The histology of this type of tumor varies from benign to malignant. Immunohistochemical markers (IHC) help in differentiating and grading of tumor. In this study we encountered 3 different types of spindle cell tumor in pediatric age group in 1 year. The first case was low grade fibro myxoid sarcoma in posterior triangle of neck. The second case was fibromatosis coli which was managed surgically. Third case was solitary plexiform neurofibroma in deep lobe of parotid gland which was also treated surgically. The preoperatively diagnosis of these spindle cell tumors was challenging despite adequate and relevant investigations. An adequate post operative analgesia is essential after tonsillectomy as pain following tonsillectomy impairs swallowing with a risk of dehydration, infection and secondary haemorrhage. Usage of mouth gargle in postoperative period is found to have significant role in controlling pain. The aim of this study is to analyse and compare effectiveness of Benzydamine and Povidone Iodine gargle on post tonsillectomy pain. The study group consists of total 60 patients who underwent tonsillectomy only by dissection and snare method in two hospitals attached to the same institute during the mentioned time period by more than one operating surgeon. All patients received the same analgesics and antibiotics during the post operative period, no steroids were given. Those patients for which diathermy or other methods of haemostasis was used were not included in the study population. Post tonsillectomy pain for 9 days was compared between two study groups of those using Benzydamine gargle [group 1] and Povidone Iodine gargle [group 2]with the help of visual analogue scale (VAS) with each group comprising of 30 patients. Our study showed homogenous distribution of age and sex among the patients as well as comparable grades of tonsil in both the study groups. The mean VAS score was 6.20 and 6.87 on post operative day 1 wheras 2.63 and 5.47on post operative day 9 in group 1 and 2 respectively. According to this study better control over post tonsillectomy pain was seen in the study group who were on Benzydamine mouth gargle when compared to Povidone Iodine. The study results were compared to various similar studies and it turned out that Benzydamine was found to be more effective than other local analgesics and Povidone Iodine. One of the studies showed improvement in post operative pain using Povidone Iodine also, but in general it was found that use of analgesic mouth gargle in post tonsillectomy helped better to tackle the pain and thereby increase the quality of life in the post operative period. These cysts arise from embryonal thymic tissue along its descent path, with mediastinal extension seen in approximately 50% of cases. Due to their rarity, thymic cysts are often misdiagnosed as branchial or lymphangiomas. A 10-year-old boy presented with a progressively enlarging, painless neck mass. Imaging studies, including ultrasound and CT scan, revealed a fluid-density lesion in the parapharyngeal space extending to the mediastinum. Fine needle aspiration cytology initially suggested an infected branchial cyst. Surgical excision was performed, revealing a 9.5 × 2.5 × 1.5 cm cystic mass. Histopathology confirmed the diagnosis of a cervical thymic cyst, with features including Hassall's corpuscles and normal thymic tissue. Thymic cysts, often misdiagnosed due to their rarity, should be considered in the differential diagnosis of lateral neck masses in children. While typically found on the left side, they can occur elsewhere. Imaging is crucial for preoperative diagnosis, but surgery poses challenges due to the cyst's proximity to vital structures and due to mediastinal extension. Complete excision is advised; however, in young children, if mediastinal thymic tissue is absent, total removal should be avoided to prevent immune dysfunction. The case series is a retrospective analysis of patients in a single centre and highlights the surge of pulmonary involvement in patients with sinonasal mucormycosis. To determine incidence of sequential pulmonary involvement in diagnosed cases of sino-nasal mucormycosis undergoing treatment in a tertiary health care facility and devise institutional algorithm for management. Records of patients diagnosed with sino-nasal mucormycosis who underwent treatment in tertiary care health facility from January 2023 to January 2025 were reviewed in view of simultaneous pulmonary involvement. We found four patients during the time duration of two years with simultaneous sino-nasal and pulmonary mucormycosis. Diabetes mellitus was most common predisposing factor in all patients with mean glycosylated haemoglobin of 10.9%. Sputum microscopy for KOH mount was negative in all four patients. One of the patients underwent computed tomography guided biopsy from right lung lesion after the onset of pulmonary symptoms. Histo-morphological features were consistent with clinical diagnosis of mucormycosis. Detection of simultaneous pulmonary involvement in sino-nasal mucormycosis is an arduous task. Timely detection of non-specific symptoms of pulmonary involvement is responsible for favourable prognosis. While it commonly affects the head and neck region among extracardiac sites, solitary lingual adult rhabdomyomas (ARs) are extremely uncommon, with few reported cases in the literature. We present a 39-year-old male who presented with a 2-month history of globus sensation, dysphonia, shortness of breath, snoring and apneic episodes at night prompting evaluation in the ENT OPD. Fine needle aspiration cytology (FNAC) of the lingual swelling revealed rhabdomyoma. The patient underwent successful complete endoscopy assisted intraoral excision of the lesion, underscoring the importance of meticulous surgical management for large rhabdomyomas. Prospective, Observational study was conducted at two hospitals - a medical college hospital and a private hospital. Financial revenue generated from both hospitals was computed for a 1-year period. Historic 3-year arrival rates of patients against the name of surgery performed were collected from the statistics department of both hospitals. From this new day care unit feasible number of patients in 1 year period were projected. Personal identifiable information or health information about patients was not collected. Medical College hospitals cater to public health insurance patients from government. The package prices would be lower in government insurance schemes, but the volume of patient arrivals will be high. This large number of arrivals would be essential for maintaining bed occupancy rates, which is ideal for student training. Private hospitals receive more volume of out-of-pocket patients; hence a comparative study is useful. The net operating margin for a large hospital stands at 45% and for a small hospital it stands at 25% per annum, without considering the capital infrastructure costs. Administrators of hospitals can strategize towards achieving profitability. Histopathologically, it comprises of granulation tissue with cholesterol crystals, multinucleated giant cells, and hemosiderin deposits from erythrocyte breakdown. While commonly found in the petrous apex, tympanomastoid cholesterol granulomas are rare, with very few cases reported in literature. It can mimic other otological conditions, leading to diagnostic challenges. We report a rare case of cholesterol granuloma presenting as acute mastoiditis in a 24-year-old female with type 1 diabetes. She presented with postauricular swelling, pain, fever, and hearing loss. Imaging revealed a cystic lesion with bony erosion in the mastoid antrum. Surgical exploration uncovered a lobulated cyst containing thick fluid, which was managed via marsupialization and reconstruction. Histopathology confirmed the diagnosis of cholesterol granuloma. They are usually asymptomatic when small in size, but can present with symptoms when the size is larger and involving oropharynx, hypopharynx or larynx. A 35 yr/M patient presented with bilobed lipoma meauring approximately 7.3 × 4.1 × 2.9 cm arising from supraglottic region extending till hypopharynx. Excision of lipoma was performed via Trans oral robotic surgery with minimal to none complications. Historically, lipomas of oro-laryngopharynx have been surgically excised using cervical, endoscopic or lateral pharyngotomy. High-grade mucoepidermoid carcinoma (MEC) can share histological similarities with SCC, making their distinction challenging. SCC is characterized by frank anaplasia and evident keratinization, serving as distinguishing features from high-grade MEC. However, in incisional biopsies lacking these characteristics, differentiation becomes problematic, as immunohistochemical findings tend to be similar. Literature indicates that MUC5AC is present in MEC but notably absent in SCC. Thus, this study aims to evaluate MUC5AC expression in both OSCC and MEC and its role as a diagnostic immunomarker for these two entities. A comprehensive literature review revealed a scarcity of studies investigating MUC5AC expression in OSCC. Therefore, the current study seeks to investigate the immunoexpression of MUC5AC in OSCC and compare it with MEC. This study includes total of 40 cases, 20 cases of OSCC and 20 cases of MEC. Immunohistochemical analysis was performed to assess MUC5AC expression. The intensity and localization of immunoexpression were assessed in all cases. 95% MECs cases were immunopositive for MUC5AC. Moderate to strong cytoplasmic immunoreactivity for MUC5AC was noted, particularly in mucous cells present within the tissue section. In contrast, none of the 20 OSCC cases exhibited immunopositivity for MUC5AC.
