The evolution of the disease is usually benign, but a malignant transformation in mycosis fungoides is reported. You have to know how to think about it when faced with a generalized hypochromic macular eruption in children. \u0000Case Report: We report a case of a 13-years-old child, photo type 4.He presented for 8 months hypochromic macular lesions, located initially at the face. The evolution was marked by the extension at the trunk and the limbs. He was treated for pityriasis versicolor but without improvement. A skin biopsy was then performed, in favor of Pityriasis Lichenoides Chronica (PLC) with a lymphoid infiltrate made up mainly of CD4 T lymphocytes. The child was put on cyclin associated with heliotherapy with a moderate improvement after 3 months of treatment. The patient was followed for 2 years during which he did not present any malignant transformation. \u0000Discussion: Pityriasis Lichenoides is less frequent in the pediatric population.Its etiopathogenesis is still incompletely elucidated. The basic lesion is an erythematous-squamous papule in the trunk. Hypochromic macules can also be observed, either at the cicatricial stage of the disease, or they occur immediately, especially in subjects with dark skin. The histology of PLC reveals perivascular lymphocytic infiltrate in the superficial dermis..The adopted therapeutic is based on heliotherapy or an antibiotic from the macrolide or cyclin family. Evolution towards Mycosis Fungoides has been rarely described. \u0000Conclusion: PLC is exceptionally hypopigmented from the outset. Despite treatment with intravenous and topical antibiotics, the ulcers did not improve, and cultures showed no growth of organisms. The patient did not have any underlying systemic disease, and a skin biopsy revealed neutrophilic dermatitis. Oral prednisone was started, which resulted in the healing of the lesions. The skin biopsy and positive response to corticosteroids confirmed the diagnosis of Pyoderma Gangrenosum (PG). PG is a rare inflammatory skin condition, and facial lesions are a rare presentation. Dermosocpy is an innocuous examination allowing to guide the clinical diagnosis and to avoid invasive examinations. \u0000Presentation of Case: 5-month-old infant presents with thickening of the thumbnail of the left hand with xanthonychia evolving for 20 days without notion of trauma. On clinical examination, xantho-pachyonychia of the thumbnail of the left hand was noted reaching the latero-distal part with subungual hyperkeratosis and peronyxis. Dermoscopy showed yellowish chromonychia, longitudinal striae, subungual hyperkeratosis with jagged edges. The mycological direct examination and culture was in favor of a Candida albicans infection The patient was put on an antifungal cream with improvement after 3 months. \u0000Discussion: The rarity of onychomycosis in infants can be attributed to several factors such as the difference in the structure of the nail plate, less exposure to trauma and rapidity of nail regrowth. The average age is 8 months, the sex ratio at 0.98 and the preferential location is the fingernails. Clinically the nail involvement is of the disto-lateral type with subungual hyperkeratosis and peronyxis. Dermatoscopy is a quick tool showing chromonychia, longitudinal streaks, subungual hyperkeratosis producing a “ruined appearance”, distal onycholysis with jagged edges and linear hemorrhages. Infant onychomycosis is most often of candidal origin (C. albicans). The variables studied; prematurity, low weight, type I respiratory distress syndrome, perinatal asphyxia, sepsis, necrotizing enterocolitis, administration of vancomycin, amikacin, amphotericin, cefotaxime and water restriction. The information was collected from the data of the clinical records and the analysis was carried out in the Epi-info software. The statistical analysis, it was performed using non-parametric tests such as Chi2 and Fisher's Exact Test. \u0000Results: Denote underweight with an OR 2, 95% CI. from 1.1 to 3.3 and <P 0.05 by Chi2 and Fisher's exact test. For; perinatal asphyxia with OR 2.1, 95% CI. from 1.3 to 3.4 and <P 0.05 through Chi2 and Fisher's exact test. For vancomycin administration OR 4.7, 95% CI. From 2.3 to 9.3 and <P 0.05 by Chi2 and Fisher's exact test. For water restriction OR 2, 95% CI. Data related to treatment was also collected and their outcome was observed. \u0000Results: Among 50 patients, majority (74%) had age of onset between 12-36 months of age. Mean age was 43±26 months, the range was between 4 months to 84 months. Female were predominant (60%). Most of the children came from urban area (86%) and belong to nuclear family (78%). They presented with stereotype movement of variable duration and there was no alteration of consciousness. Prone positioning was more frequent than prone to supine position (78% Vs 18%). There was no laboratory and EEG abnormalities among the cases. Thirty two percent patients were treated with anti-epileptic drugs from outside and among them 2 (12%) was given sodium valproate and rest 14 (88%) were treated with phenobarbitone. The study was conducted from April–December 2022. Pearson's correlation and logistic regression analyses were used to assess the relationship and correlation between the CT ratio, gestational age, and estimated foetal weight. The p<0.05 at 95%CI level of significance was set. \u0000Results: The mean bi-parietal diameter estimated foetal weight and CT diameter ratio was 66.5 ± 18.3 mm, 1.24 ± 0.87 kg and 0.46 ± 0.02 cm respectively. This form of psoriasis is often observed in children and can be difficult to distinguish from other acquired Blaschko linear dermatoses such as inflammatory linear verrucous epidermal nevus (ILVEN) and lichen striatus. The prevalence of PBL is unknown, but it is generally observed in children and is more common in boys than in girls. The clinical and histological similarity of PBL to ILVEN presents a diagnostic challenge. In this case report, we present the case of a 7-year-old child with PBL and discuss the diagnostic and therapeutic aspects of this rare form of psoriasis. We were able to confirm the diagnosis of PBL after treatment with white petroleum jelly and salicylic acid petroleum jelly 5% resulted in a significant improvement of the lesions. Topical treatments, phototherapy, and systemic medications can be used in the case of PBL. No single metric can describe their nutritional status; therefore, a series of indices and methods are required for evaluation. \u0000Aim of the Work: Evaluation of the nutritional status of children with CKD using dietetic history, anthropometric measurements, biochemical parameters and BIA. \u0000Methods: This case-control study was conducted on forty CKD children (stages 3-5) from those attending Pediatric Department, Tanta University Hospitals between March 2021 and February 2022 in comparison to forty healthy control children. All were subjected to: dietetic history, anthropometric measurements including (weight, height, body mass index, mid-arm circumference, skin fold thickness), BIA using TANITA Body Composition Analyzer MC-980 MA-N plus III device and laboratory investigations: (CBC, urea, creatinine, ABG, sodium, potassium, phosphorus, calcium, alkaline phosphatase, parathormone hormone, serum proteins, albumin and 24hrs urinary proteins). \u0000Results: CKD children had significantly lower caloric intake and other nutrient consumption including protein, carbohydrate and fat intake (p < 0.05), significantly lower anthropometric measurements including weight, height, body mass index, skinfold thickness and mid-arm circumference than controls (p < 0.05) and significantly lower in BIA measurements including fat mass (FM), fat free mass (FFM), muscle mass (MM), total body water (TBW) and bone mass (BM) than controls. There was significant positive correlation between caloric intake and protein intake with FFM, MM, SMM, BM, TBW and BMR, also there was significant positive correlation between serum albumin and FFM, MM, SMM, BM and TBW in CKD patients. \u0000Conclusions: CKD children had low caloric and protein intake, low body composition parameters, so good nutritional assessment and improvement of their nutritional status is very important. {"title":"A Case Report and Review of Increased IgE in Patients with Transient Hypogammaglobulinemia of Infancy and Atopic Dermatitis after Normalization of IgG","authors":"T. F. Raham","doi":"10.9734/ajpr/2023/v11i2217","DOIUrl":"https://doi.org/10.9734/ajpr/2023/v11i2217","url":null,"abstract":"The coexistence of a high IgE level and transient hypogammaglobulinemia of infancy ( THI ) with atopic dermatitis (AD) is well known, but the coexistence with a marked serum IgE level ≥ 1000 IU/mL is a rare finding. Literature is scarce regarding IgE levels in patients with THI and AD after treatment with immunoglobulin (Ig). We reported a female infant 8 months old, with THI and severe AD. She was treated with intravenous immunoglobulin (IVIG) infusion after the failure of conventional therapy. Initially, serum levels of IgE were high, and serum levels of IgG were low. After 4 weeks of treatment with IVIG, patient profile showed normalization of IGE and higher elevation in IgG level. The present article is the first to describe a finding of marked IgE elevation (> 1000 IU/mL) after treatment with IVIG. Furthermore, our finding is supported by our review of increased IgE levels in patients with THI and AD who received IG therapy.
