{"title":"Effect of modified ultrafiltration in patients undergoing coronary surgery using cardiopulmonary bypass. Introduction: The modified ultrafiltration is the new generation of ultrafiltration, which is used to filtrates the excess water and low molecular weight substances like IL-6, IL-8, TNF and endothelin out of the blood. These excess fluid and low molecular weight substances are the main factors of post-operative complications in coronary artery bypass graft patients. There is limited information about the use of MUF in adult surgery, but is mostly used in congenital cardiac surgery patients. It is found to be very beneficial in pediatric cardiac surgery, but we need more data for using it in adult cardiac surgery\u0000Material and Methods: This is a review article; in which we used the articles collected from world’s creditable journals and platforms. These articles have published since 2000. \u0000Results: From articles which we reviewed, 17 were about the benefits of MUF in congenital cardiac surgery and 8 articles were about the favorable effects of MUF in adult cardiac surgery. The rest of the articles were related indirectly to the MUF. About 5 of these researches proved the beneficial role of MUF on lungs functions after surgery, in 4 researches confirmed the significant decrease in post-operative bleeding and the need for blood transfusion, in 5 researches it has been measured that the level of inflammatory factors like interleukins, TNF and endothelin were decreased. In 6 articles the role of MUF in welfare of kidney functions were emphasized. And 1 article discussed about the benefits of MUF on cerebral metabolism correction. {"title":"Reducing effects of ginkgo biloba (egb761) extract on skeletal muscle ischemia-reperfusion injury in rats","authors":"H. Many studies have been conducted to date into the prevention of such ischemia-reperfusion injuries. Ginkgo Biloba extract is known to have antioxidant effects, although there is a lack of data on its effects on a skeletal muscle ischemia-reperfusion model. The present study investigates the use of Ginkgo biloba extract (egb761) for the reduction of experimentally-induced skeletal muscle ischemia-reperfusion injury. \u0000Material and Methods: A total of 32 Sprague-Dawley rats were divided into four groups as follows: Group 1 (n=8), control group; Group 2 (n=8), limb ischemia model; Group 3 (n=8), ischemia-reperfusion model; and Group 4 (n=8), Ginkgo biloba group. After the models were created in each group, soleus muscle, lung and liver tissues were removed. Tissue malondialdehyde (MDA) levels were measured biochemically to demonstrate lipid peroxidation. The percentage of viable cells in the soleus muscle was calculated through a histochemical examination.\u0000Results: The comparison of soleus muscle MDA levels revealed no statistically significant difference between Group 1 and Groups 2 and 3, whereas the levels were significantly lower in Group 1 than those in Group 4 (p<0.05). Similarly, there was a significant reduction in Group 4 than in Group 3 (p<0.05). The analysis of lung tissue MDA levels revealed no significant difference between Group 1 and Group 2, whereas MDA levels were significantly increased in Group 3 than in Group 1 (p<0.05), while there was no difference between Group 1 and Group 4. The analysis of liver tissue MDA levels showed significantly increased levels in Groups 3 and 4 than in Group 1 (p<0.05). The percentage of viable cells was significantly decreased in Groups 2 and 3 than in Group 1 (p<0.05). There was a significant reduction in Group 4 than in the control group. \u0000Conclusion: The results of the present study suggest that Ginkgo biloba (EGb761) extract may be used to reduce the local and systemic effects occurring after the restoration of blood flow following acute arterial occlusion. Concomitant arterial embolism should be kept in mind in cases with pulmonary embolism and COVID-19. Renal infarction should be suspected in a COVID-19 patients presenting with an unexpected symptom such as abdominal pain. {"title":"Early and mid-term outcomes of patients treated with catheter-directed thrombolysis combined with percutaneous rotational mechanical thrombectomy for deep vein thrombosis","authors":"A. In the current study, we aimed to evaluate the employability of the mechanical thrombectomy and combination of it with catheter-directed thrombolysis in selected patients with acute and/or subacute DVT. \u0000Material and Methods: We applied mechanical thrombectomy and catheter-directed thrombolysis in the patients who admitted with an index diagnosis of lower extremity acute and/or subacute DVT. The thrombus in the femoral vein and iliac vein was intervened by intervention from the popliteal vein. Afterwards, mechanical thrombectomy and catheter-directed tPA infusion for 24 hours were applied. In follow-up, patients were evaluated through physical examination and Doppler ultrasonography on the 1st_, 6th_, 12th_, and 18th_month follow up, and scored with Villalta. \u0000Results: The mean age was 59.9 years. Twenty-six and 21 of the 47 patients were male and female, respectively. In the 1st-month follow-up visit, there was ≥50% patency in 43 patients, and ≥90% patency in 30 patients. During the 18-month follow-up period, while three patients had recurrent DVT, one patient had a new DVT episode in the contralateral extremity. According to Villalta score, three patients were found to have post-thrombotic syndrome (PTS) at the end of 18 months.\u0000Conclusion: We think that; pulmonary embolism (PE) and PTS rates would decrease with the concomitant application of percutaneous interventions to medical treatment in patients with index DVT diagnosis. Additionally, although the cost of percutaneous interventions seems to be high, it would be much more cost effective in the long-term when the complications related to DVT are taken into account. {"title":"Features of coronary artery bypass grafting in young patients with acute coronary syndrome","authors":"Ramil A. Aliyev, K. Musayev","doi":"10.5455/azjcvs.2020.1.1.2","DOIUrl":"https://doi.org/10.5455/azjcvs.2020.1.1.2","url":null,"abstract":"Aim: Purpose of the study was retrospective analysis of hospital and early (up to six monthes after operation) results of the surgical myocardial revascularization in patients, third and fourth decates of life, with acute coronary syndrome. \u0000Material and Methods: Since 2012 year up to November, 2018 year in CCH by the same brigade, performed 500 coronary artery bypass grafting operations in the patients with acute coronary syndrome, at the age of 30-83 years old, including 15 operations in patients ≤ 40 years old (3%). Clinical manifestation of acute coronary syndrome was presented: unstable angina in 13 (86.7%) patients, 6 (40%) of them with pass myocardial infarction in anamnesis; acute myocardial infarction, accompanied by S-T segment elevation (STEMI) in 2 (13.3%) patients. In coronarography: multivessel lesion of coronary arteries was revealed in 10 (66.7%) patients, 3 of them with lesion of trunk of the left coronary artery; bivascular lesion in 4 (26.7%) patients, single vessel lesion in 1 (6.6%). In 13 (86.7%) patients were performed coronary artery bypass grafting under the cardiopulmonary bypass (CPB). Average duration of the CPB= 127±9.66 min. Mean aortic clamp time = 68±6.91 min. Number of shunts for the whole group of 15 operated were from 1 to 5, at the average (3.28±0.31). In 6 patients (40%) performed three-branches CPB, in 5 patients (33.3%) fourth-branches, in 1 patient (6.7%) fifth-branches, in 2 patients (13.3%) double-branches and in 1 patient (6.7%) single branch CPB. Used conduits: left internal mammary artery in all 15 patients (100%), autovein in 12 (80%), radial artery in 10 (66.7%) patients. In last group with using radial artery, average amount of shunts was 1.5±0.1.\u0000Results: The structure of postoperative complications consist: low cardiac output syndrome in two patients; bleeding in 2 patients. In analyzed series, no lethal outcomes watched in duration up to 30 days after operation. All patients were watched during 6 monthes after discharge. It was not watches necessity for reinterventions and lethal outcomes.\u0000Conclusion: Hospital and immediate results of myocardial revascularization in young people with acute coronary syndrome very encouraging, while long-term results are not so optimistic and unsatisfactory. Despite the development of percutaneous interventions, Coronary Bypass surgery continues to be one of the most crucial surgical procedures in modern medical history and has most likely extended more lives and provided more significant symptomatic relief than significant operations.The aim of this study was to show statistically, the percentage of complications of operations in our clinic was close to world practice\u0000Material and method. Between 2013 and 2018, 836 isolated Coronary Bypass surgeries were performed at Ganja Central Clinic. The results of these operations conducted in the study were presented \u0000Results: The length of stay of 362 (43.5%) patients was 5 days, and the length of stay of 58 (6.9%) patients was 10 days. 15 (1.8%) patients had an ischemic disorder of cerebral blood circulation in the postoperative period. 26(3.1%) patients developed acute kidney failure, and 25(3%) patients had acute lung failure. Acute liver failure was noted in 7 (0.8%) patients. An intra-aortic balloon pump was used in 14 (1.6%) patients. Postoperative MI was noted in 13 (1.5%) patients. Unfortunately, 17 (2%) patients died during hospitalization. Different types of arrhythmias were noted in 115 (13.7%) patients. 11(1.3%) underwent revision due to sternum ulcer. The mediastinum was inspected since 19 (2.2%) patients were bleeding.\u0000Conclusion. Coronary Bypass surgery continues to be one of the most crucial surgical procedures in modern medical history. {"title":"Does the anatomical shape of the heart present as a risk factor for \"off-pump\" coronary surgery?","authors":"E. We aimed to investigate the relation between the anatomical position of the heart and the incidence of conversion to on-pump coronary surgery.\u0000Material and Methods: Four hundred patients, who underwent off-pump three-vessel coronary surgery during June 2010 and February 2020. The angle between the left anterior descending (LAD) artery and the left arm of the sternal retractor was used to identify the anatomical position of the heart intraoperatively after pericardiotomy. Patients were categorized as having either transvers (Position Angle> 45°, n:200) or vertical (Position Angle <45°, n:200) heart.\u0000Results: The conversion rate was 8.75% (35/400) in our study. Thirty-three of patients with transverse heart patients converted to on-pump surgery (16.5%), whereas the two patients(1%) in patients with vertical heart. The length of intensive care unit stay was longer in patients with conversion compared to the patients with no conversion (3.17±1.10 days and 1.11±0.11 days, respectively) (p=0.001). Inotropic support and blood transfusion requirements were higher in converted patients. The mortality rate was 14.2% in converted patients while non-converted patients had no mortality (p<0.001).\u0000Conclusion: Conversion from off to on-pump during coronary surgery may cause high incidence of morbidity and mortality. Probable causes responsible for conversion should be predicted before making a decision for off-pump surgery.
