Both hypertensive NCA and stage I HD patients under examination had autonomic disturbances of varying severity. In patients with NCA of the hypertensive type, the frequency and severity of the autonomic disturbances was higher. It is often not possible to make an accurate differential diagnosis of two diseases based on data from a standard clinical examination and basic laboratory and instrumental investigation methods. By constructing prediction equations for a particular patient with a probability of more than 96.1%, it is possible to calculate the acceptability of relating him to a certain diagnosis.","PeriodicalId":344630,"journal":{"name":"Medical Bulletin of the Main Military Clinical Hospital named after N.N. This feature, on the one hand, and the rapid progress of medical science, on the other hand, require a rapid response to the changing operational situation using the resources of modern ophthalmology in order to provide care to wounded and injured ophthalmic patients efficiently and rationally. There are some differences in approaches to pre-hospital self- and mutual aid to the wounded with ophtalmic injuries on the battlefield in different schools of military medical training and tactical medicine. Some use a binocular bandage, while others use a rigid eye shield. The complexity and variety of visual injuries require good practical skills and experience in order to perform high-quality surgical treatment of perforated corneal and scleral wounds in open eye trauma. This necessitates a thorough training of specialists with practical skills. The issue of medical supply of units providing specialized care at the stages of medical evacuation, which includes a military ophthalmologist, is particularly significant. The problem of reasonable and rational update of the ophthalmological equipment at the stages of medical evacuation requires active discussion. The observation group included 139 patients aged from 18 to 72 years with renal and ureteral injuries, who were injured in traffic accidents. Complications of renal trauma occurred in 74 of 128 patients with isolated and combined injuries (59.2%), strictures were observed in 9 of 11 patients with ureteral trauma in the late period, and 1 of them had a failure of vesicoureteral anastomosis. The most common complication in the early and late periods of traumatic disease, occurring in 25.0% of cases, was pyelonephritis; patients with abscesses (12.5%) and acute renal failure (11.7%) were also common. The methods of choice for the diagnosis of combined injuries in the acute period were: ultrasound examinations according to the eFAST protocol and multislice computed tomography according to the “Polytrauma” protocol. Multiparametric magnetic resonance imaging (mpMRI) of the kidneys and ureters was indicated in the periods of early and late manifestations of traumatic disease to detect renal complications when there is a discrepancy between clinical data, ultrasound and multislice computed tomography (MSCT) findings, and there are contraindications for MSCT with intravenous contrast.","PeriodicalId":344630,"journal":{"name":"Medical Bulletin of the Main Military Clinical Hospital named after N.N. OA of the carpometacarpal joint (CMCJ) of the thumb is the most common type of hand osteoarthritis. The aim of the work was to evaluate the effect of ultrasound-guided intraarticular injection of hyaluronic acid (HA) for osteoarthritis of the CMCJ of the thumb (stage II–III Kellgren–Lawrence) in patients with persistent pain syndrome for 3 months and with failure of NSAIDs therapy. The study included 18 patients with stage II–III CMCJ OA who received intraarticular injection of HA drugs with a mass of 2400,000 kDa in the volume of 1.0 ml twice with an interval of 10 days. Demographic characteristics, body mass index, stage of osteoarthritis (CMCJ), severity of pain syndrome according to visual analog scale (VAS) and upper limb function assessed with the DASH questionnaire (disabilities of the arm, shoulder, and hand) were evaluated. Data was collected at the start of the study and in 1, 3 and 6 months after the first injection. DASH and VAS scores were compared using a variance analysis of repeated measurements. The non-parametric Friedman criteria was used to check the differences in proportions. The value of p<0.05 was considered statistically significant. There was a significant improvement in DASH values in 1, 3 and 6 months after injection. There were statistically significant differences between DASH values at the start of treatment and 6 months after therapy: χ²=45.7; df 3, p<0.001. There was also a significant decrease in the average VAS score during the study in 1, 3 and 6 months after the first injection. The differences in VAS score at the start of treatment and in 6 months reached statistical significance: χ²=47.1; df 3, p<0.001. Improvement occurred after 1 month after first injection and persisted throughout the entire follow-up period for 6 months. HA injections provide pain relief and improve upper limb function in patients who are resistant to analgesics and physiotherapy. A significant decrease in pain was observed as early as 1 month after the first intra-articular injection of GNC. This variant of the disease is more aggressive, less responsive to therapy, and recurs more often than conventional multiple myeloma. In our work, we described two cases of aggressively ongoing multiple myeloma with a mixed plasmacytic variant diagnosed at the time of setting the diagnosis. In both cases, a rapid progressive course was observed despite the wide range of both the newest drugs and the classical chemotherapy drugs used during treatment.
