This appearance is attributed to the stenotic jet stream, which is preferentially directed towards the left pulmonary artery and which results in focal post-stenotic dilatation where the jet impacts the left pulmonary arterial wall. In some cases, enlargement of the main pulmonary artery can also be appreciated, as well as right ventricular enlargement in severe cases. Initially, trepidation to the use of skin puncture testing (SPT) occurred because of reported morbidity and mortality. SPT has most frequently been utilized in the specialist office due to fear of adverse reactions. \u0000Methods: Analysis of 156,335 SPT tests (morbidity and mortality) allowed equivalency testing versus superiority testing to show 95% confidence level. The ninety-five percent confidence interval around the 11.5/100,000 SPT is (11.3-11.7/100,000). \u0000Results: The literature documents 1 death from SPT from 1990 to 2001 and 1 death from 1945 to 1987. These 2 reported deaths have been contrasted with 0 deaths in 125,295 SPT patients reviewed in a primary care setting utilizing United Allergy Service protocols (UAS). Systemic reactions (SR) occurred in only 18 patients. The SR rate amongst subspecialists has been reported as a 0.015-0.023% SR per 100,000 SPT. Statistically, this result is not significantly different from the UAS SR rate of 0.0115% without associated anaphylaxis or deaths. \u0000Conclusion: Based on the findings evident in this study, SPT is safely performed in the primary care (PC) office and allows for a larger percentage of the symptomatic population to be assessed. The observed SR rate of 11.5/100,000 SPT was statistically similar to the lowest published rate of 15/100,000 SPT (p=0.45) and statistically lower than the highest published rate of 23/100,000 SPT (p=0.02). It has not been well documented in the other parts of the country. The objective of this study was to describe the pattern of acute poisoning in a Tertiary Care Teaching Hospital in urban Sri Lanka to describe the difference of poisoning patterns in different areas of a country. \u0000Methods: This study was performed as a retrospective study in Colombo South Teaching Hospital (CSTH), Sri Lanka. The bed head tickets (BHTs) of the patients admitted to the medical units of CSTH from 1st January 2009 to 31st December 2009 were retrieved. Patients admitted to pediatric units and with snake bites and food poisoning were excluded. \u0000Results: From 1st of January 2009 to 31st of December 2009, 92668 patients were admitted to CSTH and 1.27% (1178) was admitted following acutehttps://www.omicsonline.org/open-access/current-trends-of-poisoning-an-experience-at-a-tertiary-care-hospitalhadoti-region-rajasthan-india-2161-0495-1000298.php?aid=74588www.omicsonline.org/open-access/current-trends-of-poisoning-an-experience-at-a-tertiary-care-hospitalhadoti-region-rajasthan-india-2161-0495-1000298.php. Of those patients we were able to collect data on 70.46% (828). Male to female ratio was 37:63. The commonest cause of poisoning was due to medicinal drug overdose 63.52% (526) of which paracetamol overdose accounted for 37.6% (311). Organoposphate (OP) accounted for 11.11% (92) of the admissions. Thus innovation is required to bring new leaders for our specialty, something that the private medical market is asking for. In this paper, the authors relate their experience in two different subjects. First, how to involve medicals students in the family medicine scenario. The value of exposing medical students to the real world of family practice (particularly in a country with little formal medical school experience in the discipline), demonstrates the broad perspective family doctors have and the basket of services they could be in charge. Students realize how family medicine core values, when put into practice, are good resources to become better physicians. Despite the specialty they will choose in the future, they consider family doctors as remarkable teachers who make the difference in their education. Second, how to prepare the young doctors who join SOBRAMFA staff for the growing opportunities in private practice in Sao Paulo (Brazil). In Ethiopia majority of married women practiced predominantly short acting contraceptive methods, whereas the proportion of women who were using long acting and permanent contraceptive methods were only 4%. \u0000Method: A community based cross- sectional study design was used . Multi stage sampling procedure was used to select 802 married women. Data collection was carried out from April 10 to April 25, 2014 using a pre- tested structured questionnaire. The data were entered into a computer using Epi-info 3.5.1 and then exported to SPSS for Windows version 20 for analysis. Bivariate and multivariate logistic regression analysis were done, odds ratio and 95% confidence interval were calculated. \u0000Result: Intention to use long acting and permananet contraceptive methods was 18.2%. The majority of women intended to use implant (51.4%). The finding showed that a significant positive association between intention to use long acting and permanent contraceptive methods and women’s education (AOR=1.82, 95%CI = 1.09 – 3.04), women’s occupation (AOR = 2.56, 95% CI = 1.47-4.46), joint fertility related decision (AOR = 2.76, 95% CI: 1.40-5.42), and discussion with health care provider about long acting and permanent contraceptive methods (AOR = 2.08, 95% CI: 1.40-3.09). \u0000Conclusion and Recommendation: In this study prevalence of intention to use LAPMs was (18.2%). Thus program manager and stakeholders should empower women through education, create awerness on LAMPs and adress misconceptions and fears of clients on LAPMs . Diagnosis may be influenced by preconceived notions of prevalence that are based upon personal experience fund of medical knowledge. External ear canal folliculitis (herein EECF), is a localized skin infection of the external ear canal involving hair follicles, typically caused by Staphylococcus aureus. It is our clinical experience an underappreciated diagnosis and distinct in character from otitis external. Our research is a prospective observational prevalence trial, analyzing 90 visits of adults with earache as the presenting complaint, excluding patients that recently received antibiotics. 45.6% of included cases represented EECF. The results reveal a very simple finding: earache in adult patients derives most frequently from focal inflammation, what we call EECF. We investigated the outbreak to identify the source, propose control and preventive measures. \u0000Materials and methods: We defined a case as occurrence of three or more watery stools with or without vomiting, w.e.f 27 January, till 2nd Feb2014 in residents of Ghallour sub-centre area of Jawalamukhi Block. We hypothesized it as a water borne outbreak. We determined age and sex specific attack rate. We collected information about personal history on age, sex, residence, and date of onset of symptom from case patient and established line list. We collected 9 stool samples/rectal swabs from the cases; 3 pre and 4 post chlorination water samples and sent them for microbiological testing. We mapped water supply pipelines of the area and reviewed water chlorination record. \u0000Results: We identified 390 cases. The overall attack rate was 8% with range of 1.3% to 36%. The cases patients were reported from all age groups and both the genders with the youngest student; 1 year old to the oldest one as 88 years. Two thirds of the cases reported vomiting with some complaining of pain abdomen. Villages like Jathman, Khattni, Rajol Patta, Kohara has 100% case patients of loose motions while villages like Dhroli, (100%), Sasan (63%) and Ghallour (50%) etc have combined symptoms of loose motions and vomiting. No fatality was reported. Escherichia coli was detected in the samples from water samples. Records of chlorination were not properly maintained. \u0000Conclusion/recommendation: The outbreak was caused due to contamination of water at the source in Jolly Khad. Khat is an evergreen shrub or tree found growing wild or cultivated in the east of a region extending from Southern Africa to the Arabian Peninsula. The habit of khat chewing has prevailed for centuries in this part of the world. \u0000Objective: To summarize the potential adverse health effects of khat (Catha edulis). \u0000Methodology: We used 57 different published materials for the compilation of this review article. Google search engine was used for accessing published materials from databases like Google Scholar, PubMed and HINARI. \u0000Result: Khat induces disturbance of mood, hallucination, delusions and aggressive behavior. It may also cause elevation of arterial blood pressure and pulse rate with subsequent increase in cardiovascular risk. In addition, khat may be associated with increased risk of carcinoma of the mouth and oesophagitis, anorexia and constipation. It may also have a toxic effect on the liver and kidneys. \u0000Conclusion: Many literatures reported that Khat has potential adverse health effects on organs and systems of the body. Alenzi","doi":"10.4172/2327-4972.1000155","DOIUrl":"https://doi.org/10.4172/2327-4972.1000155","url":null,"abstract":"Background: This study was designed to evaluate the health-related quality of life (HRQoL) of Saudi women who had undergone different types of delivery and to identify factors that significantly affect the HRQoL on these women. \u0000Materials and Methods: A comparative cross-sectional study was performed at Ministry of health (MOH) centers - Riyadh, Saudi Arabia. The Medical Outcome Study Short-Form 36-item survey (SF-36) was used to assess HRQoL. For comparison, the HRQoL in an equal number of healthy individuals was investigated; multivariate analysis of variance was used for comparisons between groups. \u0000Results: Results showed that delivered women (case group) had significantly higher mean scores in the physical functioning, role-physical, vitality, role-emotional and mental health subscales. As regarding body pain, delivered women (case group) reported a significantly lower mean score than controls (Comparator group). Women who underwent caesarean section had significantly worse mean scores for all HRQoL domains, except for body pain, while the normal delivery women reported the highest HRQoL scores. \u0000Conclusion: The overall test statistic was statistically significant for the eight subscales, indicating that there was a correlation between type of delivery and HRQoL. Psychiatrists should be aware of POTS because the symptoms exhibited by patients, such as tachycardia, tremulousness, fatigue, and abdominal pain, may be misinterpreted as a primary anxiety disorder, in particular panic disorder, or even as one of the somatoform disorders. \u0000Objective: The authors review literature on POTS and highlight a case of a 23-year-old female with hyperadrenergic POTS, admitted for a recurrent tachycardiac and hypertensive crisis. \u0000Method: The authors present a case report of history, diagnosis, and treatment. After being treated in the medical intensive care unit, the patient was referred to the internal medicine psychiatry service (med-psych) for management because members of her medical team hoped her symptoms would be better treated by managing her anxiety. \u0000Results: The patient had a history of multiple referrals for anxiety in spite of a known diagnosis of POTS. She also had serum catecholamines showing a normal norepinephrine while supine and comfortable, but highly elevated during a tachycardiac and hypertensive crisis. \u0000Conclusion: This case highlights the need for a broader understanding of POTS. Patients with this syndrome have a primary autonomic dysfunction driving their symptoms, though anxiety can be a component.
