Advances in prenatal diagnostics have enabled earlier identification of these conditions, creating opportunities for prenatal therapeutic intervention. Preclinical studies have demonstrated that prenatal cell therapies can result in donor cell engraftment, enzyme production, and partial correction of metabolic defects in several disease models. The fetal environment may support immune tolerance and enhance treatment effectiveness. Limited clinical experience suggests that these approaches are feasible and may be beneficial in selected conditions, including lysosomal storage diseases and infantile-onset Pompe disease. In utero gene therapy has also shown long-term metabolic correction in animal models. However, important challenges remain, including limited engraftment, immune responses, vector safety, ethical concerns, and regulatory barriers. Prenatal cellular and gene-based therapies are promising but remain experimental. Current evidence is strongest for biological rationale and preclinical proof of concept; clinical benefit has not yet been established for most disorders. Further clinical studies are needed to determine their safety, efficacy, and future role in the treatment of congenital metabolic disorders. Despite the high global frequency of this procedure, evidence regarding clinically significant postoperative changes in fat-soluble vitamin status remains inconsistent. This retrospective single-center study compared preoperative and postoperative serum 25-hydroxyvitamin D (25[OH]D) levels and vitamin K-dependent coagulation profiles in patients undergoing cholecystectomy. MATERIAL AND METHODS All data were retrieved from electronic medical records of patients who underwent cholecystectomy between January 2020 and January 2025. Two distinct cohorts were formed: the vitamin K cohort (n=101), in which prothrombin time (PT), activated partial thromboplastin time (aPTT), and international normalized ratio (INR) were evaluated at ≥7 days postoperatively; and the 25(OH)D cohort (n=78), in which serum 25(OH)D levels were evaluated at ≥3 months postoperatively. Patients with comorbidities or medications affecting these parameters were excluded. RESULTS In the 25(OH)D cohort, no significant difference was found between preoperative and postoperative median levels (19.76 vs 19.13 µg/L, P=0.248). In the vitamin K cohort, no significant changes were detected in PT (11.80 vs 11.67 s, P=0.301), aPTT (29.83 vs 30.40 s, P=0.131), or INR (1.03 vs 1.03, P=0.859). Sex-stratified analyses revealed no significant differences. CONCLUSIONS Cholecystectomy was not associated with a clinically significant decline in serum 25(OH)D or alterations in vitamin K-dependent coagulation profiles. This study evaluated the efficacy and safety of combining celecoxib with Jintiange capsules, a synthetic tiger bone formulation used in traditional Chinese medicine (TCM), to manage symptomatic KOA. MATERIAL AND METHODS This 12-week, randomized, double-blind, placebo-controlled trial enrolled 120 patients with KOA (age 40 years; visual analog scale [VAS] score 4-7). Participants received celecoxib (200 mg/day tapered to 100 mg/day) combined with either Jintiange capsules (3.6 g/day) or placebo. The primary outcome was the change in the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) total score. Secondary outcomes included WOMAC subscale scores (pain, function, and stiffness), VAS scores, and adverse drug reaction reports. RESULTS The experimental group demonstrated a 23.5-point (37.8%) reduction in WOMAC total score from baseline (P<0.001); the control group showed a 13.2-point (21.6%) reduction (P<0.001). The experimental group achieved 51.6% improvement in the WOMAC pain subscale (P<0.001), whereas the control group showed 33.3% improvement (P<0.001). Relative to the control group, the experimental group demonstrated a greater reduction in VAS score (P<0.001). Combined TCM-NSAID therapy produced significantly greater pain relief and functional improvement than NSAID monotherapy. Attempts to develop safer steroids gave rise to selective glucocorticoid receptor agonists and modulators (SEGRAMs), initially based on the premise that anti-inflammatory efficacy might be preserved while signaling linked to adverse effects was reduced. Current glucocorticoid receptor biology no longer supports such a simple binary model. Glucocorticoid receptor output is shaped by ligand-induced conformation, DNA sequence, chromatin accessibility, cofactor recruitment, and cell-specific transcriptional context, making selective modulation a context-dependent pharmacologic phenomenon rather than a fixed molecular property. In this clinically oriented narrative review, we reassess the SEGRAMs concept from receptor biology to translational and early clinical evidence, with explicit distinction between mechanistic dissociation, translational separation of efficacy from selected toxicities, and clinically meaningful improvement in the therapeutic index. Representative compounds have shown that selective glucocorticoid receptor modulation is pharmacologically feasible and, in some settings, may separate anti-inflammatory activity from selected steroid-related liabilities. However, the available evidence also indicates that any potential advantage is disease-specific, toxicity-specific, and exposure-dependent, and that mechanistic dissociation does not by itself justify therapeutic claims, particularly in the absence of long-term comparative outcome data. The most credible future for SEGRAMs lies not in universal replacement of conventional glucocorticoids, but in their potential to improve the therapeutic index in diseases for which prolonged or repeated glucocorticoid exposure remains necessary, but in which cumulative harm from toxicity is a major treatment burden.</p>","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e953742"},"PeriodicalIF":2.5,"publicationDate":"2026-09-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13587748/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148971899","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Sodium-glucose cotransporter 2 inhibitors (SGLT2i) provide cardiorenal protection beyond glucose lowering via anti-inflammatory, metabolic, and endothelial effects, but their effect on perioperative liver-related risk markers, including the Model for End-Stage Liver Disease (MELD) score, in off-pump coronary artery bypass grafting (OPCAB) remains uncertain. MATERIAL AND METHODS We retrospectively studied 111 consecutive patients undergoing elective OPCAB in 2024 (chronic hepatic or renal disease excluded). Patients were stratified by long-term preoperative SGLT2i therapy (≥3 months): no SGLT2i (n = 85) vs SGLT2i (n = 26). MELD score, bilirubin, international normalized ratio , and creatinine were assessed preoperatively and on postoperative days 1 and 6. Between-group comparisons used the Mann-Whitney U test; within-group changes were analyzed using the Friedman test, Kendall's W, and Bonferroni post hoc testing RESULTS Baseline MELD and components were comparable. On postoperative day 1, SGLT2i group exhibited lower bilirubin (P < 0.001), INR (P = 0.04), and MELD score (P = 0.03), but higher creatinine (P = 0.002). By day 6, differences attenuated. Within-group analysis showed significant time effects for MELD, INR, and creatinine in both groups, with large effect sizes (W = 0.48-0.88). Total bilirubin changed over time in the control but not the SGLT2i group. CONCLUSIONS Long-term preoperative SGLT2i therapy was associated with distinct perioperative MELD dynamics in OPCAB, with relative preservation of bilirubin and INR despite creatinine changes, suggesting the potential modulation of hepatic function during perioperative stress. Dual-phase contrast-enhanced computed tomography (CT) has potential advantages in visualizing tumor morphology and vascular characteristics; its diagnostic performance concerning pathological findings has not been fully elucidated. MATERIAL AND METHODS This prospective study included 120 patients with suspected peripheral lung cancer. All patients underwent dual-phase contrast-enhanced CT scanning; histopathological examination of biopsy or surgically resected specimens served as the reference standard. Patients were divided into 2 groups: lung cancer (n=81) and control (n=39). CT imaging features were compared between groups. The diagnostic performance of dual-phase contrast-enhanced CT was evaluated by calculating sensitivity, specificity, accuracy, positive likelihood ratio (PLR), and negative likelihood ratio (NLR), along with 95% confidence intervals. RESULTS Dual-phase contrast-enhanced CT demonstrated a sensitivity of 82.72%, specificity of 74.36%, accuracy of 80.00%, PLR of 3.23, and NLR of 0.23 for diagnosis of peripheral lung cancer. Histopathological examination confirmed 81 cases of peripheral lung cancer and 39 benign lesions. Peripheral lung cancer lesions-more frequently located in the right lung-were characterized by irregular shape, blurred margins, and spiculated margins. The incidences of vacuoles, contrast enhancement, vascular invasion, and lymph node metastasis were significantly higher in the lung cancer group than in the control group. The association between platelet transfusion and short-term outcomes in this population remains unclear. This study evaluated the association between platelet transfusion and ICU mortality within 28 days. MATERIAL AND METHODS We analyzed 936 neuro-ICU patients with intracranial hemorrhage complicated by thrombocytopenia, including 173 who received platelet transfusions and 763 who did not. After propensity score matching (PSM), 113 matched pairs were obtained. Cox proportional hazards models, Kaplan-Meier analysis, restricted cubic spline analysis, and exploratory subgroup analyses were used to assess the associations of platelet transfusion status and transfusion volume with ICU mortality within 28 days. RESULTS After PSM, ICU mortality within 28 days did not significantly differ between the transfusion and non-transfusion groups (P=0.292), nor did survival time (P=0.576). Cox regression and Kaplan-Meier analyses showed no significant association between platelet transfusion status and mortality (all P>0.05). Exploratory subgroup analyses suggested heterogeneity among patients with pneumonia (P=0.007, interaction P=0.010) and heart failure (P=0.074, interaction P=0.043). Higher transfusion volume was associated with higher mortality in unadjusted analyses; this association was not significant after multivariable adjustment.
