Mertes","doi":"10.1016/j.jeurea.2022.01.001","DOIUrl":"10.1016/j.jeurea.2022.01.001","url":null,"abstract":"<div><h3>But</h3><p>Évaluer à distance l’impact de la formation aux techniques ultrasonores en anesthésie réanimation (TUSAR) en inter région Est.</p></div><div><h3>Matériel et méthode</h3><p>Nous avons réalisé une étude descriptive entre le 05 mai 2014 et le 16 février 2015. Un questionnaire anonyme a été adressé à tous les participants au diplôme universitaire (DU) et au diplôme interuniversitaire (DIU) du TUSAR de 2008 à 2013 inclus, initialement par courrier puis par courriel via un lien renvoyant vers une version online.</p></div><div><h3>Résultats</h3><p>Cent soixante-deux personnes ont répondu sur 302 participants (53,6 %). Parmi eux 80 étaient inscrits au DU, 46 au DIU option échographie cardiaque et 36 au DIU option ALR. Les participants ont jugé favorablement l’enseignement théorique et pratique, pour la partie locorégionale (ALR) comme pour la partie échocardiographie. Après la formation, on constate une progression du nombre de participants pratiquant l’ALR échoguidée tout bloc confondu, une augmentation du nombre de blocs réalisés et un taux de réussite significativement plus important (<em>p</em> <!-->&lt;<!--> <!-->0,0001). 81,44 % des participants utilisaient l’échographie trans thoracique après le TUSAR (versus 30,39 % avant), avec un élargissement des indications. À l’issue de la formation, 18/116 participants ne pratiquaient pas d’ALR échoguidée et 18/97 ne pratiquaient pas d’échocardiographie.</p></div><div><h3>Conclusion</h3><p>Les participants ont eu globalement un bon ressenti de la formation et leurs pratiques ont évolué. Les objectifs ont été atteints pour la plupart des participants, aussi bien pour la partie ALR que pour la partie échocardiographie.</p></div><div><h3>Objective</h3><p>To assess the impact of remote training in ultrasound techniques in anesthesiology and critical care medicine in the French north-eastern sector currently stand.</p></div><div><h3>Materials and Methods</h3><p>We realised a descriptive study between May 5, 2014 and February 16, 2015. An anonymous questionnaire of 34 items was sent to all participants in the university diploma (DU) and the Inter-University Diploma (DIU) to ultrasound techniques in anesthesiology and critical care medicine from 2008 to 2013 inclusive, initially by letter and e-mail via a link back to an online version of the questionnaire hosted by the Survey Monkey website.</p></div><div><h3>Results</h3><p>One hundred sixty-two people responded. They represented 53.6% of all participants. Among them 80 were enrolled in the university diploma, 46 in inter university diploma option echocardiography and 36 option regional anesthesia. Participants considered favorably theoretical and practical part for the regional anesthesia as with echocardiography party. It puts into perspective the historical reluctance and the attempts to modify the law of 86. Trois situations peuvent se présenter : l’accouchement est imminent, en cours ou déjà réalisé. Le médecin régulateur se doit de déterminer l’imminence de l’accouchement. Il peut s’aider de scores connus tels que le score de Malinas modifié et le score prédictif de l’imminence de l’accouchement. Le signe le plus pertinent reste malgré tout l’envie de pousser. Ce seul signe peut amener à engager une Unité mobile hospitalière (UMH). Les critères de gravité fœtale ou néonatale doivent être recherchés, comme une prématurité, un retard de croissance, un liquide amniotique méconial. Ces éléments doivent permettre d’engager une UMH pédiatrique avec incubateur. Le médecin régulateur détermine la présence de facteurs de risque de l’hémorragie du post-partum. Il recherche donc des antécédents d’hémorragie du post-partum, une grossesse gémellaire ou une macrosomie, une grande multiparité, un âge supérieur à 35 ans et un travail long. En cas de naissance réalisée, il évalue l’adaptabilité du nouveau-né à la vie extra-utérine et veille à la prévention de l’hypothermie. Le médecin régulateur doit, ensuite, suivant le bilan du premier effecteur, organiser le transfert vers la maternité la plus adaptée : soit celle du suivi initial, soit la plus proche ou celle possédant un plateau technique adapté en cas d’hémorragie du post-partum ou une maternité adaptée à l’âge gestationnel (maternité de type I, IIA, IIB, III). Trois situations nécessitent un transfert immédiat vers un bloc obstétrical : l’imminence de l’accouchement sur un placenta recouvrant, la procidence du cordon ou du bras.</p></div><div><p>Calls for unexpected birth out-of-hospital represent 0.3% of calls to the center 15. Pour exemple, le tramadol est consommé en Belgique (population de 11 507 163 habitants) par un peu moins de 1 000 000 de personnes dont 300 000 avec une utilisation chronique (début de prise supérieure à 365<!--> <!-->jours. Leur distribution est pourtant sujette à une prescription médicale en Belgique. En France, la venlafaxine est la 3^e molécule la plus fréquemment utilisée pour le traitement d’une dépression. Nous présentons dans cet article un cas d’intoxication sévère avec interaction médicamenteuse entre ces deux molécules. Ce cas rappelle que les intoxications polymédicamenteuses peuvent présenter des tableaux rendus complexes par l’interaction entre les molécules ingérées. Après une intoxication poly-médicamenteuse volontaire, la patiente a présenté initialement un tableau neurologique sévère avec altération sévère de la conscience attribuée à un syndrome sérotoninergique et un choc cardiogénique nécessitant un support hémodynamique important pendant 48<!--> <!-->heures. La récupération fut complète sans séquelle tant au niveau neurologique que des autres fonctions.</p></div><div><p>Tramadol and venlafaxine uses are common in the Belgian population. For example, tramadol is used in Belgium (with a total population of 11,507,163 people) by a little less than 1,000,000 people among whom 300,000 with a chronic consumption (start of the therapy longer than 365<!--> <!-->days). Their distribution is linked with a medical prescription. In France, venlafaxine is the 3rd most common drug used for treatment of depression. We present a case of hard intoxication with drugs interactions between those two molecules. This case reminds us of the fact that difficult clinical presentation may occur due to the interaction between the ingested drugs. After a voluntary multiple drug intoxication, the patient first showed a neurological picture with alteration of her conscience attributed to a serotoninergic syndrome and a cardiogenic shock for which hard hemodynamic support was needed for 48<!--> <!-->hours. Le scanner cérébral était sans particularité et le bilan biologique révèle une hypokaliémie à 2<!--> <!-->mmol/l avec hyperchlorémie à 118<!--> <!-->mmol/l, une acidose métabolique avec un taux de bicarbonates à 13<!--> <!-->mmol/l. L’apport parentéral du potassium a permis une récupération complète des fonctions motrices, le traitement a été complété par l’adjonction d’un bolus de corticoïdes. Sur la base de cette observation, nous discuterons de la pathogénie de l’hypokaliémie profonde et son retentissement au cours de maladies auto-immune (lupus érythémateux disséminé avec syndrome Gougerot Sjögren).</p></div><div><p>A 50<!--> <!-->years old patient with systemic lupus erythematosus complicated by secondary Gougerot Sjögren syndrome for 10<!--> <!-->years with multi-visceral involvement, admitted to the emergency department of the HMIMV for flaccid tetraparesis. The cerebral scanner was without particularity and the biological assessment reveals an hypokalemia at 2<!--> <!-->mmol/l with hyperchloremia at 118<!--> <!-->mmol/l, a metabolic acidosis with a level of bicarbonates at 13<!--> <!-->mmol/l. The parenteral intake of potassium allowed complete recovery of motor functions, the treatment was supplemented by the addition of a bolus of corticosteroids.
