Recently, two types of such damage have become highly relevant. The first of them is caused by viral damage to the endothelium, which leads to uncontrolled cascade activation of the descending system. The second type is caused by aquabarotrauma and associated hydrodynamic shock. Both types of damage lead to the development of thrombotic-fibrous complications of the circulatory system due to the formation of long fibrin deposits. Such deposits are resistant to the action of the fibrinolytic system due to their irregular and chaotic structure. This, in turn, leads to severe functional complications.\u0000Aim: Systematize data on molecular and cellular features of systemic damage to the endothelium. To compare the clinical material of persons who suffered from or died of COVID-19. To compare the functional disorders of the hearing organs in persons who have experienced akubarotrauma or who have become ill with COVID-19.\u0000Results and discussion: The clinical material of persons who suffered from COVID-19 or died as a result of it is presented. The functional disorders of the hearing organs in persons who have experienced acubarotrauma or who have become ill with COVID-19 are considered. The reasons for the resistance of thrombotic-fibrous deposits in vessels with systemically damaged endothelium are discussed. Anesthetic support during total laryngectomy needs to be improved due to the possible undesirable effect of the standard scheme of intraoperative management of patients.\u0000Aim: to evaluate the effectiveness of anesthetic support during total laryngectomy using standard doses of opioids, the severity of side effects in the early postoperative period, and the feasibility of finding alternative pain relief schemes.\u0000Materials and methods: 25 patients with T3-4N0-3M0 laryngeal cancer who underwent radical surgical treatment were examined. During total laryngectomy, patients were anesthetized using standard doses of opioids without the use of adjuvant components of anesthesia. Anesthesia control was carried out on the basis of monitoring the bispectral index (BIS) and changes in the index of analgesia and nociception (ANI). In dynamics, biochemical and immunological studies of the blood of patients were carried out. Assessment of pain syndrome was carried out according to VAS indicators.\u0000Statistical analysis was carried out using classical methods of descriptive and analytical statistics using the software product STATISTICA v.6.1 (Statsoft Inc., USA, license number AGAR909E415822FA).\u0000Results and discussion: An analysis of anesthetic support during total laryngectomy using standard doses of opioids showed that the condition of patients after surgery is characterized by a certain immunosuppression with lymphopenia, a decrease in the level of interleukin 2 and tumor necrosis factor alpha, which lasts 48 hours and is most pronounced at 2-day after surgery.\u0000As a result of the study, the dependence of the pain syndrome in patients with total laryngectomy on the injected dose of opioids was revealed – the more opioids are used intraoperatively, the more patients needed them after the operation. The article analyzes the results of the use of different groups of analgesic drugs in the elimination of postoperative pain syndrome and compares them, considers the advantages and disadvantages of their use.\u0000Materials and methods: own research was concerned with the assessment of the effectiveness of postoperative methods analgesia after surgical interventions on the soft palate with the use of ketorolac and dipyrone (respectively, groups of 17 patients each). A patient questionnaire was conducted regarding possible health complications associated with taking the specified analgesics.\u0000Results and discussion: Obtained data demonstrate a certain dependence of the frequency of side effects on the drug taken by patients. It was found that headache, nausea, and tachycardia were significantly more common in subjects of the 1st group who took dipyrone, while dizziness occurred equally often in patients of both groups. Shevtsova","doi":"10.37219/2528-8253-2022-6-39","DOIUrl":"https://doi.org/10.37219/2528-8253-2022-6-39","url":null,"abstract":"Aim: study of threshold ratios between indicators of acoustic impedance and threshold tonal audiometry in military personnel – participants in hostilities.\u0000Materials and methods: the results of audiometric and impedancemetric examinations of 43 servicemen aged 18 to 42 years who received combat acute trauma in the period March-August 2022 and 20 patients with severe sensorineural deafness (2, 3, 4 stages according international classification) of non-acutraumatic genesis as a comparison group.\u0000Results and their discussion: During the examination of 43 soldiers, it was found that with significant sensorineural hearing loss on both sides (3 and 4 stages according international classification), the patients registered a full-fledged acoustic reflex during ipsilateral and contralateral stimulation. When conducting an impedanceometric examination of patients in the comparison group, it was found that in most patients the acoustic reflex was not registered at all.\u0000Conclusions:\u00001. In combat acute acoustic trauma, it is possible to register acoustic middle-ear-muscle reflex with severe hearing loss, which may be caused by damage to the receptor part of the auditory analyzer and the accompanying symptom of the phenomenon of accelerated loudness, by a central mechanism, or by a combination of central and peripheral mechanisms.\u00002. Dissociation of acoustic middle-ear-muscle reflex threshold values and threshold tonal audiometry is a characteristic feature of acutraumatic damage to the auditory analyzer.\u00003. Characteristic symptoms and modern views (sometimes hypothesis) on the causes of its occurrence are described in detail. Attention focused on diagnostic methods at the outpatient and clinical conditions of a highly specialized department. The latest data on the modern classification of this syndrome and the conclusions of the consensus of the International Barani Society in 2021 are presented.\u0000Own series with a detailed description of the features of each case, illustration of the audiometric and tomographic images and successful results of surgical treatment are presented. The state of the nasopharyngeal microbiome plays the significant role in the development of inflammatory processes of nasopharynx and adenoid hypertrophy and it is considered that the great progress can be made through its correction in conservative treatment and prevention of recurrence after surgery. Traditional cultural studies which reflect only a small proportion of bacteria present in the nasopharynx cannot describe the real state of microbiome. Therefore, the study of its quantitative and qualitative composit ion is essent ial for justified treatment.\u0000Aim: to examine the state of the nasopharyngeal microbiome in patients with CNP associated with EpsteinBarr virus and compare its characteristics with the severity of disease.\u0000Material and methods: 20 healthy persons and 32 patients diagnosed with CNP associated with EpsteinBarr virus have been included in the study. The qualitative composition of nasopharyngeal microflora and its quantitative characteristics have been studied by determining t itres of colony-forming units (CFU). The findings have been compared with the features of the clinical course of disease.\u0000Results: The healthy individuals of the control group have Lactobacillus spp. and Bifidobacterium spp., aerobic S. salivarius, which belong to commensal microorganisms, as the basis of the nasopharyngeal microbiome. There has been seen the preservation of the population level of commensal flora: lactobacilli, bifidobacteria, salivary streptococci and appearance of opportunistic flora (Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pyogenes, Escherichia coli) in small titres in the patients with mild CNP. During the moderate/severe course, there has been observed a sharp decline in the population level of commensal flora and an increase in the titer of opportunistic flora to the population level of commensals. In severe cases, the preservation of\u0000high titers of opportunistic flora, complete absence of commensal one and emergence of pathogenic microflora (Pneumococcus pneumoniae, Haemophilus influenzae, Pseudomonas aeruginosa and yeast-like fungi of the Candida type) can be seen. All patients have been observed the presence of microbiome disorders, regardless of its quantitative and qualitative changes, which are accompanied by hypertrophy of the pharyngeal tonsil.\u0000Conclusions: Dysfunction of the microbiome, i.e. changes in the quantitative and qualitative composition of microorganisms in patients with CNP, is important in the course of disease. Minor changes that are determined by a reduced level of commensal flora are associated with the mild disease course. If there is a significant reduction in the levels of commensals and emergence of opportunistic flora, the moderate/severe disease course can be observed. Despite these reasons, patients with acute pharyngitis and tonsillitis who seek antibiotic therapy want to reduce their symptoms. Therefore, the search for agents that will significantly improve patients' quality of life during acute pharyngeal diseases is crit ical.\u0000Aim: To review the literature on the therapeutic efficacy of topical biclotymol-based products for treating acute pharyngeal diseases.\u0000Results and discussion: Most cases of acute pharyngitis should be treated with over-the-counter medicines that are primarily intended to control the symptoms of the disease and improve the patient's quality of life. Biclotymol, enoxolone and lysozyme are novel substances that, at the same time, are safe for resolving pain in viral or bacterial pharyngit is. Biclotymol can be used as monotherapy and in combination with other agents. In the analysed data, biclotymol showed its therapeutic efficacy and safety.
