{"title":"[Relationships between the inferior wall of maxillary sinus and the roots of the maxillary molars].","authors":"R S Mekhtiev,&nbsp;Y M Melnichenko,&nbsp;S L Kabak,&nbsp;N A Savrasova,&nbsp;V V Zatochnaya","doi":"10.17116/stomat202310201152","DOIUrl":"https://doi.org/10.17116/stomat202310201152","url":null,"abstract":"<p><strong>Objective: </strong>The aim of the study is to evaluate the position of the roots of the first and second upper molars relative to the bottom of the maxillary sinus (maxillary sinus) according to cone-beam computed tomography (CBCT).</p><p><strong>Material and metods: </strong>The CBCT scans of 150 patients (69 men and 81 women) from the database of the X-ray department of the 11th City Clinical Hospital of Minsk who applied for dental care were studied. There are 4 variants of vertical relationships between the roots of the teeth and the lower wall of the maxillary sinus. At the contact of the molar roots and the bottom of the HPV, 3 variants of the horizontal relationship between the roots of the teeth and the bottom of the maxillary sinus in the frontal plane were identified.</p><p><strong>Results: </strong>The root apices of the maxillary molars can be located below the level of the MSF (type 0; 16.69%), contact with MSF (types 1-2; 72%) or protrude into the sinus cavity (type 3; 11.31%) at a distance of up to 6.49 mm. The roots of the second maxillary molar showed greater proximity to the MSF than the first molar roots and more often protruded into the maxillary sinus. The most common type of horizontal relationship between the molar roots and the MSF, in which the lowest point of the MSF was located centrally between buccal and palatal roots. It was found that the proximity between the roots and the MSF correlates with maxillary sinus vertical dimension. Two groups of patients were formed, depending on the type of surgery and the degree of trauma of the operation. Group 1 - low degree of injury - 28 patients; group 2 - high degree of injury - 23 patients. Patients independently filled out questionnaires at four stages of the study: before surgery, on the 1st, 3rd, and 5th days after surgery as part of scheduled visits.</p><p><strong>Results and discussion: </strong>In patients of group 2, the severity of pain symptoms was higher than in patients of group 1 in terms of pain symptoms on SF-MPQ-2 at all stages of the study, except the day before surgery. At the same time, there were no differences with the indicators of the VAS scale at any of the stages of the study. The most informative indicators of SF-MPQ-2 are the total indicator MPQ-Sum and the constant pain indicator MPQ-Const, which were significantly higher in group 2 on the 1st day after surgery, taking into account the correction for gender and age. Satisfaction was studied across 10 domains using a questionnaire. The average values of the scores obtained for different specialties of the doctor were compared using a variance analysis for each of the domains. The relationship between patient satisfaction and factor characteristics - specialty and age of the doctor, gender and age of the patient or legal representative was evaluated using multivariate linear regression analysis with the calculation of regression coefficients and 95% confidence intervals (CI).</p><p><strong>Results: </strong>At least a good level of satisfaction was found in all 10 domains for doctors of all specialties. The age of the doctor was inversely related to the domains \"Communication on equal terms\" and \"Active listening\". Respondents were statistically significantly less satisfied with interaction with dental therapists, dental surgeons and pediatric dentists in all domains compared to interaction with orthodontists, except for the domain \"Prognosis\". Satisfaction did not depend on the gender and age of the patients.</p><p><strong>Conclusions: </strong>Lower satisfaction in different domains can be explained by limited time for patient admission and/or insufficient training of dentists in terms of communication with patients. We used the WHO technique (DMFT) for examining children, a hardware method for detecting foci of enamel demineralization, which were recorded using the ICDAS II system. The level of enamel resistance was determined using the enamel resistance test. Three groups of children were formed depending on the intensity of caries: group 1 - DMFT = 0 (100 people); group 2 - DMFT =1-2 (104 people); group 3 - DMFT ≥3 (104 people). {"title":"[Expert judgement of face profile].","authors":"M A Danilova,&nbsp;P V Ishmurzin,&nbsp;A M Konkova","doi":"10.17116/stomat202310204137","DOIUrl":"https://doi.org/10.17116/stomat202310204137","url":null,"abstract":"<p><strong>Objective: </strong>The aim of the study is to determinate the values of the parameters of the face profile that ensure the harmony of the architectonics of the facial composition, taking into account sexual dimorphism.</p><p><strong>Material and methods: </strong>22 independent orthodontists were invited as experts (11 men, average age is 36.8±1.2 years; 11 women, average age is 35.3±2.2 years). Experts questioning implemented the preference vectors method using randomly arranged male and female face profile schemes with variable parameters.</p><p><strong>Results: </strong>Male profile expert judgement average maximum was obtained for following indicator values: face convex angle 8°, nasolabial angle 105°, mentolabial angle - 135°, lip protrusion angle 130°, nasofacial angle 35°, projection nose length 17 mm and 19 mm, vertical ratio of upper lip height to lower lip with chin height 0.45 and 0.50. Female profile expert judgement average maximum was obtained for following indicator values: face convex angle 8° and 10°, nasolabial angle 95°, mentolabial angle 115°, lip protrusion angle 130°, nasofacial angle 35°, projection length nose - 17 mm, vertical ratio of upper lip height to lower lip with chin height 0.50.</p><p><strong>Conclusion: </strong>Facial profile attractiveness evaluation of Caucasian face profile showed gender dependence for face convex angle, nasolabial and mentolabial angles, projection nose length. Female profile compared to male profile is characterized by greater convexity, sharper nasolabial angle, marked supramental sulcus and shorter nose length. Disorders such as stress, depression, attention deficit hyperactivity disorder, alcohol dependence, eating disorders - may be risk factors for the development of increased tooth abrasion. Comorbid conditions such as bruxism and gastroesophageal reflux are also considered. Due to the high mortality of adolescents due to mental illness, early diagnosis of disorders of psychological and mental health of adolescents is extremely relevant. Attention is focused on the possibility of early diagnosis by dentists of psychological and mental problems by the presence and type of increased erasability of teeth. The main group, including 44 root canals, was filled with a new paste based on CHC and silver nanoparticles for 7 days after preparation and irrigation. In the control group, 25 root canals were sealed with an aqueous paste of calcium hydroxide for 14 days. The presence of the endodontic microorganisms was evaluated by real-time PCR.</p><p><strong>Results and discussion: </strong>Further analysis showed that the amount of the DNA, common for <i>P. gingivalis</i>, <i>T. forsythia</i> and <i>T. denticola</i>, after treatment was less in the main group, where the new paste was applied. These results were significant at the <i>p</i>=0.05 level (<i>p</i>=0.005, <i>p</i>=0.006, <i>p</i>=0.003 according to each mentioned bacterial sample). The advantages and disadvantages of such fastening systems are described. The analysis and systematization of available data on medical adhesive compositions that can be used in maxillofacial prosthetics to hold facial epitheses has been carried out. Much attention is paid to the chemical composition of pressure-sensitive adhesives, water-based and silicone-based. Examples of such compositions are given. The physicochemical properties of medical adhesive systems that cause adhesive-cohesive interaction are described.
