She was treated with Zhu's scalp<span> acupuncture comprising one needle in the head-face area, two needles in the upper-</span></span><em>jiāo</em> area, three needles in the lower-<em>jiāo</em> area, and one needle puncturing the <em>Bǎihuì</em> (百会GV20). After <em>dǎoyǐn</em> (physical and breathing exercises), three needles were added to the auricular-temporal area, and the left Tīnggōng (听宫SI19), Tīnghuì (听会GB2), and Yīfēng (翳风TE17) were punctured. <em>Dǎoyǐn</em><span> was performed during each episode of needle puncture. After two sessions of scalp acupuncture treatment, the pure tone audiometry indicated that her hearing was normal. During the treatment, one patient in the medication group was excluded and 20 patients were included in final statistical analysis; while 21 patients completed the trial in the acupuncture group. Before treatment, after treatment and in follow-up (1 month after the whole treatment), the scores of the Cleveland clinical constipation scoring system (CCS), the self-rating anxiety scale (SAS), and the self-rating depression scale (SDS) were assessed in both groups. The changes in NPY content were compared before and after treatment.</span></p></div><div><h3>Results</h3><p>① Total score of CCS: after treatment, the total score in the acupuncture group and the medication group were (4.76 ± 2.10) points and (5.60 ± 1.88) points, respectively, which were significantly lower than those before treatment in both groups (both <em>P</em> < 0.05); and the difference in the score was not significant for the between-group comparison (<em>P</em> > 0.05). In the follow-up, the total scores were (4.19 ± 1.69) points in the acupuncture group, and was (6.35 ± 2.06) points in the medication group. The score in the medication group was higher than that after treatment in the same group (<em>P</em> < 0.05), but there was no significant difference between acupuncture group and the medication group (<em>P</em> > 0.05). ② SAS score: after treatment, the SAS scores were (50.24 ± 2.19) points and (53.45 ± 2.46) points in the acupuncture group and the medication group, respectively, which was lower than that before treatment in the two groups (both <em>P</em> < 0.05); and the difference in the score was significant between two groups (<em>P</em> < 0.05). In the follow-up, the scores were (51.95 ± 2.27) points and (54.55 ± 1.67) points in the acupuncture group and the medication group, respectively; which was lower than that before treatment in the two groups (both <em>P</em> < 0.05), and the difference in the score was significant between two groups (<em>P</em> < 0.05). ③SDS score: after treatment, the SDS scores were (50.71 ± 2.70) points and (53.15 ± 2.25) points in the acupuncture group and the medication group, respectively, which was significantly lower than that before treatment in the two groups (both <em>P</em> < 0.05); and the difference in the score was significant between the two group (<em>P</em> < 0.05). However, there are few bibliometric analysis-based studies concerning the overall trends and characteristics in acupuncture therapy for cancer management. This study aims to summarize the status and bibliometric characteristics of acupuncture as an intervention for cancer-related symptoms.</p></div><div><h3>Methods</h3><p>All clinical research papers on acupuncture for the management of cancer-related symptoms published up to October 1, 2020 were collected from the PubMed and Web of Science databases. Data on the included studies were extracted to reflect the following items: publication year, journal, country or region, institution, clinical indications, and acupuncture prescriptions. Characteristics were analyzed using bibliometric methods.</p></div><div><h3>Results</h3><p>A total of 226 papers were included. The number of such publications has increased steadily since 2004. The 226 articles were published in 94 English journals, with <em>Integrative Cancer Therapies</em> (18 articles) and <em>Acupuncture in Medicine</em><span> (14 articles) being the most prolific. The included studies are distributed across 20 countries and regions, among which the top two are the United States of America (68 articles) and China (60 articles). The types of cancer the studies addressed include breast, gastric, head and neck, etc. Acupuncture is mainly employed in the treatment of complications<span><span><span> and side effects, among which nausea and vomiting, pain, hot flashes, and fatigue are the most frequently treated cancer-related symptoms. Acupuncture is the most commonly used </span>treatment therapy, and the four most frequently used </span>acupoints<span> used are Neiguan (内关 PC6), Sanyinjiao (三阴交 SP6), Zusanli (足三里 ST36), and Hegu (合谷 LI4). The specific acupuncture prescription depends on individual patients’ symptoms.</span></span></span></p></div><div><h3>Conclusion</h3><p>Research on acupuncture as an adjunctive therapy for cancer-related symptoms is developing steadily, with results being reported in a growing number of mainstream medical journals. Based on the complex network approach, the acupoint-symptom network was established and changes based on the acupoint number were analyzed to highlight the agreement and congruence between these two textbooks, including indication rules. Using topological data, the reasons for the differences were explained to provide specific ideas and methods for the structural development and standardization of the traditional acupuncture knowledge system. In this study, 386 acupoints, 773 symptoms, and 152,163 acupoint-matched pairs from <em>New Acupuncture</em> and 403 acupoints, 253 symptoms, and 28,755 acupoint-matched pairs from <em>Meridians and Acupoints</em> were included. The results highlighted that there are numerous differences in the acupoint-symptom network between these two textbooks. The model of <em>New Acupuncture</em> displays a more typical small-world effect, which may reflect the advantage of classification by symptoms. Acupoints<span> included Băihuì (百会BL20), Sìshéncōng (四神聪EX-HN1), Fēngfŭ (风府GV16), Ānmián (安眠EX-HN18), Shéndào (神道GV11), Gānshū(肝俞BL18), Píshū (脾俞BL20), Shènshū (肾俞BL23), Shénmén (神门HT7) and Sānyīnjiāo (三阴交SP6). Acupuncture was performed once every 2 days, three times weekly, and the needles were retained for 30 min each time. The duration of treatment was 4 weeks. The </span></span>Pittsburgh Sleep Quality Index (PSQI) score and HPA- and HPO-related hormones were evaluated before and after treatment.</span></p></div><div><h3>Results</h3><p>The PSQI score was lower after treatment than before treatment. The score of each item (subjective sleep quality, sleep latency, sleep maintenance, habitual sleep efficiency, sleep disturbance, sleep medication, and daytime dysfunction) was also lower after treatment than before treatment. The levels of HPA-related hormones (cortisol, adrenocorticotrophin, and corticotropin-releasing hormone) and HPO-related hormones (follicle-stimulating hormone and luteinizing hormone) all decreased after treatment. In contrast, the level of estrogen (E<sub>2</sub>) increased. The differences before and after treatment for all the indicators were statistically significant (all <em>P</em> < 0.05).</p></div><div><h3>Conclusion</h3><p>Acupuncture can alleviate clinical symptoms and improve hormone levels in patients with perimenopausal insomnia. The other 60 rats were randomized (20 rats each) into a sham-operation group, a model group, and an EA pretreatment group (EA group). Except for the rats in the sham-operation group, the models of MIRI were prepared by ligating the left anterior descending coronary artery<span><span><span> in the model, EA, and EA + RVLM lesion groups. The rats of the EA group were intervened with EA at “Shénmén (神门HT7)” and “Tōnglĭ (通里HT5), 1 mA in current intensity and 2 Hz in frequency, for 20 min each time per day. Before modeling, the intervention was given for seven consecutive days. In the EA + RVLM lesion group, 3 weeks after microinjection with the neuronal apoptotic </span>virus at bilateral RVLM, the same EA intervention as the EA group was provided. Afterward, the MIRI models were prepared. In the model group, no EA intervention was given. Using Powerlab electrophysiolograph, ST segment displacement value and arrhythmia score were recorded and analyzed before modeling, 30 min after ligation, and 120 min after reperfusion in each group. The concentration of </span>cardiac troponin (cTnl) was detected with an ELISA assay kit. Using </span></span>immunofluorescence staining, the expression level of c-fos protein of RVLM was detected in the sham-operation, model, and EA groups separately. Plexon multichannel acquisition processor was adopted to record the neuronal firing and field potential of RVLM in the sham-operation, model, and EA groups.</p></div><div><h3>Results</h3><p>ST segment displacement value, arrhythmia score, and cTnl concentration 30 min after ligation and 120 min after reperfusion were all elevated in the model group compared to the sham-operation group (all <em>P</em> < 0.01). ST segment displacement value, arrhythmia score, and cTnl concentration were lower in the EA group compared to the model and EA + RVLM lesion groups (<em>P</em> < 0.01). Compared with the sham-operation group, the expression level of c-fos and the total firing frequency in RVLM were significantly higher in the model group (<em>P</em> < 0.01). However, the expression level of c-fos and the total firing frequency in RVLM were lower in the EA group compared with the model group (<em>P</em> < 0.01).</p></div><div><h3>Conclusion</h3><p>Electroacupuncture pretreatment may induce changes in c-fos protein expression and neuronal activity in RVLM to mitigate myocardial lesions. Several published studies have revealed that electro-acupuncture may relieve OSAS symptoms. However, the mechanism of electro-acupuncture in the treatment of patients with OSAS remains unclear.</span></p></div><div><h3>Methods and analysis</h3><p>We evaluated the efficacy and mechanism of electro-acupuncture in the treatment of patients with OSAS. This was a two-group parallel randomized controlled trial<span><span>. Sixty patients with OSAS will be randomly divided into an electro-acupuncture or shallow acupuncture<span> group in a proportion of 1:1, and the patients will be administered 36 sessions of electro/shallow acupuncture treatment (three times a week for 12 weeks, 20 min for each session). The </span></span>Epworth Sleepiness Scale<span> (ESS) will be used as the primary outcome. Secondary outcomes will include polysomnography (PSG), sleep quality, emotional state, and cognitive function. In addition, patients will receive neuroimaging scanning before and after the entire acupuncture treatment to evaluate neuroplasticity changes, including gray matter density, white matter integrity, and functional connections.</span></span></p></div><div><h3>Expected outcomes</h3><p>We believe that the use of electro-acupuncture will be more effective in patients with mild to moderate OSAS and that the cognitive and emotional status of patients will improve after acupuncture treatment. In addition, we aim to clarify which brain areas can be altered by acupuncture in patients with OSAS.</p></div><div><h3>Discussion</h3><p>This randomized clinical trial aims to explore the curative effect and central mechanism of acupuncture treatment for OSAS. We will explore gray matter density, white matter integrity, and functional connections of the brain using fMRI technology. The reinforcing manipulation with heat sensation (RMHS) and the reducing manipulation by inducing cool sensation (RMCS) are two representative methods of reinforcing-reducing manipulations. This trial aims to investigate the characteristic of cerebral responses to these two typical acupuncture manipulations.</p></div><div><h3>Methods</h3><p><span>A total of 35 healthy participants will be included and receive acupuncture stimulation with RMHS and RMCS in a random order. The psychology, cognition, and body constitution of participants will be measured by the Self-rating Anxiety Scale (SAS), the Self-rating Depression Scale (SDS), the mindful attention awareness scale (MAAS), the Constitution in Chinese Medicine Questionnaire (CCMQ), and the needle sensation will be evaluated after each acupuncture stimulation immediately. {"title":"Acute herpes zoster treated with surrounding fire needling combined with electroacupuncture at Jiājĭ (夹脊EX-B2): A randomized controlled trial","authors":"Jing PANG (逄静) , Hong-Na YIN (尹洪娜) , Zhong-Ren SUN (孙忠人) , Kun-Peng XIA (夏昆鹏)","doi":"10.1016/j.wjam.2023.02.002","DOIUrl":"https://doi.org/10.1016/j.wjam.2023.02.002","url":null,"abstract":"<div><h3>Objectives</h3><p><span>To investigate the clinical efficacy of surrounding fire needling combined with electroacupuncture (EA) at Jiājĭ (夹脊EX-B2) on acute </span>herpes zoster<span>, and explore the potential mechanism of this combined treatment by measuring the changes of serum inflammatory cytokines and pain mediators.</span></p></div><div><h3>Methods</h3><p><span><span><span><span>Sixty patients with acute herpes zoster were randomly assigned to either acupuncture plus western medicine group or western medicine group. In the western medicine group, the patients took orally </span>mecobalamin and </span>ganciclovir<span><span> for 14 consecutive days. In the acupuncture plus western medicine group, on the base of the western medicine, the surrounding fire needling (fire needling was operated swiftly toward the center of skin lesions.) combined with EA at EX-B2 was provided. </span>Acupoints<span> of EX-B2 were selected based on the nerve ganglion<span><span> corresponding to the skin lesions, as well as two adjacent nerve ganglions(one level upwards and one downwards) to the lesions, and were stimulated with EA once daily for 14 days. The change of symptoms from the blister eruption to scabbing and decrustation were observed. The score of visual analogue scale (VAS), serum inflammatory cytokines (interleukin-1β, interleukin-6, tumor necrosis factor-α) and pain mediators (calcitonin gene related peptide, substance P) before and after treatment, the scores of </span>Pittsburgh sleep quality index (PSQI) and </span></span></span></span>Hamilton depression scale (HAMD) before and after treatment as well as at 1 month of follow-up, and the overall effective rate and the incidence of post-hepatic </span>neuralgia at 1 month of follow-up were assessed and compared between two groups, respectively.</p></div><div><h3>Results</h3><p>After treatment completion, the time of blister relief, scabbing and decrustation in the acupuncture plus western medicine group was significantly shorter when compared with that of the western medicine group (all <em>P</em> < 0.05). After treatment, VAS score, PSQI score and HAMD score were all reduced significantly when compared with that before treatment within group (<em>P</em> < 0.05). These scores in the acupuncture plus western medicine were reduced significantly compared with the western medicine group at the same time point (all <em>P</em> < 0.05). After treatment, the incidence of post-hepatic neuralgia was 6.89%, lower than that in western medicine group (50.00%, <em>P</em> < 0.05). After treatment, the values of serum inflammatory cytokines (IL-1β, IL-6, TNT-a) and pain mediators (CGRP, SP) were reduced when compared with the those before treatment within group (<em>P</em> < 0.05), and the reduction in the acupuncture plus western medicine group was larger than that in the western medicine group (<em>P</em> < 0.05). Sliding cupping was applied on the back, followed by moxibustion along the running course of the governor vessel. After 6 treatments, the symptoms and signs of patients were evaluated according to guideline and reported.</p></div><div><h3>Results</h3><p>Compared with before treatment, the score of symptoms and sighs all decreased after treatment(7.64 ± 1.62 vs 1.86 ± 0.65 ,2.57 ± 1.26 vs 0.96 ± 0.57, both <em>P</em> < 0.05). Among 28 patients, the treatment was remarkably effective in 14 patients, effective in 12 patients and no effect in 2 patients. The total effective rate was 92.86%.
