Ghojazadeh","doi":"10.12816/0002647","DOIUrl":"https://doi.org/10.12816/0002647","url":null,"abstract":"Surfactant administration via thin endotracheal catheter (Tec) seems to be less invasive than InSurE method .We analyzed data obtained from multicenter hospitals. This multi center randomized clinical trial study was conducted concomitant within NICU of three university hospitals of Tabriz, Isfahan and Mashhad for a period of 20 months on neonates with gestational age of 27-32 weeks. All infants received nCPAP before administration of surfactant. If we required fio2>30%to maintain spo2 >85% then surfactant was administered. A 5f vascular catheter was placed through 1 to 2 cm below the vocal cords and surfactant was administrated slowly. Mean gestational age in TEC was 29.6 weeks and in InSurE was the same. Mean weight in TEC was 1339 grams and in InSurE was 1304 gram. Surfactant was administered to 66 preterm via TEC method and to 70 infant via InSurE. Mortality in TEC group was significantly lower than InSurE. P<0.01RR=0.56, CI, 95(0.420.76). NEC reduced in TEC. CPAP duration decreased in two centers, p<.05 but increased in another center. Fio2 decreased rapidly and HCO3 increased 2 hours after surfactant P<0.03 in TEC. The average mean of Oxygen supplementation, BPD, Mechanical ventilation between two groups was not statistically significant. TEC method was effective in treating RDS. Mortality was significantly decreased in TEC group. Its anticancer activities have been studied against Ehrlich Ascites Carcinoma (EAC) cells in swiss albino mice by monitoring the parameters like tumour cell growth inhibition, tumour weight, survival time, peritoneal cells and haematological parameters. It was found that this compound significantly reduces tumour cell growth rate, decreases tumour weight and increases life span in comparison to those of EAC bearing mice. The test compound restored the depleted haematological parameters of EAC bearing mice towards normal. The compound also enhanced number of macrophages in normal mice. However, a few recent studies have acknowledged its efficacy in certain disorders, which necessitate further scientific studies on the subject. It is also a remarkable observation that a therapy described hundreds of years ago has proved its efficacy on the most recent parameters in modern times. Physical examination revealed abdominal distention and diminished bowel sounds. Laboratory test results were as follows: sodium, 129 mEq/L (135–145 mEq/L); potassium, 4.3 mEq/L (3.6–4.8 mEq/L); blood urea nitrogen, 180 mg/dL (10–20 mg/dL); serum creatinine 3.2 mg/dL (0.4–1 mg/dL); and white blood cell count, 10,300 cells/mm3. The patient had a past medical history of hypertension and diabetes mellitus for 20 years, coronary bypass for 10 years, and Alzheimer's disease for 3 years. Plain abdominal x-ray revealed dilated small bowel loops with air-fluid levels (Figure 1). Abdomen ultrasound revealed dilated bowel loops. Abdominal computed tomography demonstrated dilated ileal loops, suggestive of ileus, and a foreign body lodged in the terminal ileum, with wall thickening (Figure 2). The patient was hospitalized and hydrated with intravenous saline with potassium supplementation. She was referred to emergency operation. Operative findings showed adhesion and strangulation of the terminal ileum. A 30-cm necrotic segment of the intestinal tract was surgically removed. Macroscopic findings were a largely dilated small bowel proximal to a napkin present in the ileum (Figure 3). Histopathological examination of the ileum revealed necrosis. Intravenous metronidazole and ceftriaxone were administered for 10 days. After treatment, blood urea nitrogen, serum creatinine, and potassium were normal. Rapid weight gain and certain conditions causing rapid weight loss, such as bariatric surgery, are among the risk factors for gallbladder stone formation [3]. Concurrent prophylactic cholecystectomy in sleeve gastrectomy (SG) remains controversial.
