To correct the reactive changes that develop during testosterone blockade in the organs under study, scientists use the active ingredient quercetin to study its effect on oxidative stress. Its antioxidant effect can be used for pathogenetic therapy and prevention of changes in the body caused by a decrease in testosterone levels [12, 13, 18]. The aim of the study was to investigate morphological changes in corticosterocytes zones of the cortex and medullary endocrinocytes of the adrenal glands of white rats during the correction of central testosterone deprivation at the sixth month of the experiment. Materials and methods: The adrenal glands of 25 male white rats were studied and divided into 3 groups: group 1 – 5 intact rats, group 2 – 10 rats injected with triptorelin acetate subcutaneously at a dose of 0.3 mg of active substance per kg of body weight, and group 3 – 10 rats injected with quercetin 100 mg per kg of body weight once a day 3 times a week for 6 months against the background of triptorelin acetate administration. The average size and volume of the nucleus (N) of corticosterocytes of all cortical zones (CZ), the average size and volume of the nucleus of the ChC, the density of the cords of the adrenal cortex zones and the density of the cords of the adrenal medulla were measured and calculated. Conclusions: 1. Correction of central testosterone deprivation at the 6th month of the experiment causes a decrease in the synthetic activity of corticosterocytes of the glomerular zone and an increase in the protein synthetic function of fascicular zone cells. 2. A significant decrease in the average size of corticosterocytes of the reticular zone and restoration of cytoplasmic basophilia at 6 months of correction proves the positive effect of quercetin on the synthetic activity of these cells. 3. Reduction of the average size and volume of chromophilic endocrinocyte nuclei and increase in their number during the correction of chemical castration at 6 months of the experiment confirms the significant positive effect of quercetin. 4. {"title":"RESTORATION OF TEETH AFTER ENDODONTIC TREATMENT WITH ENDOCROWNS","authors":"K. Materials and methods of the study: A literature search was conducted using the electronic database PubMed to find studies related to the use of surgical templates during implant surgery. The search term was \"endocrown\", and the search range covered the period from January 2012 to December 2022. The review included in vitro studies. Literature reviews, abstracts, articles describing the use of endocrowns for the treatment of temporary teeth, incisors, canines, premolars, and technical articles were excluded. Scientific novelty: The data on the adaptation and strength of endocrowns for the restoration of endodontically treated teeth was structured and clarified. Conclusions: In the course of the retrospective analysis, a sample of 32 scientific articles was formed. The majority of the analyzed studies demonstrate that the design of the tooth preparation and the choice of material for the restoration of endodontically treated teeth are crucial for fracture resistance. The need for post-and-pin restorations has decreased with the introduction of adhesive methods. Endocrowns can be effectively used as an alternative to standard post and fixed partial dentures, especially for the restoration of severely damaged teeth. The advantages of endocrowns over traditional procedures include better aesthetics, improved mechanical performance, lower cost and shorter waiting times in the clinic. However, clinicians should be cautious when choosing the type of restoration after endodontic treatment. {"title":"ORGAN-PRESERVING MICROSURGERY FOR PLEOMORPHIC ADENOMA OF THE PAROTID SALIVARY GLAND","authors":"D. Among benign tumors of the parotid gland, the most frequent tumor (61.2–90%) is pleomorphic adenoma. The aim of the research is to develop and apply in clinical conditions microsurgical methods of organ-preserving surgery for pleomorphic adenomas of the parotid gland, to determine the main clinical indications for the use of a specific method of surgical treatment, to reduce the risk of developing postoperative complications. The material of this study is based on the results of the surgical treatment of 90 patients with pleomorphic adenomas of the parotid gland, which was carried out in the clinic of maxillofacial surgery on the basis of the Department of Maxillofacial Surgery of MI “Poltava M.V. Sklifosovskyi Regional Clinical Hospital”. Preoperative diagnosis was made on the basis of clinical examination, results of ultrasound and magnetic resonance computer tomography of the parotid glands, as well as the cytological examination of material obtained during puncture biopsy of the tumor. In 80 patients, the tumor was localized in the superficial part, and in 7 patients – in the deep part of the parotid gland. In 2 patients, there was total tumor damage to almost the entire gland; in 1 patient, the multinodular nature of the tumor process in both the superficial and deep parts of the gland was noted. As a result of the conducted research, we believe that performing a partial resection of the parotid gland using microsurgical principles of the operation allows us to expand the generally accepted indication (tumors up to 2 cm in size) for performing this operation for pleomorphic adenomas of the superficial part of the gland. In our opinion, the main criterion for the use of this operation is the absolute size of the tumor and the relative percentage of tumor involvement in the superficial part of the parotid gland. If the results of the examination show that the tumor does not occupy the entire superficial part of the gland, and it can be removed within the limits of healthy tissues while preserving some part of the gland, preference should be given to this method, rather than subtotal resection of the gland. Hryn","doi":"10.31718/mep.2023.27.1-2.04","DOIUrl":"https://doi.org/10.31718/mep.2023.27.1-2.04","url":null,"abstract":"Suture material used in abdominal surgery must have more or less antigenic properties so that during wound healing it could be destroyed by an immune response to rejection, which is known to involve cytotoxic T-lymphocytes and macrophages. This, in turn, can lead to the development of aseptic peritonitis. Therefore, while planning an experimental simulation of aseptic peritonitis, we decided to use catgut thread as an antigenic stimulation of the local immune system. The aim of our research was to study the morphological criteria of the microscopic structure and tinctorial properties of catgut thread. The material of the research was the sterile catgut thread sized 2/0, 0.3 mm thick, of equal lengths, which we twisted arbitrarily into flat compact globules occupying an area of approximately 1 cm2. In order to carry out a histological examination, some catgut globules were subjected to total staining with a hematoxylineosin solution and subsequent embedding of these globules in a paraffin block and making appropriate sections from it. The other half of the catgut globules were studied using the method of epoxy plastination with the production of grinds with staining by a 1% solution of methylene blue on a 1% borax and without any staining. After the total staining of the catgut globules with a hematoxylin-eosin solution, a clear acidophilic reaction was revealed, which was manifested in the intense pink color. Catgut thread has a non-uniform fibrous structure, in which dark streaks stand out, dividing it into a series of longitudinal, lighter, layered fascicles, which consist of a dense collection of thin fibrillar elements. These fibrillar elements are orderly fascicles of collagen fibers, while the dark streaks are layers of loose fibrous connective tissue. Leukocytoclastic vasculitis of the skin (LCV) is an isolated cutaneous vasculitis without damage to internal organs. At present, vasculitis is considered a polyetiological disease. The most frequent causes of the development of vasculitis limited to the skin include various acute or chronic infections: bacterial, viral, and fungal. It can be caused by medications, and sometimes the cause cannot be found. When LVC is suspected, an extensive examination is usually required to determine whether the process is limited to the skin or is a manifestation of systemic vasculitis or other systemic diseases. Diagnosis and treatment of LCV is a complex problem in the practice of doctors of many specialties, such as rheumatologists, dermatologists, and therapists, family doctors, which requires close cooperation of specialists. We present 3 clinical cases of patients who were hospitalized in the clinic with symptoms of skin lesions and suspicion of LCV after taking medications. The paper describes various skin manifestations and the results of the examination and treatment. The differential diagnostics enabled us to diagnose LCV in 2 cases, in one case – generalized urticaria due to taking furazolidone. It should be noted that drug reactions with skin lesions constitute a serious medical problem, whose relevance is increasing every year. This is largely due to widespread uncontrolled drug intake among the population, and frequent polypharmacy during treatment under the supervision of medical professionals. Multidisciplinary problems, which are solved by doctors of various specialties at different stages and levels of medical care, including drug-induced skin lesions in patients, are particularly difficult. Comparing the course of experimental pathological conditions in mice with human pathology requires deep knowledge of the morphology of the lungs of mice. Although it is advisable to obtain pathomorphological conclusions from an experienced pathologist, in reality, this is not always possible. Therefore, many scientists have to conduct such an analysis by themselves. The aim of the research was to study and describe the normal lung morphology of BALB/c mice for the improved characterization of phenotypes of experimentally induced lesions. Materials and methods. The study included 10 healthy male BALB/c mice, 8 weeks old, housed in individual conditions with unlimited access to food and water. Animals were euthanized by inhalation of ethyl ether. The lung necropsy was performed by tracheal infusion with 10% neutral buffered formalin. The research was carried out on paraffin sections of 3 μm, part of the left lung or one of the right lobes. Sections were stained with hematoxylin-eosin, Schiff-iodic acid, and according to Mallory. Morphology was analyzed using a light microscope. Results. The applied method of euthanasia by inhalation with ethyl ether does not cause agonal changes in the lungs. Fixation by tracheal formalin infusion prevents lung collapse and ensures optimal morphology. For the study of experimental human pathology, it is optimal to examine the left lobe of the lungs, with its longitudinal orientation for obtaining sections, which is of great importance. A blinded way of describing microspecimens does not always provide the best result. Collagen deposits in the outer layer are observed around the large arteries of the lungs; goblet cells are rare, and only in large bronchi. Bronchus-associated lymphoid tissue (BALT) is barely visible in healthy lungs of mice. Natural mouse pathogens can alter the physiology of the host, making it unsuitable for many experimental purposes, and although the number and prevalence of these pathogens have decreased significantly, many of them can be found in laboratory animals and are undesirable in research. Conclusion. {"title":"THE EFFECT OF THE DRUG MUMIJO ON THE PARAMETERS OF PLATELETS IN RESIDENTS OF AREAS WITH A CONSTANT COMBINED EFFECT OF NATURAL AND TECHNOGENIC IONIZING RADIATION","authors":"P. Kovalenko","doi":"10.31718/mep.2023.27.1-2.08","DOIUrl":"https://doi.org/10.31718/mep.2023.27.1-2.08","url":null,"abstract":"Peripheral blood platelet parameters were studied in 53 apparently healthy female volunteers living in the city of Kropyvnytskyi, Kirovohrad region. 2 groups were formed: the main group received the biologically active supplement (BAD) mumijo once a day for 30 days, and the control group did not receive this drug. The content of platelets in the blood (PLT), the mean platelet volume (MPV), the relative width of the distribution of platelets by volume, i.e. the platelet heterogeneity index (PDW), and plateletcrit – fraction (%) of the volume of whole blood occupied by platelets (PCT), were studied before and after the course of the BAD mumijo. The results of the study showed a positive effect of the BAD mumijo on platelet parameters. Thus, the frequency of detection of persons with a reduced platelet content probably (Р<0.05) decreased, the limits of individual fluctuations of the platelet content shifted upwards to the limits of the norm, the average indicators probably increased (Р<0.05) with a tendency towards normalization. Indicators of plateletcrit, which reflects the fraction (%) of the volume of whole blood, made up of platelets, the average volume of platelets reliably (Р<0.05) increased to the average indicators of the norm. The limits of individual variations in the width of the distribution of platelets shifted to the normalization of the indicator. Changes in platelet parameters indicated an increase in their content due to the proportion of young and more functionally capable platelets. It has been established that the BAD mumijo is an effective radiomodifier of the effect of constant combined exposure to natural and technogenic man-made low-intensity ionizing radiation. However, a significant array of information needs systematization and generalization, taking into account the specifics of providing dental care by dental institutions in different regions. The authors conducted a retrospective study of the main statistical regularities based on the results of 32 cases of treatment of secondary anodontia by replacing bounded and finite distally unlimited defects of the lower and upper jaws with the support of dental endosseous implants as exemplified by outpatient care at a private dental office in the city of Poltava (Ukraine). Dental implantation and subsequent prosthetics were performed at one medical institution, by one doctor in cooperation with one dental laboratory in the city of Poltava (Ukraine). The term of retrospective observation was 5 years. The condition for inclusion in the study was the manufacture of a non-removable orthopedic appliance with the non-removable fixation on dismountable and full-body intraosseous screw dental implants. In all cases, metal and ceramic structures with cement fixation on intraosseous dental implants of the Implife Classic, Implife Solo (Inpride company, Ukraine) Vitaplant VK Vitaplant V2Kn (Vitadent company, Ukraine) implant systems were manufactured. Observation of the treatment outcomes was carried out an average of 32 months after the fixation of 69 orthopedic appliances. It turned out that the protocol of permanent prosthetics to replace partial defects of the dentition was implemented mainly in the middle-aged group. Periodic outpatient monitoring of orthopedic appliances with cement fixation on intraosseous implants takes place within the period from 6 to 60 months. The average observation period was 32.8 months. Characteristic statistical regularities related to the selection of individuals and the most common types of implants for solving standard clinical tasks were determined. The patient had all the characteristic signs of a severe course of SLE with the development of a complication - severe anemia (frequent exacerbations of the main disease, a significant and constant decrease in the number of erythrocytes and the level of hemoglobin in the general blood test against the background of glucocorticosteroid therapy, the severity of clinical symptoms). Damage to the hematopoietic system is characteristic of such patients and prevailed in the clinical picture of the disease and became the direct reason for seeking medical help. The appointment of mycophenolate mofetil made it possible to reduce the activity of manifestations of the main disease, reduce the development of possible complications, and in the future achieve clinical and laboratory remission. The clinical feature of this case is the effective use of mycophenolate mofetil in a patient with high activity of the course of SLE and the constant occurrence of a severe complication - severe anemia, against the background of glucocorticoid therapy. Damage to the hematopoietic system in the patient prevailed in the clinical picture of the disease and became the direct reason for seeking medical help. According to the literature, the use of mycophenolate mofetil in patients gives a positive clinical result in the treatment of such patients: reducing the activity of the course of the main disease, preventing complications, extending the period of clinical and laboratory remission. Myofascial pain syndrome, localized in the muscles and fascia, can range from mild discomfort to excruciating pain. To optimize personal rehabilitation programs in health care practice, for additional visualization of educational and practical activities, it was suggested to consider the rehabilitation process as a \"motor and mental dialogue\" at the same time, and also to use a topological object - the Möbius strip as a visual, multi-vector, interdisciplinary, plastic model of rehabilitation. The paper focuses on the study of the effectiveness of using the ecological approach in rehabilitation practice. The research was conducted on the basis of the Municipal Enterprise \"Poltava Regional Clinical Hospital for War Veterans\" of Poltava Regional Council. 42 military servicemen - participants of combat activities with a diagnosis of the myofascial pain syndrome of the cervical area were included in the study. As a component of the ecological approach, an individually developed program of comprehensive rehabilitation intervention aimed at the biomechanically interconnected muscle spirals, the diaphragm and masticatory muscles, and the temporomandibular joint was used. The method of active rehabilitation in patients using a program of comprehensive rehabilitation intervention for biomechanically interconnected muscle spirals, diaphragm and masticatory muscles, and temporomandibular joint, was based on a course of training with a multidisciplinary rehabilitation team lasting 21 days - 3 - 5 sessions with the doctor every day and 6-8 sessions according to the video sample recorded by the doctor. A statistically significant decrease in the intensity of pain according to VAS and an increase in the angle of painless lateral movement with an element of rotation may indicate the effectiveness of rehabilitation measures already on the seventh day of treatment. A decrease in the intensity of pain and an increase in the angle of painless lateral movement with an element of rotation in the experimental group, as compared to the control group, may indicate the advantage of using the proposed program at the same time as standard approaches in rehabilitation. The model proposed in the study can act not only as a treatment and rehabilitation tool but also as a visual training tool for future specialists, which has several advantages over traditional models.
