Nurses and midwives therefore need to be proficient in its use. This paper reports on a cross-sectional survey of factors influencing nurses' and midwives' use of IT in a large hospital in England. A self-completed questionnaire was distributed to 343 nurses and midwives. A response rate of 38% was achieved with 129 questionnaires returned. The majority of practitioners had access to and used computers at work. Practitioners' general IT skills (e.g. e-mail, Internet) were more developed than their skills in using work-related software. The major barriers to using IT in clinical practice related to a lack of time, difficulty accessing computers, and social and cultural considerations whereby there was a perceived lack of peer and managerial support for using IT in the workplace. Strategies need to be developed to ensure that obstacles to nurses and midwives using IT are addressed. In addition to extending the IT skills of nurses and midwives there is an urgent need to develop a culture in which the use of IT becomes the accepted norm in clinical practice. Those expectations include a high standard of care with continually improving practice and outcomes; that is, what we might regard as practice development. The inability to satisfy these expectations is currently attributed, at least in part and somewhat simplistically, to a perceived \u0000failure of leadership. The immediate response to remedy this perceived failure has been a proliferation of leadership development and support programmes. Not \u0000surprisingly, these programmes are yet to have any significant effect on complex problems deeply rooted in the structures and cultures of the health systems. \u0000 \u0000Clinical leadership is fundamental to practice development, and practice development teams have been characterized by high profile leaders who tend to move onwards and upwards so there is a high turnover of leaders among these teams. A re-examination of the notion of leadership development and maintenance is both timely and necessary, as is some \u0000consideration of the place of leadership in the dynamics of clinical teams. The creation of transformational leaders can be highly effective in changing culture and performance but the task of doing this in a context that is hostile to its principles may be more than most leaders can endure \u0000and still survive intact. Originating in the UK, it is increasingly being adopted in other countries. Its perceived value is in its facilitative approach to engaging, empowering, enlightening, evolving and evaluating patients, users and professionals within the processes of innovation and change through collaboration, team working and partnership building. Despite the plethora of literature outlining the relative merits and demerits of practice development to the healthcare professions and professionals and health and social care organizations there is limited evidence substantiating the existence of a knowledge base within the field of practice development. \u0000 \u0000This paper aims to present the underpinning evidence confirming the existence of a knowledge base within the field of practice development. \u0000 \u0000An in-depth critical review and synthesis of the literature on CINAHL, undertaken at the end of 2004 using the key words ‘practice development’ revealed 416 articles. This was limited to 130, which made direct reference to practice development in the title. From these articles, 23 were research focused and 107 associated with informing the practice development debate. \u0000 \u0000The in-depth critical review and synthesis of the literature demonstrated that practice development can be defined and deciphered to show its impact in supporting innovation and change to individual, team and organizational practice. Practice development's primary principles are centred on promoting patient-centredness through the utilization of a facilitative approach to team working, collaboration, and partnership building and by networking. By nature of design and virtue practice development is not linear but a messy concept, yet among the mess of daily practice it does adopt a systematic and organized holistic approach to advancing and evaluating practice. \u0000 \u0000The in-depth critical review and synthesis of the practice development literature reinforces the need to consolidate a position in order to move forward rather than back. This review confirms that practice development is an emerging discipline within health and social care demonstrating an acquired body of knowledge and an evidence base to substantiate its case. By plotting its history and reviewing the current evidence base, it would be fair to state that practice development is here to stay and due to grow within all professional groups. The nursing profession has become increasingly interested in the concept of facilitated reflective practice (Department of Health, 1993) to provide this supportive environment. The more widely recognized strategies that have been adopted include clinical supervision, critical mentorship and action learning. A relatively new approach has been developed called critical companionship, which is described by Angie Titchen (2001) as \u0000 \u0000 \u0000 \u0000a metaphor for a helping relationship in which a critical companion accompanies a less experienced practitioner on their personal experiential learning journey. \u0000 \u0000 \u0000 \u0000This paper outlines how two colleagues, Bernadette and Clare, developed their working relationship and facilitation skills in an environment of support and challenge using this framework. The Unit has currently achieved Stage 1 accreditation and are presently striving to achieve full accreditation. \u0000 \u0000 \u0000 \u0000One of the important factors required in a successful PDU is having effective leadership that is able to both motivate the team and achieve the desired results. \u0000 \u0000 \u0000 \u0000This article is a reflective piece examining the problems and obstacles that were encountered by the two leaders of the PDU on their journey to be a fully accredited unit. These problems included lack of team ownership, lack of devolved leadership and lack of time. The article also sets out ways in which these problems have been overcome, and gives the reader practical tips on things to take into account before becoming a leader of a PDU.
