The aim is also to assess the susceptibility of dural sinuses to injury in descending orders. Additionally, the research examines the relationship between types of fracture and the challenges in controlling bleeding in patients with traumatic dural venous sinus injury. Furthermore, the study analyzes the factors that influence the morbidity and mortality rate among patients with traumatic dural venous sinus injury.\u0000Methodology: Between January 2021 and October 2022, a prospective study was carried out, involving 22 patients diagnosed with dural venous sinus injury. The study encompassed both surgically and conservatively treated patients. For each participant, demographic information, such as age, sex, causative trauma, preoperative GCS, preoperative deficit, image findings, name, and part of the injured sinus, as well as the presence and absence of a fracture and its relation to the sinus were recorded. The intraoperative method employed to control bleeding and the amount of blood transfusion were also documented, along with postoperative GCS and postoperative complications. The Glasgow Coma Scale (GCS) score was utilized to assess the neurological outcome.\u0000Findings: The most frequently injured dural venous sinus was the superior sagittal sinus, accounting for 77.3% of cases (8).Among patients with a linear skull fracture crossing the venous sinus, 83.3% had bleeding that could be easily controlled by placing Gelfoam over the injured sinus . However, in patients with a skull fracture parallel to the sinus over the venous sinus and those with depressed fractures (with/without penetration), we encountered greater difficulty in controlling bleeding (14). Only 16.7% and 10% of these cases, respectively, were easily controlled by placing Gelfoam over the injured sinus, necessitating additional techniques to control bleeding in the remaining patients. Out of the total patients, 20 were managed surgically, while two were managed conservatively due to a Glasgow Coma Scale (GCS) score of 3 and signs of brain death.\u0000Unique Contributions to Theory, Policy and Practice: The superior sagittal sinus was identified as the most commonly injured sinus. Road traffic accidents (RTAs) accounted for the most frequent cause of dural sinus injury, representing 40.9% of cases. Bleeding control proved more challenging when dealing with linear fractures parallel to the sinus or depressed (and/or penetrating) fractures over the sinus, compared to linear fractures crossing the sinus. The most significant factor influencing the mortality rate was the Glasgow Coma Scale (GCS) score on admission. Mortality and morbidity rates were highest in cases involving injury to the posterior part of the superior sagittal sinus, sigmoid sinuses, and combined injury to the anterior and middle parts of the superior sagittal sinus. Ventriculoperitoneal shunting is one of the commonest procedures for management of hydrocephalus. Assessment and evaluation of the clinical outcome, following VP shunt, should be carried out to decrease rates of complications of this procedure. Clinical audit plays a vital role in identifying and developing effective methods for improving patient care. This paper sheds light on 37 neurosurgical patients with hydrocephalus who underwent ventriculoperitoneal shunt surgery. Our aim was to identify defects in steps in management of hydrocephalus by VP shunt from time of admission to discharge. \u0000Methodology: In this audit study 37 patients with hydrocephalus were selected between November, 2021 and December, 2022 at West Erbil emergency hospital and Erbil teaching hospital. VP shunt surgery was performed for them. Information was gathered from patient records, diagnostic imaging, and operation notes, and subsequent clinical follow-up evaluations were carefully analyzed and reviewed. The gathered data encompassed demographic details, clinical manifestations, radiological studies, reasons for surgery, operative findings, and complications associated with the shunt. \u0000Findings: The prevalent causes observed were congenital hydrocephalus (45.9%), posterior fossa tumors (13.5%), and post meningitis hydrocephalus (18.9%). A right-sided VP shunt utilizing a medium-pressure valve system emerged as the most prevalent treatment modality. Three patients exhibited symptoms and signs indicative of shunt infection. Additionally, one patient experienced an epidural hematoma at the site of the burr hole. \u0000Unique contribution to theory, practice and policy: Patients with hydrocephalus and undergoing VP shunt surgery should be evaluated carefully and every step from time of admission to discharge is crucial in decreasing the complications which are associated with this surgery.

Somatic symptom disorder is characterized by persistent physical symptoms without a discernible medical explanation, leading to impaired daily functioning, reduced quality of life, and increased healthcare utilization. Given the limited research on somatic symptom disorder among medical students in Pakistan, this study aims to assess the prevalence and severity of somatic symptom disorder among medical students at the Islamic International Medical College in Rawalpindi, Pakistan. The assessment will utilize the Patient Health Questionnaire Physical Symptoms (PHQ-15) as a measurement tool. \u0000Methodology: A convenience sampling study was conducted among 500 medical students, with 220 respondents completing the PHQ-15. Descriptive and inferential statistics were used to analyze the data. \u0000Findings: The prevalence of somatic symptom disorder among medical students was found to be 44.5%, with a significantly higher prevalence among females (53.96%) compared to males (31.89%). The severity level of somatic symptom disorder was classified as minimal in 17.3%, low in 38.2%, medium in 28.6%, and high in 15.9% of cases among medical students. \u0000Unique Contribution to Theory, Policy and Practice: This study highlights a high prevalence of somatic symptom disorder among medical students in Pakistan, particularly among females. Findings suggest the need for targeted interventions such as stress management programs, mental health services, and changes to the medical education curriculum to promote student well-being. Further research is needed to explore the causes and consequences of somatic symptom disorder in this population.

Mavole
Purpose: Maternal and child healthcare play crucial role in reducing global mortality rates, with a significant impact on the overall burden of disease. The Sustainable Development Goal 3 aims to reduce the global maternal mortality ratio to less than 70 per 100,000 live births, end preventable deaths of newborns and children under 5 years of age, with all countries aiming to reduce neonatal mortality to at least as low as 12 per 1,000 live births and under-5 mortality to at least as low as 25 per 1,000 live births by 2030. Sub-Saharan Africa, including Tanzania, carries a high burden of mortality rates and experiences a significant share of maternal and child deaths, exacerbating the global figures. The average prevalence of below 5 years mortality in SSA between 2010 and 2018 was 4.10% according to Ahinkorah (2021). Recognizing the severity of this issue, the government of Tanzania through the Development Agenda 2025, Draft health policy 2020 and the current Health Sector strategic plan 2021-2026 plans to reduce infant and maternal mortality rates through mother and child healthcare investment. The Health Sector Strategic Plan focuses on strengthening the health systems in the country and aims to sustain the achievements made in enhancing the health of reproductive, maternal, newborn, children, and adolescent populations. This study sought to assess the benefits of investing on mother and child healthcare in Tanzania. The study was guided by the investment commodity theory developed by the economist John Maynard Keynes in 1936. The study evaluated the current status of mother and child health indicators, the benefits of investing in mother and child health on overall health outcomes and identified the existing gaps and challenges in the provision, access and utilization of mother and child healthcare services in Tanzania. \u0000Methodology: The study was a desktop review-based study which relied on existing published information, data, and literature. \u0000Findings: The study found out that there have been improvements in certain areas of mother and child health indicators but maternal and child mortality rates remain high. Health interventions such as vaccination programs, improved access to healthcare services, and the use of maternal and child health insurance cards showed positive effects on reducing child mortality, improving child health outcomes, and enhancing maternal healthcare utilization. The significant gaps and challenges in the provision, access, and utilization of mother and child healthcare services include disparities in access to healthcare services based on factors such as education level and wealth status, inadequate healthcare infrastructure, long distances to health facilities, sociocultural barriers, and financial constraints. \u0000Unique contribution to theory, practice and policy: The study recommends strengthening of healthcare infrastructure, enhancing access to maternal and child healthcare services, especially for vulnerable pop

The clinical manifestations are variable and non-specific. The aim of our work is to describe the clinical aspects, evolution and management of adrenal hemorrhage in the neonatal period.Methodology: This report is based on a presentation of a newborn with an adrenal hematoma who was hospitalized in our neonatology unit.Findings: The patient is a three-day old newborn female admitted in a state of shock. The anamnesis does not find any particular antecedent in antenatal or perinatal except an early rupture of membrane for more than 6 hours. The newborn was urgently transfused with phenotyped red blood cells and an antibiotic therapy was prescribed. The abdominal ultrasound showed a right adrenal hematoma measuring 42mm x 33mm.Unique Contribution to Theory, Policy and Practice: The clinical manifestations of adrenal hemorrhage depend on the severity of the hemorrhage and the extent of which the adrenal cortex is involved. In extreme cases, the newborn can present a hypovolemic shock, as shown in our observation, requiring an emergency blood transfusion. Acute adrenal insufficiency is rarely reported in adrenal hemorrhage because the bleeding is mainly subcapsular and hormonal insufficiency does not occur below 90% of adrenal tissue damage.
