The aim of the study was to conduct a comparative analysis of methods for determining the optimal individual position of the mandible using a hydrostatic mouth guard and transcutaneous electrical stimulation (TENS) techniques.</p><p><strong>Material and methods: </strong>The study comprised 188 patients with musculoskeletal and articular dysfunction of the TMJ. The subject of the study is cone beam computed tomography and magnetic resonance imaging of the TMJ of these patients. Before starting treatment, all patients underwent magnetic resonance imaging of the TMJ with functional tests (in the open and closed mouth position) and seizure of the masticatory muscles.</p><p><strong>Results: </strong>It was found that the results of the therapeutic and diagnostic procedure for finding the optimal position of the lower jaw using a hydrostatic mouth guard and the TENS method are comparable to each other.</p><p><strong>Conclusion: </strong>CBCT and TMJ MRI are informative diagnostic tools that can be effectively used in the diagnosis and treatment of patients with musculoskeletal dysfunction. Sechenov a dental examination of 52 patients with liver cirrhosis of various etiologies was conducted. The main diagnosis was established based on the results of biochemical blood parameters and ultrasound examination of the abdominal organs, according to which the patients had all 3 classes of liver cirrhosis according to Child-Pugh.</p><p><strong>Results: </strong>Comparison of the periodontal index in groups depending on the severity of liver cirrhosis showed a significant difference between groups (<i>p</i><0.05): the maximum was determined in patients with Child-Pugh C (59±0.14 percentage), the minimum - with Child-Pugh A (36.4±0.13 percentage). Comparison of periodontal pocket depth (PD index) in groups depending on the degree of severity showed a significant difference between groups (<i>p</i><0.05): the maximum was determined in patients with Child-Pugh C (4.02 mm), the minimum - in Child-Pugh A (2.96 mm). The rehabilitation of patients with cleft lip and palate also depends on various other factors. To a great extent, the success of treatment depends on an integrated approach that includes both surgical interventions and subsequent rehabilitation measures. Wound healing of oral mucosal tissues has specific characteristics, and therefore, when choosing wound dressings, it is essential to consider the environment and the nature of the wound healing process. It has been observed that the correction of palatal defects in patients following a series of surgeries for clefts is further complicated by the scarring of palatal tissues. The main objectives of modern wound dressings are to protect and stimulate regenerative processes in the surgical site. T.</p><p><strong>Materials and methods: </strong>The study included works where AI was used for the analysis of dental images, with a focus on deep learning methods and convolutional neural networks. The search was conducted in six leading databases, including PubMed, IEEE Xplore and others.</p><p><strong>Results: </strong>AI significantly improves the accuracy of diagnosing dental conditions, such as caries and periodontitis, and promotes a more personalized approach to patient treatment. However, existing studies are often limited by small data sets, highlighting the need for further research.</p><p><strong>Conclusion: </strong>AI has the potential to greatly enhance dental practice, but its full integration into clinical practice requires additional studies. {"title":"[Factors of health formation in disabled children with congenital cleft lip and/or palate].","authors":"K S Tutova, A A Muzychina","doi":"10.17116/stomat202510402130","DOIUrl":"https://doi.org/10.17116/stomat202510402130","url":null,"abstract":"<p><strong>Objective: </strong>The aim of the study to determine the factors of health formation in disabled children with congenital cleft lip and/or palate.</p><p><strong>Material and methods: </strong>The data of rehabilitation records and interviews with families raising 393 children with congenital clefts of the upper lip and/or palate aged 3 to 12 years were analyzed.</p><p><strong>Results: </strong>Registration of disability status was untimely in 57.25% of children with congenital palate, in 12.05% of children with congenital cleft lip and/or palate, and in 11.46% of children with congenital cleft. The late registration of the relevant documents on the child's status in 3.56% of parents was influenced by reluctance to consider their child disabled for psychological reasons, in 9.16% of cases - by low qualification of a pediatrician/neonatologist, in 4.33% - by untimely diagnosis, in 65.9% - by ignorance of the possibility to register the disability status, in the remaining 17.05% of cases - by organizational reasons.</p><p><p>Among 61.83% of mothers in the maternity hospital there was no instruction and/or practical training in the peculiarities of feeding children with congenital cleft lip and/or palate. A fairly large percentage of children (47.8%) were transferred from the maternity hospital to the neonatal pathology department without vital indications due to the «impossibility» to organize their proper feeding. In the maternity hospitals of Donetsk Oblast, only 3 children out of 100 newborns with congenital cleft lip and/or palate were consulted by a maxillofacial surgeon during the analyzed period. The organization of rehabilitation of children with congenital cleft lip and/or palate in the pediatric area does not meet the standards. Only 46.06% of parents received specific and detailed medical and organizational recommendations from the pediatrician during the first week of the child's stay at the site. Children with congenital cleft palate were especially often out of the field of vision of both pediatricians and surgeons. Only 23% of children were examined by an oral surgeon in the first two months of life. 72.01% of parents were insufficiently informed on most of the issues related to treatment and rehabilitation of their child. 9.41% of the surveyed parents had no such knowledge. Only 76.08% of parents followed all or part of the recommendations of specialists on child care at home.</p><p><p>Analysis of the morbidity structure of children with congenital cleft lip and/or palate has shown that the first ranks, regardless of the type of malformation, are occupied by diseases of the blood and hematopoietic organs, respiratory diseases, diseases of the endocrine system, nutritional and metabolic disorders, and skin diseases. Among them are patients with various types of defects: a defect of only the alveolar process (3 patients, 12%), a defect of the alveolar process and half of the palatine process on the resection side (7 patients, 28%), a defect reaching the midline of the hard palate or a defect involving the tissues of the hard palate on both sides of the midline (12 patients, 48%), any defect involving soft palate tissues (3 patients, 12%). To replace the defect of the upper jaw, prostheses of various configurations were made for patients: a partial removable plate prosthesis with an obturator (13 (52%)), a complete removable upper jaw prosthesis with an obturator, including one collapsible complete removable prosthesis with an obturator (7(28%)), a clasp prosthesis with an obturator (5 (20%)).</p><p><strong>Results: </strong>It was revealed that the use of an upper jaw prosthesis with an obturator significantly improves speech recognition and reduces the number of speech defects in patients with acquired upper jaw defects. It was found that the presence of a patient with an upper jaw defect in the anamnesis of radiation therapy for an upper jaw tumor negatively affects the quality of sound reproduction. The assessment by category was performed on a rating six-point scale based on a calculated integral coefficient (IC) equal to the sum of the ratings by category.</p><p><strong>Results: </strong>The values of the Integral coefficient of the first round of estimates differ significantly, <i>p</i>≤0.001. The Duncan test identifies 9 homogeneous subsets. The estimates of tasks No. 10, 1, 9, 4, 3, 11 simultaneously form 4 groups of subsets for which a second round of evaluation is made. The integral coefficient of the points of the second round of the expert assessment is significantly different, <i>p</i>≤0.001. The Duncan test identifies 2 homogeneous subsets. The experts rated task No. 3 significantly higher (M=10.9). The integral coefficient of the third round of assessments of tasks 3.12.13 is significantly different, <i>p</i>=0.023, «reasonableness» (<i>p</i>=0.815) and «clarity» (<i>p</i>= 0.082) do not differ, the \"complexity\" of task 13 is higher than that of task 3 (<i>p</i>=0.026), differences in the \"complexity\" of tasks 3 and 12 (<i>p</i>=0.209), 12 and 13 (<i>p</i>=0.383) no. In the first round, the reasonableness (3.23±1.34 points), complexity (3.15±1.26), clarity (3.04±1.15) of the tasks did not significantly differ (0.055≤<i>p</i>≤0.422). In the second round, the reasonableness (2.98±0.89) of the tasks is higher than their complexity (2.52±0.86) and clarity (2.54±0.74), (<i>p</i>=0.005 and <i>p</i>=0.003), but the complexity and clarity of the tasks are the same (<i>t</i>=0.18, <i>p</i>=0.860). Reasonableness (5.67±0.48), complexity (5.38±0.49) and clarity (5.62±0.49) do not differ in the third round (0.058≤<i>p</i>≤0.763).</p><p><strong>Conclusion: </strong>The situational tasks selected in the study are distinguished by an integral coefficient of validity, complexity and clarity higher than other evaluated tasks, which allows for a multi-level assessment of the knowledge of orthodontists in the management of patients with combined dental anomalies. In dentistry, local anesthesia is more often used, which minimizes pain and has fewer side effects. One of the key issues in anesthesiology is the effectiveness of anesthesia, which depends on many factors, including the type of surgery, the physiological characteristics of the patient, and economic components. The main sources of information for this work were scientific articles published in peer-reviewed medical journals. It was noted that the elements of assistance system for the patients with maxillofacial wounds and their evacuation, as well as the proposed therapeutic methods used during the Great Patriotic War, had previously undergone detailed testing during the battles at Lake Khasan (1939) and the Khalkhin Gol (1939) and during Soviet-Finnish (Winter) war (1939-1940).The special activity of the medical service during the Great Patriotic War was evident in connection with the provision of medical care to the population and treatment at all stages, during which medical and medical measures were taken from the conditions of the military and health situation. Author presents data on the treatment of maxillofacial wounded in the army, military, front-line and inland regions. The author describes the success in the development and use of new methods for restoring the shape and function of damaged facial organs, which reduced the treatment and rehabilitation periods.
