Alwunais","doi":"10.1016/j.jdds.2015.03.005","DOIUrl":"10.1016/j.jdds.2015.03.005","url":null,"abstract":"<div><p>Lepromatous leprosy is usually generalized at diagnosis and may consist of erythematous macules, papules and nodules containing abundant <em>Mycobacterium leprae</em> and affecting wide spread areas of the skin. The primary lesions are pea-sized pustules classically described as half-pustular, half-clear flaccid blisters. Histologically the most important feature is a subcorneal accumulation of neutrophils with the absence of spongiosis or acantholysis. In this paper we present the case of a 7-years-old boy diagnosed with SCPD based on the characteristic clinical and histological features. It was reported to possess numerous pharmacological effects related to several organs of the body. In this article, the literature pertaining to dermatological effects of <em>N. sativa</em> is reviewed. Most of these are contact dermatitis as a result from contact with a chemical substance. Contact dermatitis can be either irritant or allergic type. Each type has a different mechanism while the clinical presentation is the same. Management of contact dermatitis must include both medical treatment and workplace modifications as appropriate to reduce exposure to the causative agents. Our aim was to evaluate the correlation between serum PRL levels and severity of psoriasis, vitiligo and alopecia areata.</p></div><div><h3>Patients and methods</h3><p>We performed a case–control study on 100 subjects: 75 patients; suffering from psoriasis, vitiligo and alopecia areata; 25 patients in each group and 25 age- and sex-matched healthy controls.</p></div><div><h3>Results</h3><p>Serum prolactin levels were significantly high in all three dermatological diseases in comparison with the control group (<em>P</em> <!-->=<!--> <!-->0.000). The mean<!--> <!-->±<!--> <!-->SD of the serum prolactin levels was 21.8<!--> <!-->±<!--> <!-->11.5<!--> <!-->ng/ml, 16.9<!--> <!-->±<!--> <!-->6.8<!--> <!-->ng/ml, and 16.6<!--> <!-->±<!--> <!-->8.0<!--> <!-->ng/ml in patients with alopecia areata, psoriasis and vitiligo respectively. It may present as blepharoconjunctivitis, and is therefore often neglected. We present a 36<!--> <!-->year-old female who suffered itching and irritation at eyelids. A careful examination of the eyelids objectified blackish granules of cilia. Dermoscopic examination revealed the presence of nits attached to the eyelashes with tiny brownish punctuation found on the skin, which they could easily recognize as adult lice that were mobilized around the eyelids. She was successfully treated with oral ivermectin. After clinical and dermoscopical control, no more lice or nits were present in the patient. Our case describes the clinical course of a patient with chronic hepatitis C treated with pegylated α2 INF plus ribavirin. After 6<!--> <!-->weeks of therapy he developed psoriasis. However, given the progressive improvement of his transaminase levels and viral load decrease, we continued the protocol of the treatment and we started topical calcipotriol/betamethasone dipropionate ointment for psoriasic lesions. After this, the psoriasis improved in a slow but comprehensive manner. {"title":"Clinico-epidemiological profile of vitiligo patients in Najran Region, Saudi Arabia","authors":"Hamad Ayed AL Fahaad","doi":"10.1016/j.jdds.2014.05.001","DOIUrl":"10.1016/j.jdds.2014.05.001","url":null,"abstract":"<div><h3>Objectives</h3><p>To assess the prevalence of vitiligo among patients with dermatological diseases in Najran, South-Western Saudi Arabia, and to explore the epidemiological profile of vitiligo patients in the region.</p></div><div><h3>Materials and methods</h3><p>This single-observer, cross-sectional, descriptive study was conducted over a period of six months by recording the clinico-epidemiological profile of 101 vitiligo cases. The patients were diagnosed by history, clinical findings and Wood’s lamp at the dermatology out-patient clinic in the King Khalid Hospital, Najran. Demographic and clinical data of the patients were collected using a pre-structured data collection form. Pearson <em>χ</em><sup>2</sup> test and Fisher’s exact test were used to explore the significant association of selected characteristics with ‘type’ and ‘site’ of vitiligo among the study-patients.</p></div><div><h3>Results</h3><p>The estimated prevalence of vitiligo cases among the dermatology out-patient clinic attendees was 1.98%. Vitiligo patients were predominantly males (57.4%), Saudis (93.1%), housewives (24.8%), and non-smokers (95%). The mean age of the study-patients was 34.33<!--> <!-->±<!--> <!-->15.82 years, and the highest number (28.7%) belonged to the age group ‘31–40<!--> <!-->years’. Of the cases, 5.9% had a family history of vitiligo, 6.9% had diabetes mellitus, and 4% were suffering from anemia. Approximately 99% of the patients or the parents of the pediatric patients were affected by either moderate or mild stress due to vitiligo. Highest number (48.5%) of the patients presented localized type of vitiligo, whereas 22.8% had acrofacial, 20.8% showed vulgaris, 5% presented universal, 2% had acral, and 1% experienced segmental type of vitiligo. Around one-fourth (25.7%) of the patients had vitiligo only on their extremities, while remaining cases experienced vitiligo on the other parts of their body. Median duration of the disease was 4<!--> <!-->years (IQR: 2–7.5<!--> <!-->years). The type of vitiligo showed a significant association with sex, underlying diseases, and stress grade (<em>p</em> <!-->&lt;<!--> <!-->0.05), while the site of vitiligo was significantly associated with sex and stress grade (<em>p</em> <!-->&lt;<!--> <!-->0.05).</p></div><div><h3>Conclusion</h3><p>Localized vitiligo was found to be the most common type of vitiligo prevalent in the Najran Region. A 42-year-old woman suffering from Primary Sjögren’s syndrome retained on the basis of clinical and serological findings, was admitted 3<!--> <!-->years later for genital bleeding. The diagnosis of Bowen’s disease was established by clinical and histolopathological examination. Classical association of lymphoma and Sjögren’s syndrome is described, but to the best of our knowledge, it is the second report of BD associated to Sjögren’s syndrome in published literature.
