Developed at Kidz First Children's Hospital in Auckland, New Zealand, the guideline deals with the prescription of epi-pens to children who have experienced an anaphylactic reaction. Traditionally, epi-pen prescription and follow-up support have been fragmented, with responsibility spread across a range of service providers, each operating largely on its own. To address this issue, the seamless care model has been used to establish a fully integrated care delivery system that facilitates a smooth transition from acute to community-based care. This development serves to demonstrate how seamless care can facilitate practice development and organisational change, as well as enhancing child and family health outcomes. This research study was designed to identify ways of enhancing patient safety during the administration of medications within the New Zealand context. We employed a multi-method approach that included a survey using the Safety Climate Survey tool, focus groups and three clinical practice development groups. The outcomes of this study indicate that practice development initiatives, such as the ones outlined in this project, can have a positive effect on nurses' perceptions of organisational safety, which in turn has been demonstrated to have a positive impact on patient safety (Pronovost et al., 2003). Its purpose was to provide a framework to help organizations to integrate the process of improvement in patient care and services at the levels of the patient care team and the organization as a whole. \u0000 \u0000 \u0000 \u0000This article outlines the rationale for the programme and describes its philosophy and approach to delivery. It also presents a critical evaluation of the programme's impact and of lessons for design and delivery of future programmes of a similar nature. The programme received a positive evaluation from participants. Reflecting upon the scope and nature of practice development work enabled participants to clarify their thinking and focus their attention on developments at the clinical level to ensure that improvement initiatives are directly grounded in and from practice. At the same time, the importance placed on diagnosing organizational culture and the artefacts already present within the systems provided valuable insights into designing engagement strategies and sustainability of improvement work. This particular practice development work demonstrates how service users, in this case older people with intensive rehabilitation needs, were involved in the evaluation work. The evaluation work, comprising four methods (Patient Centreometer Questionnaire, Service Questionnaire, Focus Group and Patient/Carer Stories) was used to facilitate or enable multi-disciplinary practitioners in the practice development group actively to learn through a different form of engagement with older people outside of the usual ‘patient’ and ‘caregiver’ roles. The learning achieved from the process and the evidence obtained through the evaluation methods provided motivation, increased commitment and generated a practice development action plan that enabled changes in practice to be implemented based genuinely on users' experiences. This article also shows how this practice development work moved away from the notion of the baseline and comparative evaluation method to more of an ongoing and integrative method of evaluation. Solution-focused work in practice development draws on solutions-focused therapy but is distinct from it in a number of important ways. In the article there is an emphasis on the energy that emanates from the recognition of internal (personal or team) strengths and the ways that facilitators may guide practice developers to build confidence in their own ability to find solutions in the seemingly messy world of everyday clinical practice.
