Moissonnier","doi":"10.1016/j.anicom.2022.08.001","DOIUrl":"10.1016/j.anicom.2022.08.001","url":null,"abstract":"<div><p>Cet article décrit un cas de prise en charge chirurgicale d’un neurotmésis sciatique au moyen d’une neurosynthèse par entubulation veineuse (greffon jugulaire) chez une chienne Griffon Bleu de Gascogne de deux ans présentée en consultation à la suite d’un traumatisme de chasse. Un neurotmésis correspond à une section nerveuse où le périnèvre est rompu. On en distingue deux types dans la classification de Sunderland, le premier où l’épinèvre est intact (type IV) et le second où il est rompu (type<!--> <!-->V). La prise en charge chirurgicale par suture du nerf sciatique améliore considérablement la récupération fonctionnelle du membre mais est peu décrite en médecine vétérinaire. Cet article souligne les éléments cliniques permettant le diagnostic de neurotmésis sciatique, les examens complémentaires à réaliser, puis décrit la technique chirurgicale utilisée. Sont ensuite discutées les différentes options thérapeutiques possibles et leur pronostic.</p></div><div><p>This article describes a clinical case of surgical management of a sciatic neurotmesis by means of neurosynthesis through venous entubulation (jugular vein graft) on a two-year-old Griffon Bleu de Gascogne dog presented following a hunting trauma. A neurotmesis corresponds to a nerve section where the perineurium is ruptured. There are two types in the Sunderland classification, the first where the epineurium is intact (type IV) and the second where it is ruptured (type<!--> <!-->V). Surgical management by suturing the sciatic nerve considerably improves functional recovery of the limb but is rarely described in veterinary medicine. This article outlines the clinical elements leading to the diagnosis of a sciatic neurotmesis, the further exams to propose, and then describes the surgical technique used. {"title":"Congenital trans-mesenteric hernia as a cause of small intestine strangulation in a dog","authors":"G. Redolfi , J.G. Grand","doi":"10.1016/j.anicom.2023.01.005","DOIUrl":"10.1016/j.anicom.2023.01.005","url":null,"abstract":"<div><p>A 1-year-old, 5-kg, intact male Lhassa Apso dog was presented to the author's institution with a 10- days history of dysorexia, vomiting and diarrhea. The patient did not have any history of former disease, previous abdominal surgery, or trauma. Abdominal ultrasonographic examination revealed intestinal obstruction. Exploratory laparotomy was subsequently performed. A loop of jejunum was incarcerated through a 10-cm in diameter mesenteric defect with secondary intestinal strangulation and concomitant partial mesenteric torsion. This was reduced without the need for intestinal resection. Closure of the mesenteric defect was performed using an omental pedicle flap elevated from the left gastroepiploic artery. The dog made an uneventful recovery. At the last telephone follow-up 8 months after surgery, the dog was in excellent physical condition with no recurrence of clinical signs. This is the first reported case of congenital trans-mesenteric hernia in a dog.</p></div><div><h3>Key Clinical Message</h3><p>Trans-mesenteric hernia may be associated with intestinal entrapment leading to intestinal obstruction and potential strangulation. This highlights the need for an early diagnosis and surgical intervention.</p></div><div><p>Un chien Lhassa-Apso mâle entier de 1 an et de 5<!--> <!-->kg est présenté pour dysorexie, vomissements et diarrhée depuis 10<!--> <!-->jours. Le patient est en ordre de vaccination et de vermifugation. Aucun antécédent de traumatisme ou de chirurgie est rapporté. L’échographie abdominale révèle une obstruction intestinale. Une laparotomie exploratrice est réalisée. Une anse de jéjunum est incarcérée au travers d’un défaut du mésentère de 10<!--> <!-->cm de longueur avec une strangulation intestinale secondaire et une torsion mésentérique partielle concomitante. La hernie et la torsion mésentériques sont réduites sans nécessité d’une résection intestinale. La fermeture du déficit mésentérique est réalisée au moyen d’un lambeau omental pédiculé basé sur l’artère gastro-épiploïque gauche. Aucune complication post- opératoire est rapportée. Lors du dernier contrôle téléphonique à huit mois postopératoire, le chien présente un excellent état général sans récidive des signes cliniques. Il s’agit du premier cas rapporté de hernie trans-mésentérique congénitale chez un chien.</p></div><div><h3>Message clé</h3><p>Une hernie trans-mésentérique peut être associée à une incarcération intestinale conduisant à une obstruction et une strangulation potentielle. Fast and noninvasive, it enables an early detection of some dog's cardiac abnormalities, as arrhythmias and murmurs, in less than 60</span> <!-->seconds and in normal examination conditions, without hair shaving, wireless, without clamps or sticky electrodes and without immobilization. It combines 2 essential signals in cardiology, keeping their natural synchronicity: electrocardiogram and phonocardiogram. Recordings were obtained in 44 dogs from 30 different breeds, with age ranging from 2,5 months to 19 years and weight from 2<!--> <!-->kg to 36<!--> <!-->kg, in thin and obese individuals with different types of hair, in healthy animals or dogs presenting with different cardiac diseases. Very satisfying results were achieved: the device has many experimental assets to perform a cardiac examination on a dog with no stress induced, by easy and fast recording in every type of dog and enabling to precisely measure the heart rate and to detect rhythm disorders from good quality electrocardiograms, according to age and weight of dogs.</p></div><div><p>Cet article introduit un nouveau dispositif médical intelligent, mobile et connecté : Bimod Vet®. Rapide et non invasif, il permet la détection précoce de certaines anomalies cardiaques chez les chiens, comme les arythmies et les souffles, en moins de 60 secondes et dans des conditions d’examen normales, sans tonte, fils, pinces ou électrodes adhésives ni immobilisation de l’animal. Il combine 2 signaux cardiaques essentiels, en conservant leur synchronicité naturelle : l’électrocardiogramme et le phonocardiogramme. Des essais ont porté sur 44 chiens de 30 races différentes, âgés entre 2,5 mois et 19 ans, de poids compris entre 2<!--> <!-->kg et 36<!--> <!-->kg, avec état d’embonpoint variable, pelages différents, sains ou présentant différentes maladies cardiaques.
