The non-moxibustion group received standard western treatment, including 48 weeks of antiretroviral therapy (ART). The moxibustion group received ART combined with moxibusion therapy, administered three times weekly. Each treatment lasted 10 weeks, with four courses completed over 48 weeks, separated by 2-week breaks. At different time points, plasma levels of CD4<sup>+</sup>, CD8<sup>+</sup>, CD45RA<sup>+</sup>, CD45RO<sup>+</sup>, CD4<sup>+</sup>CD28<sup>+</sup>, CD8<sup>+</sup>CD38<sup>+</sup>, and CD4<sup>+</sup>CD38<sup>+</sup> were compared between the two groups.</div></div><div><h3>Results</h3><div>A total of 200 eligible patients were included and divided into two groups, with 100 in the non-moxibustion group and 100 in the moxibustion group. At Week 24, the CD4<sup>+</sup>T cell count was (180.71 ± 79.62) cells/μL in the non-moxibustion group and (218.22 ± 82.02) cells/μL in the moxibustion group. By Week 36, the counts were (204.83 ± 96.78) cells/μL and (239.35±81.90) cells/μL, respectively. At Weeks 24 and 48, the CD8<sup>+</sup> T cell and CD45RO<sup>+</sup> counts were higher in the moxibustion group than in the non-moxibustion group (<em>P</em> < 0.05). By Week 48, the CD45RA<sup>+</sup> count was also higher in the moxibustion group (<em>P</em> < 0.05). The retrieval period was from the inception to April 1, 2023. Two researchers independently screened the literature, extracted data, and used the Scientific, Transparent, and Applicable Rankings (STAR) tool to evaluate the quality of guidelines/expert consensus that contained acupuncture and moxibustion recommendations, and analyzed acupuncture and moxibustion recommendations in guidelines/expert consensus and their evidence base.</div></div><div><h3>Results</h3><div>A total of 56 guidelines/expert consensus related to the prevention and treatment of chemotherapy- and/or radiotherapy-induced nausea and/or vomiting were included, and 22 guidelines/expert consensus contained acupuncture and moxibustion recommendations, with a total of 34 recommendations on acupuncture and moxibustion (27 supports, 4 neutrals, 3 objections). The STAR evaluation (excluding 1 patient version of guideline) showed that none of the guidelines/expert consensus was with high quality, 95.24% (20/21) with medium quality, and 4.76% (1/21) with low quality. Although there were many recommendations of acupuncture and moxibustion in the existing guidelines/expert consensus, there was great heterogeneity among the acupuncture and moxibustion treatment regimens. However, its specific effects are still not clear. This study aims to evaluate the blinding effect of a non-penetrating sham FSN needle, developed for the first time by the authors' research team in healthy participants. Furthermore, to provide a reliable control tool to assess the specific effects of FSN.</div></div><div><h3>Methods</h3><div>We conducted a randomized, sham-controlled trial in 60 healthy participants divided into FSN group and sham FSN group in a 1:1 ratio. The participants were assessed according to blinding index (BI), pain intensity, sensation type and acceptability of needling.</div></div><div><h3>Results</h3><div>The James' BI was (0.56, 95 %CI [0.45, 0.67]) for two groups. The Bang's BI was (0.50, 95 % CI [0.24, 0.76]) in the FSN group and (−0.36, 95 % CI [−0.67, −0.07]) in the sham FSN group. There was no significant difference between needling pain intensity (<em>P</em> > 0.05) and type of needling sensation. This is the first clinical practice guideline (CPG) on acupuncture and moxibustion for nonspecific low back pain approved by an international academic organization, which provides the evidence-based recommendations and practical therapeutic protocols for international acupuncture practitioners. This CPG was developed by following Grades of Recommendation, Assessment, Development, and Evaluation (GRADE) methodology, and <em>the Principles of the World Health Organization Handbook for Guideline Development</em>. The guideline development group (GDG) from different countries, with different professions, played a critical role in the formulation of clinical questions, recommendations, and therapeutic protocols. Recommendations are the key content of a CPG and the direct answers to clinical questions. Hence, this article focuses on the recommendations of this CPG. The recommendations were formulated using the modified Delphi method and the GRADE grid rules, based on the updated systematic reviews of clinical evidence. The CPG was developed according to the Grades of Recommendation, Assessment, Development, and Evaluation (GRADE) methodology, referring to the principles of the <em>World Health Organization Handbook for Guideline Development</em>. During the development of the CPG, the guideline development group (GDG) played an important role, which is composed of multi-national and multi-disciplinary experts. The clinical questions, recommendations and therapeutic protocols were all formulated by GDG using the modified Delphi method and basing on the results of the latest systematic review. This article focuses on the recommendations in the CPG. The CPG contains ten recommendations for ten clinical questions, including nine conditional recommendations for the intervention, one conditional recommendations for either the intervention or the comparison. These tools include the evidence quality assessment tool, the evidence summary tool, and the evidence-to-decision (EtD) framework. Although there is a methodologically guided standard pathway for formulating recommendations in clinical practice guidelines, it remains unclear how to translate this theoretical pathway into the practical process for formulating recommendations and what the specific steps in this process entail. Therefore, this paper takes the World Federation of Acupuncture-Moxibustion Societies <em>Clinical Practice Guidelines on Acupuncture and Moxibustion: Allergic Rhinitis</em> as an example to detail the full process of formulating recommendations and summaries it into a three-step method. <strong>Step one</strong>: Collect original materials from different sources to create EtD table. <strong>Step two</strong>: Distribute and collect EtD table, and preliminarily gather the recommendation voting results. <strong>Step three</strong>: Organize “salami-slicing pattern” guideline development group consensus meeting to formulate recommendations. The three-step method for formulating recommendations in clinical practice guidelines fully utilizes the GRADE EtD table to provide guideline development group with decision-making foundation and reference from multiple dimensions, including the importance of clinical questions, balance of benefits and harms, quality of evidence, resource required, values and preferences, health equity, acceptability, and feasibility. This approach enhances the scientific and practical nature of guideline recommendations.
