Caffeine (Caff) is the most common psychoactive substance utilized around the world. Caff has been demonstrated to possess anti-fibrotic actions in the liver. Silymarin (Sily) is a natural product largely used for its hepatoprotective actions through antioxidant, anti-inflammatory, and anti-fibrotic effects. Aim of the study: The current study aims to investigate the hepatoprotective and antioxidant effects of Caff and Sily alone or in combination in a rat model of liver fibrosis focusing on their effects on lysophosphatidic acid receptor 3 (LPAR3) expression. Materials and methods: 200 mg/kg of thioacetamide (TAA) was injected intraperitoneally twice weekly for 8 weeks to induce liver fibrosis. Caff, Sily, and their combination were orally administered to rats at a dose of 50 mg/kg/day for 8 weeks together with TAA injection. Results: Caff and Sily alone or in combination significantly improved liver function tests compared with fibrotic group. Moreover, they significantly decreased hepatic malondialdehyde (MDA) content and significantly increased the hepatic glutathione (GSH) concentration as compared with the fibrotic group. They significantly decreased fibrosis and necro-inflammatory scores with a superior significant effect in the necro-inflammatory score for the combination group each drug alone. Caff and Sily alone or in combination significantly decreased the hepatic LPAR3 gene and tissue expression, which was significantly correlated to their hepatoprotective effects. Conclusion: Caff and Sily alone or in combination protected against hepatic fibrosis through down-regulation of LPAR3 as well as antioxidant effects.","PeriodicalId":345040,"journal":{"name":"Bulletin of Pharmaceutical Sciences. The study used a modified American Society of Health-System Pharmacists (ASHP) national survey related to Saudi Arabia hospitals. The pharmacy directors of the Ministry of Health hospitals in Tabuk region were contacted by email. They received a soft copy of the survey questionnaire and a link to the survey formulated using the online survey platform “Google forms”. The survey comprised checklist items inquiring about the nature of professional pharmacy services offered in hospital pharmacies. The responses of the pharmacy directors were received, exported to an Excel sheet, and then to statistical software for analysis. Ten hospitals participated in the survey. The nature of pharmacists' services was mostly distributive pharmacists in six (60%) hospitals. Four hospitals (40%) reported a regular evaluation of physician adherence to medication-use policies. Five hospitals (50%) reviewed compliance with clinical practice guidelines. Seven hospitals (70%) had computerized prescriber order entry (CPOE) systems with clinical decision support systems (CDSSs) to handle the medication orders. Automation was not much implemented in medication distribution, where three hospitals (30%) used automated dispensing cabinets, and three hospitals (30%) routinely utilized machine-readable coding to verify doses before dispensing. Six of the approached hospitals (60%) had an IV admixture preparation area. Pharmacists were not much engaged in clinical services. More efforts are needed to implement electronic technologies to improve the safety and efficiency of medication prescribing, transcribing, and administration. Pharmacists should expand their role in medication use review, compounding sterile preparations, optimizing medication administration records (MAR), and participation in patient care units.","PeriodicalId":345040,"journal":{"name":"Bulletin of Pharmaceutical Sciences. The designed compounds were synthesised, and their structures were characterized by spectroscopic analysis. The anticipated anti-inflammatory activity for the synthesized compounds was assessed by in-vitro cyclooxygenases (COX-1/COX-2) inhibition assay. Most of tested compounds demonstrated moderate COX-2 inhibitory activity and selectivity (IC 50 = 0.068 – 0.80 µm, SI = 7.5 - 175) in comparison to indomethacin (IC 50 = 0.079 µm, SI = 12.5) and celecoxib (IC 50 = 0.046 µm, SI = 315.22). Molecular modelling study of the synthesized compounds into the active site of COX-2 enzyme was performed to rationalize their preferred binding affinity; except for compound 5f , all compounds showed binding with amino acids at the selectivity pocket Additionally, In-silico simulation studies explored the drug drug-likeness parameters of the synthesized compounds, all synthesized compounds exhibited promising physicochemical parameters and pharmacokinetics according to the in-silico simulation results.","PeriodicalId":345040,"journal":{"name":"Bulletin of Pharmaceutical Sciences. Pharmaceuticals are possible source of gluten due to excipients like wheat starch. This study examined some pharmaceuticals (non-prescription and prescription type) on gluten and partially hydrolyzed gluten fragments content using sandwich and competitive ELISA techniques, respectively. Partially hydrolyzed gluten fragments were detected in ca. 37% of tested samples (Total: 38). Only 5 samples had gluten concentration above 20 ppm. None of the pharmaceuticals had detectable levels of glutens upon sandwich ELISA application. Oral administration of four dosage forms at recommended doses provided celiacs with a daily gluten intake exceeding the daily tolerance limit. Excipients in patient information leaflet were absent in most cases. The findings confirmed that pharmaceuticals could increase the burdens of celiac patients. Accordingly, regular examination of pharmaceuticals on gluten content is advised. Further studies are recommended that would include a larger sample size, and analytical methods to determine gluten in pharmaceuticals should be developed. The disease is linked to obesity, insulin resistance, and type 2 diabetes mellitus (T2DM), thus addressing these interconnected disorders could be an effective approach for its management. Methods : In this randomized double-blinded clinical trial, 120 patients with NAFLD who had been referred to Gastrointestinal and Liver Diseases Research Center, Rasht, Iran, between September 2015 and January 2016. Patients were randomly assigned to 4 groups. Each group received melatonin, metformin, UDCA, or placebo in addition to a weight loss diet for 3 months. Results : A significant reduction was observed in levels of steatosis, alanine aminotransferase, alkaline phosphatase, body weight, BMI, waist circumstance and triacylglycerol among participants in groups with metformin or melatonin administration. In addition a significant decrease in fasting plasma glucose, and total cholesterol concentration was detected following metformin and UDCA administration, respectively. Conclusions : The present study suggests that the addition of metformin or melatonin to a low-caloric diet may be effective in the treatment of patients with NAFLD.","PeriodicalId":345040,"journal":{"name":"Bulletin of Pharmaceutical Sciences.
