Miguelez PhD","doi":"10.1016/j.enfi.2021.05.003","DOIUrl":"10.1016/j.enfi.2021.05.003","url":null,"abstract":"<div><h3>Aims</h3><p>To identify commonly used intravenous drugs that may produce endothelial damage.</p></div><div><h3>Methods</h3><p>An experimental research study was performed using a sample of 62 intravenous drugs commonly used in emergency care, pH and osmolarity were measured. Subsequently, based on these values, the theoretical capacity to cause irritation or endovascular damage was determined and classified as high, moderate, and low.</p></div><div><h3>Results</h3><p>Samples from 19 drugs for fluid therapy, 21 antibiotics and 22 drugs for intravenous use were studied. Glucose solutions, sodium bicarbonate 1M and mannitol 10% showed a high capacity to cause venous irritation. Vancomycin, ciprofloxacin, amiodarone, haloperidol, and labetalol solution presented a high capacity for irritation based on their acidic pH. The antibiotics, dexketoprofen, diazepam, digoxin, etomidate, phenytoin, levetiracetam and metamizole also showed high osmotic values in their reconstituted or undiluted presentations. Moreover, osmolarity of diazepam, digoxin and phenytoin remained high despite being diluted in 100<!--> <!-->mL of saline.</p></div><div><h3>Conclusions</h3><p>Knowing the pH and osmolarity of intravenous drugs allows their capacity to cause endothelial damage to be assessed. Pressure on the National Health System has created unique demand particularly in certain services and care units like the critical care units.</p></div><div><h3>Objective</h3><p>To learn about the experience of nurses in caring for people with coronavirus in critical care units.</p></div><div><h3>Method</h3><p>A qualitative phenomenological study that constitutes the second phase of a mixed methodology project. We conducted interviews with 17 nurses caring for patients affected by COVID-19 in critical care units. The interviews were audio-recorded, transcribed, and analysed using thematic discourse analysis.</p></div><div><h3>Results</h3><p>This article shows the findings around the theme “the value of human resources” which is nuanced through sub-themes “it's not the beds, it's the expert staff”, “shouldering the patient's burden”, and suffering because “they have not cared well”.</p></div><div><h3>Discussion</h3><p>Expert nurses have emerged as leading professionals in the care of the critical patient during the COVID-19 pandemic. This leadership is executed from a profile framed in an inclusive philosophy where wisdom, agility and intuition are the key elements underpinning problem identification and solving in a creative way, while adapting to the needs of the patient and healthcare team as they emerge.</p></div><div><h3>Conclusions</h3><p>Expert nurses have played an advanced role in the management of care and human resources by exercising effective leadership in the clinical setting. Grade of sedation was assessed using the Spanish version of the CBS-S and the Bispectral Index on sedation, once per shift over one day. Grade of withdrawal was determined using the CBS-S and the Withdrawal Assessment Tool-1, once per shift over three days.</p></div><div><h3>Results</h3><p>A total of 261 critically ill paediatric patients with a median age of 5.07 years (P25:0.9-P75:11.7) were included in this study. Pérez-Pérez RN","doi":"10.1016/j.enfi.2021.03.007","DOIUrl":"https://doi.org/10.1016/j.enfi.2021.03.007","url":null,"abstract":"<div><h3>Introduction</h3><p>Multiple attempts during peripheral cannulation can have major consequences for patients, relatives, and healthcare professionals, therefore we set out to determine the extent of this problem in a paediatric intensive care unit (PICU).</p></div><div><h3>Objectives</h3><p>The main aim was to describe peripheral venous catheter (PVC) and peripherally inserted central catheter (PICC) cannulation in children in the PICU. Secondary objectives were to determine the success rate of the first cannulation attempt, to quantify patients with difficult venous access (DVA), and to explore the association between DVA and sociodemographic, technique and nursing-related characteristics.</p></div><div><h3>Method</h3><p>A cross-sectional descriptive study. Consecutive sampling was used to recruit patients aged 0-18 years admitted to the PICU who required peripheral venous cannulation. An ad hoc questionnaire was used for this purpose, including the presence of DVA as an independent variable.</p></div><div><h3>Results</h3><p>A total of 163 venous cannulations were reported. A total of 55.8% (91) were performed in patients under 1 year of age. Of these, 38.7% (63) were successful on the first attempt and 36.8% (60) had DVA. When there was DVA, 85% (51) of patients had complications, median time to cannulation by short CVP was 30<!--> <!-->minutes [15-53] and 2 or more nurses were required on 80% (48) of occasions.</p></div><div><h3>Conclusions</h3><p>We found a low success rate at first attempt and a high proportion of DVA. More nurses and time were employed during cannulation and complications increased if the patient had DVA.
