The median follow-up was 31.5 (range, 10-45) months. The primary endpoint was sinus node dysfunction at the mid-term follow-up period.\u0000Results: No early or late mortality occurred. In the early postoperative period, sinus node dysfunction was observed in 32.4% and 7% of cases after double-patch technique and the Warden procedure, respectively (risk ratio, 6.15; 92% confidence interval, 1.45-26.35; p=0.02). At follow-up, sinus node dysfunction remained in two (5%) patients after double-patch correction. All patients had normal sinus rhythm after the Warden procedure. No early or late pacemaker implantation occurred in either group. No patients had significant pulmonary veins or superior vena cava stenosis.\u0000Conclusion: The double-patch technique and Warden procedure both showed best mid - term consequences without mortality. The Warden procedure was linked with less sinus node dysfunction in the early postoperative period than the double-patch technique. Fedakar","doi":"10.5455/azjcvs.2022.11.019","DOIUrl":"https://doi.org/10.5455/azjcvs.2022.11.019","url":null,"abstract":"Aim: We aimed to evaluate the demographic characteristics and early postoperative complications of patients who underwent conventional carotid endarterectomy (CEA) in a secondary level hospital retrospectively.\u0000Material and Methods: Forty-six patients who operated by a single surgeon at Sinop Ataturk State Hospital between January 2015 to December 2016 were included in the current study. Data regarding to demographic characteristics, symptoms, distribution of lesions, diagnostic methods, operation technique and early complications have been collected from handwritten and electronic clinical records. \u0000Results: The mean age was found as 70.9±9.7. Thirty patients were male. The most common risk factors were hypertension (93.5%), hyperlipidemia (50%), cigarette (41.3%) and diabetes (34.8%). Conventional CEA (CCEA) was performed to all patients. Patch plasty was applied to only one patient. According to the internal carotid artery stump pressure, T shunt was performed to 17.4% of patients. One patient died in early postoperative period because of myocardial infarction. In four patients; transient dysphagia, in three patients; hematoma requiring revision, in one patient; uncontrolled hypertension, and in one patients wound infection occurred. Furthermore, delirium occurred in one patient.\u0000Conclusion: Our experience CEA are compatible with the literature. Our results have supported that CCEA with measurement of carotid arter stump pressure are reliable surgical procedure for patients with carotid artery stenosis. Cardiac complications are rare but it can be mortal. Minor surgical complications are more common than death and stroke risk. Patients divided into two groups in line with the development of new-onset AF as AF group (n=62) and non-AF group (n=428). Groups were compared in terms of preoperative clinical features and perioperative data, and multivariate analysis accomplished to identify independent clinical predictors of AF following off-pump CABG.\u0000Results: AF was presented in 62 (12.7%) of the patients, and those were significantly older and had significantly higher additive EuroSCORE scores than without AF. Also patients with postoperative AF had significantly prolonged intensive care unit and longer hospital stays at postoperative period. In the multivariant logistic regression analysis performed, advanced age, mean Euroscore, Mild to moderate mitral regurgitation, Prolonged air leak from thorax drain, mean ventilation time, severe pleural effusion, need for inotropic drug support were found to be risk factors for postoperative AF.\u0000Conclusion: Our study showed that age, high Euroscore, mild to moderate mitral regurgitation, prolonged ventilation time, severe pleural effusion and low cardiac output were risk factors of postoperative AF in patients underwent off-pump CABG. Also In patients who developed postoperative AF, prolonged intensive care unit and hospital stays were observed. Mete","doi":"10.5455/azjcvs.2022.11.020","DOIUrl":"https://doi.org/10.5455/azjcvs.2022.11.020","url":null,"abstract":"Aim: Carotid endarterectomy (CEA), which is performed to prevent the development of cerebrovascular events (CVE) due to carotid artery disease (CAD), is currently the most effective treatment. Perioperative adverse clinical conditions may develop due to patient factors, technique or other reasons. Perioperative stroke is the cause of mortality and morbidity that can develop within 30 days postoperatively. The aim of this study is to present the early results of patients who underwent CEA and investigate the factors affecting the development of early stroke. \u0000Material and Methods: Patients who underwent CEA between 2013-2020 were retrospectively analyzed. 529 patients were evaluated. A total of 550 endarterectomy procedures were performed in 529 patients. Demographic data, presence of symptoms, contralateral carotid artery stenosis, anesthetic method, operation technique, postoperative stroke, mortality were recorded from the hospital database. \u0000Results: The analyses were based on the results of 550 endarterectomy. The mean age of the patients was 67.0±8.4 years. While 164 (29.8%) of the patients were symptomatic, concomitant contralateral carotid artery stenosis was detected in 138 (25.1%) patients. Considering early postoperative adverse effects, there was no cranial nerve damage, while reoperation for bleeding was necessary in 27 (4.9%) patients. Postoperative ischemic stroke occurred in 20 (3.6%) patients. Mortality occurred in eight (1.5%) patients. Gender, rate of DM, HL, CRF or current CAD did not differ in patients with and without postoperative stroke. The patients with postoperative stroke tended to more frequently have contralateral ICA stenosis but the groups did not differ significantly (p=0.117). In early stroke development after CEA, the presence of pre-procedural symptoms was a significant factor (p<0.001). According to the multivariate analysis preoperative presence of symptoms increased risk by 6.41 (95% CI: 2.22-18.54, p=0.001).\u0000Conclusion: Carotid artery revascularization is a safe and effective treatment modality for the prevention of ischemic CVE with low mortality and morbidity rate. Being symptomatic is a significant risk factor for the development of stroke in the early period.
