Although vitamin D has been associated with pulmonary outcomes in other populations, the association between serum 25-hydroxyvitamin D [25(OH)D] status and pulmonary manifestations, particularly atelectasis, in CVID remains unclear. MATERIAL AND METHODS We conducted a retrospective cross-sectional study of 48 adult patients with CVID stratified by serum 25(OH)D concentration (<30 ng/mL [insufficient] vs ≥30 ng/mL [sufficient]). Pulmonary outcomes (atelectasis, bronchiectasis, recurrent pneumonia, and chronic obstructive pulmonary disease [COPD]) were obtained from medical records. Associations were assessed via Fisher's exact test. Exploratory analyses included a longitudinal imaging review and descriptive Kaplan-Meier curves illustrating atelectasis-free disease duration by baseline 25(OH)D status. RESULTS Among 48 patients, 35 (73%) had 25(OH)D concentrations below 30 ng/mL. Atelectasis was more frequent in the insufficient group (15/35; 43%) (odds ratio=0.12; 95% confidence interval, 0.01-0.97; P=0.02). No significant associations were observed for bronchiectasis (63% vs 46%; P=0.53), recurrent pneumonia (91% vs 84%; P=0.49), or COPD (3% vs 8%; P=0.46). Kaplan-Meier curves did not show an apparent separation between groups. CONCLUSIONS Serum 25(OH)D sufficiency was associated with lower atelectasis prevalence in patients with CVID. This finding reflects a cross-sectional association and does not establish causality; it supports a potential link between 25(OH)D status and atelectasis. Prospective studies are needed to further evaluate this relationship.</p>","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e953313"},"PeriodicalIF":2.5,"publicationDate":"2026-09-18","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13596011/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149002722","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This narrative review covers both patient-facing VR applications (preoperative education, pain and anxiety relief, vestibular rehabilitation, swallowing assessment and therapy, hearing rehabilitation, and tinnitus management) and nurse-facing VR applications (professional training and skills development). By synthesizing recent clinical studies and technological advances, this review highlights the potential for VR to enhance the patient care experience, improve nursing quality and efficiency, and address common challenges such as severe surgical anxiety and poor adherence to rehabilitation; supporting evidence varies across applications. Vestibular rehabilitation is supported by strong evidence from randomized controlled trials, whereas swallowing therapy and tinnitus management are primarily supported by preliminary exploratory studies. We discuss technical, economic, and clinical limitations hindering implementation of VR, then present practical recommendations for future research and clinical practice. This review also highlights recent advances, including the integration of VR with artificial intelligence, wearable biosensors, and telerehabilitation platforms, to facilitate personalized remote otolaryngology nursing. Despite the potential for VR to improve rehabilitation outcomes and treatment adherence, current evidence remains heterogeneous due to variations in study designs and outcome measures. Rigorous, standardized, large-scale studies are urgently needed to strengthen the evidence base and support the reliable, standardized implementation of VR in otolaryngology nursing. Nonunion or disruption at this level may compromise distal radioulnar joint (DRUJ) stability, although the clinical significance of associated USFs in distal radius fractures (DRFs) remains debated. This retrospective cohort study compared outcomes after mini-anchor fixation vs no ulnar styloid fixation in patients with DRFs and concomitant Hauck type II USFs. MATERIAL AND METHODS Forty-five patients treated between 2021 and 2023 were included. Twenty-three patients underwent mini-anchor fixation of the ulnar styloid (group A), and 22 did not (group C). Primary outcomes were USF union and ulnar-sided or DRUJ stability-related complications. Secondary outcomes included wrist range of motion, grip strength, the Modified Mayo Wrist Score, and the Quick Disabilities of the Arm, Shoulder, and Hand score. RESULTS All DRFs healed in both groups. USF union occurred in all patients in group A, whereas 2 nonunions occurred in group C (P=0.233). Ulnar-sided pain was less frequent in group A than in group C (P=0.346), and DRUJ subluxation occurred only in group C (P=0.489). Range of motion, grip strength, and functional outcome scores were comparable between groups. CONCLUSIONS Mini-anchor fixation was associated with higher union rates and fewer ulnar-sided or DRUJ-related complications, although differences were not statistically significant, and overall wrist function was similar between groups. This study evaluated 24-month outcomes of CRFA alone versus CRFA combined with intra-articular botulinum toxin type A (BoNT-A). MATERIAL AND METHODS In this retrospective observational comparative-effectiveness cohort study, 90 patients with moderate-to-severe knee OA underwent CRFA alone (n = 50) or CRFA plus intra-articular BoNT-A (n=40). The combination group received BoNT-A at baseline and a planned repeat injection at ~12 months. Pain and function were assessed using the Numerical Rating Scale (NRS) and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). The primary durability endpoint was the between-group difference in longitudinal NRS trajectories over 24 months, assessed using adjusted mixed-effects modeling. RESULTS Both groups demonstrated substantial early improvement; outcome trajectories diverged after ~12 months. Significant treatment-by-time interactions were observed for NRS (ß=-0.84, 95% confidence interval [CI] -1.21 to -0.47; P=0.0002) and WOMAC outcomes (ß=-6.3, 95% CI -9.4 to -3.2; P=0.0003). At 24 months, mean observed NRS scores were 4.2±2.3 after CRFA alone and 1.8±0.9 after CRFA plus BoNT-A; responder rates were 51.1% and 86.5%, respectively (P=0.0004). CONCLUSIONS In this retrospective observational cohort, a longitudinal treatment pathway combining CRFA with intra-articular BoNT-A was associated with more sustained pain relief and functional improvement over 24 months than CRFA alone. We evaluated short-term cuff pressure changes after repositioning according to measurement time, patient position, sedation level, and body mass index. MATERIAL AND METHODS This prospective observational study included 103 mechanically ventilated adult ICU patients. Before repositioning, cuff pressure was adjusted to 20 to 30 cm H₂O when necessary, and the resulting value was recorded as T0. Patients were placed in the head-elevated supine, right lateral, or left lateral position. Cuff pressure was remeasured at 1 (T1) and 5 (T2) minutes after repositioning. Repeated measurements obtained during routine positioning cycles over a 2-day period were analyzed via mixed-effects models. RESULTS In total, 4944 cuff pressure measurements were analyzed. Cuff pressure significantly increased over time, from 27.67±2.27 cm H₂O at T0 to 28.11±2.89 cm H₂O at T1 and 28.29±3.23 cm H₂O at T2 (P<0.001); the absolute magnitude of change was small. No significant differences were detected among target positions (P>0.05). Higher Richmond Agitation-Sedation Scale (RASS) scores were independently associated with increased cuff pressure (ß=0.94 cm H2₂ per 1-point increase; P<0.001) and higher odds of out-of-range cuff pressure (odds ratio, 7.21; P<0.001). CONCLUSIONS Endotracheal tube cuff pressure showed statistically significant short-term changes after repositioning. Higher RASS scores were associated with greater cuff pressure variability and higher odds of out-of-range cuff pressure. This study assessed the safety and efficacy of phacoemulsification combined with iStent inject implantation (phaco-iStent) vs goniotomy using the Kahook Dual Blade (phaco-KDB) in patients with open-angle glaucoma or ocular hypertension. MATERIAL AND METHODS This prospective randomized trial included 68 eyes (35 phaco-iStent, 33 phaco-KDB) operated on by a single surgeon between 2024 and 2025. Surgical success was defined as IOP reduction ≥20% from baseline or IOP ≤15 mm Hg, with subcategories of complete and satisfactory success. Postoperative assessments were performed up to 12 months, with mean follow-up duration of 8±2.4 months. RESULTS Both procedures led to a similar reduction in IOP (P=0.355 at 6 months). Mean IOP decreased from 19.37 ± 4.03 mm Hg to 16.58±3.64 mm Hg in the phaco-iStent group and from 18.15±3.76 mm Hg to 15.46±3.15 mm Hg in the phaco-KDB group (approximately 14% reduction in both groups). The mean number of glaucoma medications decreased from 1.29±0.71 to 0.00 ± 0.00 in the phaco-iStent group and from 1.00±0.61 to 0.00±0.00 in the phaco-KDB group. Complete surgical success at 6 months was achieved in 68.8% of phaco-iStent eyes and 62.5% of phaco-KDB eyes. The most common adverse events were corneal edema and hyphema, which occurred more frequently in the phaco-KDB group; all resolved without additional surgical intervention. CONCLUSIONS Both the phaco-iStent and the phaco-KDB techniques significantly reduced IOP and medication burden with few postoperative complications. Advances in prenatal diagnostics have enabled earlier identification of these conditions, creating opportunities for prenatal therapeutic intervention. Preclinical studies have demonstrated that prenatal cell therapies can result in donor cell engraftment, enzyme production, and partial correction of metabolic defects in several disease models. The fetal environment may support immune tolerance and enhance treatment effectiveness. Limited clinical experience suggests that these approaches are feasible and may be beneficial in selected conditions, including lysosomal storage diseases and infantile-onset Pompe disease. In utero gene therapy has also shown long-term metabolic correction in animal models. However, important challenges remain, including limited engraftment, immune responses, vector safety, ethical concerns, and regulatory barriers. Prenatal cellular and gene-based therapies are promising but remain experimental. This retrospective single-center study compared preoperative and postoperative serum 25-hydroxyvitamin D (25[OH]D) levels and vitamin K-dependent coagulation profiles in patients undergoing cholecystectomy. MATERIAL AND METHODS All data were retrieved from electronic medical records of patients who underwent cholecystectomy between January 2020 and January 2025. Two distinct cohorts were formed: the vitamin K cohort (n=101), in which prothrombin time (PT), activated partial thromboplastin time (aPTT), and international normalized ratio (INR) were evaluated at ≥7 days postoperatively; and the 25(OH)D cohort (n=78), in which serum 25(OH)D levels were evaluated at ≥3 months postoperatively. Patients with comorbidities or medications affecting these parameters were excluded. RESULTS In the 25(OH)D cohort, no significant difference was found between preoperative and postoperative median levels (19.76 vs 19.13 µg/L, P=0.248). In the vitamin K cohort, no significant changes were detected in PT (11.80 vs 11.67 s, P=0.301), aPTT (29.83 vs 30.40 s, P=0.131), or INR (1.03 vs 1.03, P=0.859). Participants received celecoxib (200 mg/day tapered to 100 mg/day) combined with either Jintiange capsules (3.6 g/day) or placebo. The primary outcome was the change in the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) total score. Secondary outcomes included WOMAC subscale scores (pain, function, and stiffness), VAS scores, and adverse drug reaction reports. RESULTS The experimental group demonstrated a 23.5-point (37.8%) reduction in WOMAC total score from baseline (P<0.001); the control group showed a 13.2-point (21.6%) reduction (P<0.001). The experimental group achieved 51.6% improvement in the WOMAC pain subscale (P<0.001), whereas the control group showed 33.3% improvement (P<0.001). Relative to the control group, the experimental group demonstrated a greater reduction in VAS score (P<0.001). Combined TCM-NSAID therapy produced significantly greater pain relief and functional improvement than NSAID monotherapy. CONCLUSIONS Celecoxib combined with Jintiange capsules provided clinically meaningful improvements in pain relief and functional outcomes relative to celecoxib monotherapy in patients with KOA. These findings support integration of TCM with conventional pharmacotherapy for osteoarthritis management.</p>","PeriodicalId":48888,"journal":{"name":"Medical Science Monitor","volume":"32 ","pages":"e951501"},"PeriodicalIF":2.5,"publicationDate":"2026-09-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13587749/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148963820","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
