores, lymphocyte subsets, and serum immunoglobulin levels were compared before and after treatment between the two groups.</p><p><strong>Results: </strong>After treatment, both groups showed a significant reduction in the frequency of allergic rhinitis episodes (P < 0.05). The RQLQ scores indicated that the observation group showed more significant improvements in daily activities, rhinitis-related behaviors, nasal symptoms, emotional responses, and overall score (P < 0.05), while the control group showed improvements in only some of the indicators (P < 0.05). In the observation group, NK cell levels (6.28% [3.46; 8.55] vs. 7.80% [5.90; 13.87], P = 0.025), Treg cell levels (0.83 ± 0.50% vs. 1.20 ± 0.47%, P = 0.016), serum IgA (0.97 ± 0.34 vs. 1.49 ± 0.58 g/L, P = 0.001), and IgG levels (9.32 ± 2.85 vs. 11.11 ± 2.09 g/L, P = 0.019) were significantly increased, while CD8 + T cell levels decreased (28.84 ± 5.24% vs. 25.62 ± 5.85%, P = 0.037). The role of fungal pathogens and the need for antifungal treatment remain controversial. We aimed to evaluate clinical characteristics, diagnostic challenges, and microbiology results in fungal NOE and assess considerations for antifungal therapy.</p><p><strong>Methods: </strong>This retrospective cohort study was conducted at two tertiary centers. NOE patients, based on clinical and radiological findings, were retrospectively reviewed. Clinical characteristics, microbiological findings, and disease persistence were compared between patients categorized by antifungal treatment status: Combined antifungal-antibiotic therapy versus antibiotic therapy alone.</p><p><strong>Results: </strong>Of 125 patients, 25 received combined treatment (13 as initial treatment and 12 after persistent disease) and 100 received antibiotics alone. Combined treatment patients were younger, had prolonged symptoms, and more cranial nerve palsy, requiring longer treatment for persistent and aggressive disease. Fungi were found in 72% of combined-treatment cultures, but most were deemed unreliable by infectious diseases specialists, inconsistently leading to antifungal therapy.</p><p><strong>Conclusion: </strong>Antifungal therapy decisions in NOE lack standardization. We propose a protocol incorporating clinical and microbiological data to guide antifungal therapy. A retrospective review was performed on r-HNC cases evaluated in the last 10 years by an HNC expert pathologist at a referral center. Major diagnostic discrepancies (MDD), defined as changes in pathological diagnosis that significantly influenced clinical management, were analyzed.</p><p><strong>Results: </strong>Among the 30 surveyed participants, 67% sought a 2°op for r-HNC, often leading to changes in diagnosis or treatment. In 130 reviewed cases, 66% had no initial pathological diagnosis; of the remaining 34% (44) with a previous diagnosis, 50% were changed after the review. Minor salivary gland tumors of the oral cavity (57%) had the highest rate of MDD, followed by the sinonasal tract (43%). 14% (6) involved only descriptive diagnosis reformulation/reclassification whereas of MDD (16%,7), 29% involved a change from benign to malignant, 14% a change from malignant to benign.</p><p><strong>Conclusions: </strong>According to the complexity of HNC district, a second opinion is often needed and when a preliminary histopathological diagnosis was formulated, a MDD results in 16%. Although ABR and ASSR principally show strong correlations in hearing threshold estimation, there is preliminary evidence that temporal bone malformations might be associated with significantly greater differences between ABR- and ASSR-results. Therefore, the present study aimed to compare hearing threshold estimation derived from ABR and ASSR in a larger cohort of single-sided deafness (SSD) of various etiology, including temporal bone anomalies.</p><p><strong>Methods: </strong>The diagnostic consistency between ABR and ASSR using narrow-band-chirp-stimuli at 1000, 2000, and 4000 Hz was analyzed in 47 children with single-sided deafness with varying MRI-morphologic findings: cochlear nerve malformation (CNM, n = 24), cochlear malformation (CM, n = 7) vs. combined malformation (CM + CNM, n = 8) vs. absent temporal bone and inner ear pathology (n = 8). Children with additional health issues other than SSD were excluded.</p><p><strong>Results: </strong>ABR and ASSR showed a strong correlation in deaf ears without malformation (r = 0.728, p < 0.0001), a moderate correlation in isolated cochlear malformation (r = 0.574, p = 0.01), and a weak correlation in case of cochlear nerve anomaly (r = 0.189, p = 0.112 in CNM and r = 0.235, p = 0.268 in CM + CNM). Ears with isolated CNM showed an average discrepancy of 23.40 ± 15.19 dB, p < 0.00001 between ABR and ASSR (vs. 17.08 ± 15.81 dB, p = 0.0008 in CNM + CM, vs. 7.63 ± 8.56 dB, p = 0.008 in CM, vs. 4.38 ± 4.96 dB, p = 0.036 in ears without malformation and vs. 0.36 ± 4.75 dB, p = 0.748 in healthy control ears). In ears with highly discrepant ASSR and ABR values, enlarged ABR wave I and otoacoustic emissions were frequently present.</p><p><strong>Conclusion: </strong>In cochlear nerve malformation, ASSR and ABR frequently provide significantly discrepant hearing threshold estimations, probably derived from a cochlear origin. ASSR should only be used in conjunction with conventional ABR in the diagnostic management of suspected severe-profound hearing loss or deafness in children. This study assessed the potential of a novel \"Black Bone\" (BB) MRI sequence to determine RW visibility during posterior tympanotomy.</p><p><strong>Methods: </strong>Patients underwent BB MRI as part of pre-operative CI planning. Two independent radiologists performed 5 landmark-based MRI measurements. RW visibility was recorded during posterior tympanotomy as > 50% or < 50% visibility. Patients with undefinable landmarks or absent surgical grading were excluded. Mann-Whitney U or t-tests compared MRI measurements to RW visibility whilst step wise logistic regression determined significant predictors.</p><p><strong>Results: </strong>86 patients (40 male; median 10.5 years) and 129 implanted ears (43 unilateral, 43 bilateral; 52 adult, 77 paediatric) were evaluated. There were 109/129 ears with > 50% and 20/129 ears with < 50% RW visibility. The external auditory canal (EAC) angle was increased in paediatric patients with > 50% RW visibility (P = 0.033; AUROC 0.689; 73.4% sensitive, 69.2% specific with threshold 6.2<sup>o</sup>) with ICC = 0.598. The facial recess distance, facial nerve location, and modified RW niche angle were increased in adult patients with > 50% RW visibility but only modified RW niche angle was significant on stepwise regression (P = 0.016; AUROC 0.805; 82.2% sensitive, 71.4% specific with threshold 17.95<sup>o</sup>) with ICC = 0.578.</p><p><strong>Conclusion: </strong>The visibility of RW at posterior tympanotomy may be determined with pre-operative BB MRI. In this Letter, we reflect on the study's key findings, its public health implications, and areas requiring further investigation. While the observed decline in age-standardized mortality rates is encouraging, significant disparities-particularly in Eastern Europe, Central Asia, and parts of Southeast Asia-underscore persistent risk heterogeneity. Patients treated with the combined approach were included; those managed solely with sialendoscopy, exclusively via incision, or conservative treatment were excluded. Medical records were reviewed for outcomes including stone removal success, symptom resolution, and complications.</p><p><strong>Results: </strong>Of 243 patients diagnosed with sialolithiasis, 33 underwent the combined approach - eight involving the parotid gland and 25 the submandibular gland. Complete stone removal was achieved in 28 cases (85%), and symptom resolution was observed in 29 cases (88%). Postoperative infections occurred in five parotid and four submandibular cases. Temporary tongue paraesthesia was noted in two submandibular cases; no nerve injuries were reported in parotid cases.</p><p><strong>Conclusion: </strong>The combined approach is a safe and effective gland-preserving technique for large salivary gland stones, with high success rates and minimal complications. {"title":"Distinct gut microbiota profiles in acute vs. chronic sinusitis: potential biomarkers.","authors":"Hongwei Cao, Yan Fan, Hanbing Yu, Dongsheng Xing, Aihui Yan","doi":"10.1007/s00405-025-09714-0","DOIUrl":"https://doi.org/10.1007/s00405-025-09714-0","url":null,"abstract":"<p><strong>Purpose: </strong>This study explored the causal association between genetically predicted gut microbiota and sinusitis.</p><p><strong>Methods: </strong>This two-sample Mendelian randomization (MR) study used summary data from genome-wide association studies of gut microbiota (n = 211) and acute and chronic sinusitis. The main analysis was conducted using the inverse variance weighted (IVW) method as primary statistical analysis, with robustness tested using the weighted median, weighted mode, and MR-Egger regression methods. Sensitivity analyses included MR-Egger, MR-PRESSO, Cochran's Q, and leave-one-out for the detection of heterogeneity and pleiotropy.</p><p><strong>Results: </strong>Suggestive associations with acute sinusitis were identified for order Rhodospirillales (OR = 1.20, P = 0.01) and family Prevotellaceae (OR = 1.19, P = 0.02). Protective associations included genus Subdoligranulum (OR = 0.78, P = 0.01). For chronic sinusitis, significant associations were found for genus Coprobacter (OR = 1.23, P = 0.01), while protective associations included family Defluviitaleaceae (OR = 0.84, P = 0.04) and genus Tyzzerella3 (OR = 0.87, P = 0.011). Additional significant protective associations were observed across various other taxa, including order Burkholderiales and family Erysipelotrichaceae. However, none of these associations remained statistically significant after FDR correction. Sensitivity analyses supported the overall robustness of the findings.</p><p><strong>Conclusion: </strong>This MR study provides exploratory evidence for potential associations between gut microbiota and sinusitis subtypes. Although these findings did not remain significant after correction for multiple testing, they highlight distinct microbial patterns in acute versus chronic sinusitis and may serve as hypotheses for future research. {"title":"Considerations on multiportal compartmental approach extended to the temporal fossa.","authors":"Enrico Fazio, Luca Gazzini, Giulia Gobbo, Monir Abousiam, Giovanni Giorgetti, Aurel Nebiaj, Giuseppe Nucera, Alberto Schreiber, Luca Calabrese","doi":"10.1007/s00405-025-09674-5","DOIUrl":"https://doi.org/10.1007/s00405-025-09674-5","url":null,"abstract":"<p><strong>Purpose: </strong>To illustrate the applicability of compartmental surgery (CS) principles in the radical management of advanced oral cavity squamous cell carcinoma (OSCC) involving the high infratemporal fossa (ITF), in cases where neoadjuvant therapy is not feasible.</p><p><strong>Methods: </strong>We report a case of recurrent OSCC of the retromolar trigone with extensive involvement of the masticatory space, including the medial and lateral pterygoid, masseter, and temporalis muscles. A multiportal surgical approach was employed, integrating a transnasal endoscopic route with extended transoral and transcervical corridors. Surgical steps followed the CS concept, including en-bloc resection of tumour spread pathways and musculature insertions.</p><p><strong>Results: </strong>Radical resection including the involved masticatory musculature and temporalis muscle insertions was achieved. The final histopathology confirmed advanced-stage disease (rpT4b N3b R0 L1 V0 pn1 G2), with complete macroscopic and microscopic tumour removal.</p><p><strong>Conclusion: </strong>This case supports the feasibility and oncologic efficacy of a CS-based multiportal approach in selected advanced OSCC involving the ITF.
