{"title":"A SYSTEMATIC REVIEW OF THE EVALUATION OF NEONATAL SEPSIS DIAGNOSIS USING INTERLEUKINS","authors":"Samantha Adrianne","doi":"10.53555/nnmhs.v9i6.1726","DOIUrl":"https://doi.org/10.53555/nnmhs.v9i6.1726","url":null,"abstract":"Aim: This study aims to summarize and evaluate the diagnostic potential and accuracy of interleukins (ILs) level as a diagnostic marker for neonatal sepsis. \u0000Methods: A systematic search strategy was conducted across several electronic reference databases (PubMed, Cochrane Library, Google Scholar) and included articles published between 2019–2023. Duplicate publications, review articles, and incomplete articles were excluded. \u0000Results: Database searches identified a total of 19553 articles. Of these, 100 articles passed the screening process, resulting in 20 articles for full-text assessment. Among them, 11 articles did not evaluate the outcome of interest. Hence, we found 9 appropriate studies included. The ILs reported include IL-6, IL-10, IL-18, and IL-27, with the most common reported was IL-6. \u0000Conclusion: Current evidences supports the potential predictive utility of ILs level, especially IL-6, as a potential diagnostic marker for detecting neonatal sepsis. There is concern that borehole, sachet water and bottle water produced in developing nations may be sold without adequate monitoring, and that water treatment may be subject to contamination. The purpose of this quantitative study was to examine the level of contamination of drinking water sourced from boreholes, bottle water and sachet-packaged water around Anambra State Polytechnic, Mgbakwu, Southeast Nigeria. The main purpose of the research was to develop a coefficient of cardiorespiratory system status, which will take into account both frequency and power characteristics of external respiration and hemodynamics activity. Compare an informativeness of this coefficient with a universally accepted Hildebrandt one. \u0000Materials and Methods. 154 apparently healthy young people of both sexes were examined. On a bicycle ergometer with a pedaling rhythm of 40 per minute, five standard stages of short-term physical exercise load ranging from negligible to moderate were provided. Spirography, tonometry and pulsometry were carried out before loading, at its peak and after it. The parameters specified were recorded for each subject simultaneously. Systolic volume was calculated using Starr formula with subsequent defining cardiac minute output. \u0000Statistical analysis was performed using Statistica ® 7.0 package (StatSoft Inc., USA). Assessment of results’ significance was calculated using Student's t-test and sign test. \u0000Results. In the case of low short-term physical exercises pulmonary ventilation is increased due to greater reactivity of external breathing amplitude and to a lesser extent due to change in the frequency of respiratory movements. This type of reactions of the respiratory component in cardiorespiratory interaction is more efficient. In the given conditions, body’s adaptation is ensured by breathing component dominance, while responses of systemic arterial pressure and pulse turn out to be less pronounced, less stabile. Taking into account greater adaptive importance in power characteristics changes of respiratory and cardiovascular activity, a coefficient of cardiorespiratory system status is proposed – MBV/RMV (ratio of minute blood flow volume to respiratory minute volume). FC occurs in 95% of children without an organic reason. Lower urinary tract symptoms (LUTS) and urinary tract infection (UTI) often co-occur. The International Children's Continence Society (ICCS) coined the term bladder and bowel dysfunction (BBD) to stress the frequent combination of bladder and bowel issues. \u0000Aim: The purpose of this research is to investigate bladder symptoms in children with functional constipation. \u0000Methods: This study confirmed that it met all of the requirements by evaluating the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020 criteria. This allowed the researchers to guarantee that the study was as current as possible. Publications that were published between 2015 and 2023 were incorporated into the search strategy, which encompassed a number of different electronic reference databases (including Pubmed and SagePub). We did not consider review papers, duplicate publications, or articles that were only partially written. \u0000 Result: PubMed database search results yielded 213 articles, while SagePub database search results yielded 112 articles. In 2015, the search results for SagePub returned 13 articles while PubMed returned 19 articles. We compiled a total of eight articles (6 from PubMed and 2 from SagePub) and included three studies that met the inclusion criteria. \u0000 Conclusion: We can't say for sure how often bladder problems show up in children with FC. This is because the frequency varies a lot, so there aren't many studies. GTD is accompanied by the potentially fatal complication of hyperthyroidism. We created a review consisting of five case reports. This case report was conducted on women who became pregnant when they were >35 years old. They showed consistent laboratory results, in which there was an increase in hCG levels. TSH and thyroid stimulating antibodies cannot account for the hyperthyroidism associated with GTD. The source of the thyroid stimulating agent is trophoblastic tissue. The thyrotropic effects of ?-hCG mediate GTD hyperthyroidism. Other research examined the prevalence of hyperthyroidism in women with normal pregnancy, hydatidiform mole, and choriocarcinoma who had ?-hCG serum levels below 100,000 IU/ml. Early prenatal screening may significantly reduce hyperthyroidism rates. Thyroid hormone synthesis inhibitors treat hyperthyroidism in pregnancy. Thioamides, propylthiouracil (PTU), and methimazole (MMI) are the most prevalent U.S. antithyroid medications (ATD). Plasmaphoresis may be an option for people who are resistant to therapy or need emergency surgery. GTD can cause fatal hyperthyroidism. Anti-thyroid medicines can handle most GTD hyperthyroidism. Tinnitus is defined as the sense of sound in the absence of an obvious external source. Tinnitus is typically accompanied by hearing loss, but this is not always the case. It is conceivable for it to occur either by itself or in conjunction with a loss of hearing of any type. Tinnitus and hearing loss are both caused by a wide variety of conditions, the most frequent of which is age-related hearing loss. Other, less common causes include malignancies of the brain and skull base. Hearing loss can be broken down into three basic categories: sensorineural, conductive, or mixed (where both conductive and sensorineural components are present). The perception of sound in the absence of external acoustic stimulation for an extended length of time (more than three months) is the hallmark of chronic tinnitus. Tinnitus is quite common, with an estimated incidence of 10–15% in the adult population; however, prevalence estimates can vary widely from study to study. Tinnitus is characterized by a buzzing or ringing sound in the ear. A minority of people may experience severe psychological distress as a direct result of the situation, despite the fact that the majority of people are able to adapt to the perception and report very mild emotional distress as a result of it. Tinnitus Questionnaire (abbreviated as TQ), Tinnitus Handicap Inventory (abbreviated as THI), and Tinnitus Functional Index (abbreviated as TFI) are the three self-report questionnaires that are used the most frequently for quantifying tinnitus-related suffering. The total scores of these measures properly capture both the suffering associated with tinnitus and the change that is attributable to treatments. Because infections are typically kept under control by a robust immune response, patients typically have either no symptoms at all or only mild symptoms. Breast milk is regarded as the best diet for newborns because of the anti-infectious properties it possesses in addition to its high nutritional value; yet, breast milk can also act as a vehicle for the transmission of infectious diseases such as viruses and bacteria. Breast milk is the vehicle via which the great majority of human cytomegalovirus (HCMV)-seropositive women pass the virus on to their children. Because their mothers have antibodies that are specific to HCMV, full-term neonates who acquire infections during pregnancy normally show no symptoms and do not experience any difficulties. In contrast, preterm neonates with very low birth weight (VLBW) are more likely to develop symptomatic infections, such as neutropenia, thrombocytopenia, sepsis-like syndrome, and, less frequently, pneumonia and enteric infection. These infections are more likely to occur in VLBW premature newborns. It would suggest that infections that are acquired postnatally clear up on their own without influencing the clinical result. It is possible for it to be carried out at any point during a woman's menstrual cycle, as well as right after a child is born or an abortion is performed. Hysteroscopy, laparoscopy, or even a mini-laparotomy might be performed instead. It is important to have a conversation about the danger of regret as well as the characteristics that increase the likelihood of regret, such as a young age at the time of sterilization (less than 30 years), a lower parity, sterilization administered in the immediate postpartum period, divorce or remarriage following sterilization, and being poor or of Hispanic origin. It would appear that being a young adult at the time of the sterilization is the most significant indicator of regret. It is extremely important to keep in mind that this does not provide full protection. According to the findings of the CREST study, the failure rate across all procedures was 18.5 out of every 1000 procedures over the course of a 10-year period. {"title":"GESTATIONAL WEIGHT GAIN AND RISK OF AUTISM SPECTRUM DISORDERS IN OFFSPRING : A SYSTEMATIC REVIEW","authors":"Dea Nabila Ratu Alicia","doi":"10.53555/nnmhs.v9i5.1700","DOIUrl":"https://doi.org/10.53555/nnmhs.v9i5.1700","url":null,"abstract":"Autism, also known as Autism Spectrum Disorder (ASD), is a neurodevelopmental disorder characterized by abnormalities in social behavior, language skills, and communication abilities. A correlation was found between ASD patients and their mothers' excessive weight gain during pregnancy. However, the relationship between BMI before pregnancy and ASD incidence is inconclusive. In addition, research investigates the potential role that preventative factors, such as folate and fatty acid consumption, play in the development of the disease. There is growing evidence that certain factors, including advanced maternal and paternal age, valproate consumption, exposure to toxic chemicals, maternal diabetes, enhanced steroidogenic activity, immune activation, and possibly altered zinc–copper cycles and treatment with selective serotonin reuptake inhibitors, are associated with an increased risk of autism spectrum disorder in offspring. The large, multisite case-control study discovered that severe maternal obesity (obesity Class 2/3) was associated with approximately double the risk of ASD (with or without ID) and increased odds of DD. In the United States, an investigation was conducted. ASD was also associated with GWG when gestational age was considered, and this association was particularly strong in male offspring. Still, no connection between GWG and DD could be established. No correlations were found between the GWG rate and ASD or DD when the data was evaluated based on how well it matched clinical recommendations. Patients with autism spectrum disorder whose mothers gained an unhealthy quantity of weight during pregnancy were found to have a correlation. {"title":"LETROZOLE COMPARED WITH CLOMPIHENE CITRATE FOR POLYCYSTIC OVARIAN SYNDROME : A SYSTEMATIC REVIEW","authors":"Dea Nabila Ratu Alicia","doi":"10.53555/nnmhs.v9i5.1701","DOIUrl":"https://doi.org/10.53555/nnmhs.v9i5.1701","url":null,"abstract":"According to the Consensus on the Management of PCOS, polycystic ovary syndrome, is an endocrine condition that produces oligo-anovulation, clinical and biochemical symptoms of hyperandrogenism, and distinctive ovarian morphological characteristics on ultrasonographic examination. PCOS is also associated with a higher risk of endometrial cancer. Both hyperandrogenism and anovulation are difficult conditions to manage. PCOS women often have higher GnRH pulsatility, which in turn causes increased pituitary LH secretion and an elevated LH/FSH ratio. Granulosa cells are responsible for the conversion of androgens to estrogen and the maturation of follicles, while LH is responsible for promoting androgen synthesis in theca cells. Intraovarian androgens produce atresia in later antral stages, although they do boost initial follicle recruitment by increasing preantral and early antral follicle growth. In contrast to CC on its own, letrozole was shown to increase the number of live births, and the overall level of evidence was moderate. There was insufficient evidence that there was a difference in the rate of live births between CC plus metformin and CC alone, and the overall certainty of the evidence was poor due to the risk of bias and imprecision. When contrasted with the use of CC by itself, the potential benefit of CC in combination with metformin was more apparent in women whose baseline blood insulin or HOMA-IR values were higher than average. In comparison to clomiphene citrate, letrozole was related with higher rates of ovulation, pregnancy, and successful delivery of a live baby.
