While these programs seem to achieve their objective in the short run, their ability to provoke lasting changes remains to be demonstrated. The identification of granting mechanisms likely to maximize the incentive effect of these rewards should be based on knowledge about individual attitudes evidenced by behavioral economics. These points (1-32 points) are attributed after the publication of a scientific paper according to the quality of the journal (based on the impact factor) and the doctor's rank among the authors. These points are then transformed into a sum of money received by the health facility (1 point = approximately 648 euros), 4 years in a row. Ces études ont notamment permis le développement et l’utilisation en clinique de différentes immunothérapies, notamment l’utilisation d’inhibiteurs de points de contrôle immunitaire, qui ont révolutionné la prise en charge de certains cancers [1] (➜). Néanmoins, les voies de signalisation et les facteurs de transcription impliqués dans les réponses proet anti-tumorales restent imparfaitement définis. Historiquement, le facteur de transcription NF-κB (nuclear factor kappa-lightchain-enhancer of activated B cells) a été décrit comme un modulateur majeur des réponses immunitaires innées et adaptatives [2]. NF-κB est un complexe multimoléculaire constitué de plusieurs sousunités qui fonctionnent sous la forme de différents homoou hétérodimères : les sous-unités NF-κB1, RelA et c-Rel de la voie canonique ; et les sous-unités NF-κB2 et RelB de la voie alternative. {"title":"[Contraception in the context of PCOS].","authors":"Poli Mara Spritzer","doi":"10.1051/medsci/2022002","DOIUrl":"https://doi.org/10.1051/medsci/2022002","url":null,"abstract":"<p><p>Polycystic ovary syndrome is a frequent endocrinopathy, affecting between 8% and 13% of women of childbearing age and characterized by hyperandrogenism, chronic anovulation and polycystic ovary morphology. Women with PCOS also have a higher prevalence of obesity, metabolic disorders and an increased risk of diabetes, systemic hypertension and dyslipidemia. The first-line treatment for women with PCOS who do not plan to conceive in the short term includes lifestyle changes and combined oral contraceptives, offering, in addition to contraception, endometrial protection and reduction of hyperandrogenism. Progestin-only contraceptives are recommended for women with contraindications to estrogen contained in combined oral contraceptives. Cosmetic procedures can be added to pharmacological treatment for hirsutism. Severe cases may require anti-androgen drugs which will be combined with contraception. For overweight patients with cardiometabolic risk factors, including insulin resistance or dysglycemia, metformin may also be combined with contraception.
