{"title":"[Features of general anesthesia during dental interventions in children with dystrophic epidermolysis bullosa].","authors":"A S Tishchenko, A S Dobrodeev","doi":"10.17116/stomat202510401163","DOIUrl":"10.17116/stomat202510401163","url":null,"abstract":"<p><strong>Objective: </strong>To summarize and present the experience of general anesthesia with nasotracheal intubation for children suffering from dystrophic epidermolysis bullosa (DBE) during dental interventions on an outpatient basis.</p><p><strong>Materials and methods: </strong>In 2023, 18 children with DBE received dental treatment under general anesthesia, two patients underwent general anesthesia twice a month with a break due to the large volume of intervention.</p><p><strong>Results: </strong>The paper highlights the main features of preoperative examination of this category of patients, preparation for monitoring, induction of anesthesia, and ensuring patency of the upper respiratory tract. The practical recommendations and techniques mentioned in the article allow the anesthesiologist to minimize the risks of damage to the skin and mucous membrane of the mouth and pharynx. The duration of anesthesia ranged from 50 to 225 minutes, the average duration of anesthesia was 135 minutes. The observation time in the wake-up ward ranged from 105 to 360 minutes, the average time of postoperative observation was 177 minutes. {"title":"[Improving the symptomatic diagnosis of mesial occlusion - a stage of differential diagnostics of dentofacial anomalies].","authors":"Yu M Malygin, A A Bednyakov, S I Abakarov, A V Basov, A M Malygin, A S Abakarova","doi":"10.17116/stomat202510401142","DOIUrl":"10.17116/stomat202510401142","url":null,"abstract":"<p><strong>The aim of the study: </strong>Was to improve the quality of symptomatic diagnostics of mesial occlusion.</p><p><strong>Materials and methods: </strong>The object of our epidemiological study was 2842 schoolchildren aged 6 to 18 years, and 458 conclusions of Professor Yu. M. Malygin to patients who sought consultative assistance. The analysis of the archive of diagnostic models of jaws of the Department of Orthopedic Dentistry of the Central Institute of Orthopedic Medicine and the Department of Orthodontics of the Russian Medical Academy of Postgraduate Education in the volume of 20.000 pairs and a sample of 2490 models of jaws of patients with mesial occlusion were conducted, and then an in-depth biometric study of 205 pairs of models of jaws of patients with permanent mesial occlusion was carried out.</p><p><strong>Results: </strong>Clinical and epidemiological characteristics of mesial occlusion showed the percentage of occurrence among bite anomalies 5.6+2.03; it occurs more often in boys than in girls and more often in mixed dentition than in permanent dentition. The efficiency of determining individual types of mesial bite varied from 100% to 14%, and on average the visual diagnosis was established correctly in 44.7%. A comparative analysis of 205 pairs of jaw models with mesial bite relative to the norm showed that differences in 39 parameters out of 43 were not statistically guaranteed, so the inclinations of the anterior teeth were also studied, and then 9 typical types of mesial bite were determined using the \"diagnostic key\" of Yu. M. ICCs were associated with incompletely erupted first (6 children) or second (2 children) permanent molars. Only in 5 cases did they manifest as pain in the area of the affected teeth, in the remaining cases they were radiographic findings during examination before orthodontic treatment. At the same time, the teeth in the projection of the cyst retained their vitality in all cases, which was confirmed by the cold test and continued root growth after cystectomy. On OPG, ICCs were manifested by a wide variety of localizations (in the bifurcation projection, below, distal or medial to the roots of the teeth), but CBCT made it possible to detect a common symptom - predominantly the vestibular location of the lesion. {"title":"[A case of the traumatic carotid-jugular junction of the neck].","authors":"I V Kan, N V Litvinyuk, F S Kulakov, A G Simonov, S S Mahnach, E A Bakova","doi":"10.17116/stomat202510402177","DOIUrl":"10.17116/stomat202510402177","url":null,"abstract":"<p><p>The most common cause of the arteriovenous junction between the carotid artery and the jugular vein is iatrogenic damage to the vessel walls during vein catheterization, however, this article describes a rare clinical case of traumatic carotid jugular junction of the neck. In addition to the well-known local symptoms of this pathology such as swelling, pulsation, redness of soft tissues, the formation of purulent crusts in the right eye was observed in the patient, which was not characteristic of arteriovenous anastomosis. The article presents the outcomes of multispiral computed tomography (MSCT) of the neck and brachiocephalic vessels with intravenous contrast enhancement confirming the diagnosis, as well as angiography of the common carotid artery basin. During the operation, the ligation of the vessels involved in arteriovenous discharge, the facial vein and artery, was performed. The results of surgical treatment are: reduction of swelling and redness of the soft tissues of the right buccal and suborbital areas, cessation of pulsation and formation of purulent crusts in the right eye after sleep. The article presents the main results of studying foreign literature on this topic: statistical data on the frequency and etiology of arteriovenous junction, characteristic clinical manifestations of this pathology and methods of differential diagnosis. Additionally, it lists treatment tactics used in world practice. {"title":"[TMJ features MRI study in patients with complete edentia in the absence of clinical manifestations of TMJ dysfunction].","authors":"S I Abakarov, D A Lezhnev, A V Basov, A S Abakarova, A V Starodubova, E A Yegorova","doi":"10.17116/stomat202510401150","DOIUrl":"10.17116/stomat202510401150","url":null,"abstract":"<p><strong>The aim of the study: </strong>Was to determine the signs of TMJ dysfunction in patients with complete removable dentures who did not complain of discomfort or pain.</p><p><strong>Materials and methods: </strong>The study included 264 patients with complete absence of teeth, who had complete removable dentures for at least 6 months. Two groups of subjects were formed, 20 of whom (the control group) had no clinical signs, MRI examination in the TMJ did not reveal any pathological changes, and manual examination did not reveal pathological symptoms. The remaining 244 patients formed the main group, who also did not complain of any complaints. In these patients, tooth extractions were performed 6-12 months ago.</p><p><strong>Results: </strong>In the main group, it was determined that only 36 patients (15%) presented complaints when using complete removable dentures, and 224 patients (85%) had no symptoms, and crunching and clicking in the TMJ, accompanied by discomfort and minor pain were observed in 7 patients (20% of those presenting complaints). According to the results of the MRI study, it was determined that all 56 patients regardless of gender have an asymmetry in the shape of the articular heads and their sclerotic changes, 44 patients with complete edentia showed a posterior displacement of the joint heads and a narrowing of the joint space. At the same time 46 of the 56 patients were over 70 years old and used dentures for more than 4 years, a visual examination determined a decrease in the height of the lower part of the face, including due to the wear of artificial teeth installed in the denture.</p><p><strong>Conclusion: </strong>Thus, it has been determined that a significant number of patients using complete removable dentures have dysfunctional disorders in the temporomandibular joint, which are detected only by special examination methods. {"title":"[Features of the temporomandibular joint morphology and function in the adults with different growth patterns].","authors":"Y I Afanasyeva, N Yu Oborotistov, S N Ermolev, S V Tekucheva","doi":"10.17116/stomat202510401129","DOIUrl":"10.17116/stomat202510401129","url":null,"abstract":"<p><strong>The aim of the study: </strong>Was to improve methods for diagnosing the morphofunctional state of the TMJ in individuals with normal occlusion with various types of facial skull growth.</p><p><strong>Materials and methods: </strong>The study comprised 35 volunteers aged 18-45 years (average age 25.6±4.8 years) with normal occlusion and intact dentition. Telerentgenography (TRG), cone beam computed tomography (CBCT), magnetic resonance imaging, and ultrasound examination of the TMJ were performed.</p><p><strong>Results: </strong>Horizontal type of growth of the facial part of the skull was established in 13 people; 12 had neutral type and 10 had vertical type. According to CBCT data, in patients with horizontal growth type, the highest values of the parameters of the width of the articular head, the smallest parameters of the length of the condyle process and the size of the articular gap in the upper-anterior and upper-posterior sections were determined. In patients with the vertical type, an inverse correlation of values was established, which may be due to the anatomical features of the TMJ structure: the articular head is narrower and longer, and the articular gap is wider. According to MRI data, in all patients, regardless of the type of growth of the facial part of the skull, the posterior thickening of the articular disc was located at «11-12 hours» mark in the sagittal plane, and the poles of the articular disc did not extend beyond the poles of the articular head of the same name in the coronal plane. According to ultrasound data, the indicators of the size of the articular gaps were fully consistent with the conclusions obtained from CT and MRI. Group 1 consisted of 5 patients with the diagnosis of «Upper micrognathia», group 2 included 5 patients with «Upper micrognathia after a series of surgeries for unilateral cleft lip and palate» and group 3 comprised 5 patients with diagnosis of «Upper micrognathia after a series of surgeries for bilateral cleft lip and palate». Movement of the maxillary fragments was measured 3 and 6 months after activation using cone beam computed tomography and intraoral scanner data. The data was analyzed using specialized software Amira 5.4.5 to create a 3D-model of the upper jaw. This allowed for precise measurement and analysis of the movement of the maxilla over time.</p><p><strong>Results: </strong>Despite the initial goal of surgically assisted maxillary expansion being to correct transverse deficits it has been found that spatial changes occur in all planes and axes. Patients with upper micrognathia resulting from bilateral cleft lip and palate are more likely to experience recurrence of displacement of the fragments of the upper jaw following expansion, compared to patients with a unilateral cleft during the postoperative period. The results of two-factor analysis of variance confirmed a statistically significant dependence of the average values of O and C content in enamel over the centuries (<i>p</i><0.0001), the average values of P, Zn, O and Mg in dentin (<i>p</i><0.0001), the average values of O and C content in cement (<i>p</i><0.0001). {"title":"[Retrospective evaluation of inferior alveolar nerve neuropathies after impacted third molar surgery with mandibular canal close position].","authors":"A V Kuzin, A P Vedyaeva, V V Sogacheva, G V Remizov, A I Potapova","doi":"10.17116/stomat202510402137","DOIUrl":"https://doi.org/10.17116/stomat202510402137","url":null,"abstract":"<p><strong>The aim of the study: </strong>Was to reduce the risks of surgical trauma to the inferior alveolar nerve by considering individual topographic and anatomical features, improving diagnostic methods and techniques for removing impacted third molars with close location of the mandibular canal.</p><p><strong>Material and methods: </strong>An analysis of cases of postoperative paresthesia of the lower lip and teeth was conducted in patients with trauma to the inferior alveolar nerve after surgery to remove an impacted tooth in the long term.</p><p><strong>Results: </strong>The frequency of damage to the inferior alveolar nerve after removal of an impacted tooth with close location of the mandibular canal is 18.2%. After 3 months, restoration of the function of the damaged nerve is observed in 84%. Permanent paresthesia lasting a year or more was detected in 4 patients (1.4%).</p><p><strong>Conclusion: </strong>The main causes of nerve trauma during surgery to remove an impacted tooth were identified.
