Additionally, it seeks to assess the specific and general complications experienced by both groups.\u0000Methods: The study was conducted at the Department of Orthopedics, Government Medical College, and Rajindra Hospital in Patiala, spanning from March 2021 to December 2022. It was a prospective, manipulative, controlled study involving a total of 40 cases of tibia diaphyseal fractures that were presented to the orthopedics department. Fractures were classified according to the AO fracture classification.\u0000Results: The average time for union in the dynamic group was 15.60 weeks (with a standard deviation of 1.27). A significant statistical difference was observed, favoring the dynamization nailing group (p<0.01), indicating a strong trend toward faster union. Out of the 40 patients, 16 (40%) experienced at least one complication. In conclusion, dynamic IMN osteosynthesis permits micromotion between fracture fragments, directly stimulating bone formation and the development of callus.\u0000Conclusion: For closed or open tibial diaphyseal fractures with minimal comminution (types A and B based on the AO classification) up to Gustilo 3A, dynamic IMN assembly is considered a safe and effective treatment option.

Eighty pediatric age group patients diagnosed to be having anemia on clinical examination were included in this study. Demographic details such as age and gender were noted in all cases. A through history was obtained and a clinical examination was done. Presenting complaints were also noted. Complete blood count with peripheral blood smear examination was done in all cases. The Mentzer index was determined from complete blood count reports.\u0000Results: Out of 80 studied cases, there were 57 (71.25%) boys and 23 (28.75%) girls. The mean age for boys was 9.12±3.14 years and for girls, it was slightly higher at 10.24±3.46 years. Fatigue (73.75%), pallor (71.25%), and anorexia (56.25%) were common complaints. About 55% had mild anemia 22.5% had moderate anemia and 22.5% suffered from severe. Mean hemoglobin concentration of studied cases was found to be 8.42±3.98 g/dL. The most prevalent blood picture was the normocytic normochromic blood picture, representing 60% of cases. Mentzer index ranged from a minimum of 10.83 to a maximum of 27.76, with the mean value being 17.40±2.92. Notably, the vast majority of patients (97.5%) had a Mentzer Index >13, suggesting a high prevalence of iron deficiency. Conversely, only 2.5% of the patients had a Mentzer Index at or below 13, indicating a much smaller subset in whom Hb electrophoresis was needed to rule out thalassemia trait.\u0000Conclusion: Although most of the children with anemia are secondary to iron deficiency Mentzer index should be determined in all patients of anemia so as to avoid inadvertently prescribing iron supplementation in cases of thalassemia trait.

Cost-effective screening methods are a boon to patients. This study aims to detect gram-negative bacilli and their antibiotic sensitivity patterns, as well as detect the ESBL and AmpC-producing isolates among Gram-negative bacilli.\u0000Methods: A prospective study was conducted with 150 samples. Gram-negative bacilli were isolated, and their antibiotic sensitivity tests were performed by the Kirby–Bauer disk diffusion method. Potential ESBL producers were screened using Ceftazidime disc, and AmpC producers were screened by Cefoxitin discs by the disc diffusion method. ESBL producers were confirmed by the combined disc diffusion assay method using ceftazidime and ceftazidime/Clavulanic acid disc. AmpC producers were confirmed by the Cefoxitin Cloxacillin Double Disc Synergy Test.\u0000Results: About 38% of 150 samples were gram-negative bacilli, of which 40.35% were Escherichia coli, followed by Pseudomonas aeruginosa (35.08%). Maximum sensitivity by E. coli was found toward imipenem, meropenem, and cotrimoxazole. P. aeruginosa showed maximum sensitivity toward piperacillin/tazobactam, imipenem, meropenem, and ceftazidime. 28.07% of Gram-negative isolates were ESBL producers, with E. coli (11 isolates) being the maximum, and 15.78% were AmpC producers, with E. coli (four isolates) being the maximum. Seven isolates were both ESBL and AmpC producers.\u0000Conclusion: Routine screening and timely reporting of ESBL and AmpC producers help in preventing the spread of multidrug-resistant strains. It is among the most common cause of community-acquired pneumonia in patients with underlying medical conditions, such as diabetes and hypertension. In this case, the patient’s pneumonia did not respond to empiric antibiotic treatment with ceftriaxone and azithromycin. However, his symptoms resolved after treatment with levofloxacin. This case report highlights the importance of considering S. maltophilia in the differential diagnosis of community-acquired pneumonia in patients with underlying medical conditions. Eighty patients infected with hepatitis B (on the basis of serological markers) were included in this study on the basis of predefined inclusion and exclusion criteria. Demographic details such as age, gender, and socioeconomic status of all the patients were noted. A detailed history was taken including any history of intravenous drug use, tattooing, sexual contacts, history of blood transfusions, and familial liver disease. Clinical and laboratory profiles (liver function test and serological markers) of all the patients were studied. Ultrasound imaging was done in all the cases. p<0.05 was taken as statistically significant.\u0000Results: Out of 80 studied cases, there were 49 (61.25%) males and 31 (38.75%) females. The mean age for males and females was 32.30±8.03 and 34.70±13.52 years, respectively. There was no significant difference in mean age between males and females (p=0.3218). Mother-to-child transmission was evenly distributed with 7 males (8.75%) and 9 females (11.25%) affected. Other risk factors, such as dental implants and surgeries, showed a lower prevalence, with dental implants impacting 2 males (2.5%) and 3 females (3.75%) and surgeries concerning 5 males (6.25%) and 3 females (3.75%). Majority of the patients (52.5%) were asymptomatic and were found to be hepatitis B surface antigen (HBsAg) positive incidentally. HBsAg was positive in all cases. 12 (15%) patients showed the presence of hepatitis B e antigen suggestive of high infectivity. Anti-HBc and anti-HBe were positive in 68 (85%) patients each. Hepatomegaly was seen in 23 cases (28.75%) on ultrasound. Moderate fibrosis on elastography was seen in 2 (2.5%) cases.\u0000Conclusion: In cases of hepatitis B, the majority of the patients remained asymptomatic, and many cases no risk factor for hepatitis B virus infection could be identified.
