This article attempts to describe the emergence of the ‘one-stop shop’ approach in surgical outpatients and preoperative assessment clinics. It portrays the ways in which one clinical team moved their ideas forward to develop an integrated, multidisciplinary service, and describes the tools and methods they adopted to cross the borderlands between practice development and service improvement, to enable success. We interviewed a sample of young people and their parents, who had used the service using a semi-structured interview designed to encourage reflection on the intensive home-based service. Interviews were recorded, transcribed and framework analysis undertaken. Key themes are discussed in the context of current strategic developments of children's services. We concluded that it is important for families to know what services are available, and feel that they have choice in treatment plans. Both positive and negative impacts on family life in caring for a young person with mental illness at home emerged as key themes. Dewing","doi":"10.1002/PDH.295","DOIUrl":"https://doi.org/10.1002/PDH.295","url":null,"abstract":"This article describes and relates to classic and emerging theory, and the journey of one person, based at a Dutch university (Fontys University of Applied Sciences), towards becoming a facilitator concerned with transformational teaching practice. Following on from some background on facilitation within the context of practice development, the article summarizes and offers illustrations from a critical conversation between the two authors. Three themes relevant to climbing the tree were identified as (1) working with clarified values and beliefs; (2) movement: moving from technical single issues to working with complex questions and multiple practice development principles and (3) construction and reconstruction of a personalized embodied facilitation framework. These are used as a structure for the discussion. The case study concludes that becoming a facilitator, rather than the technical application of facilitation techniques and tools, is a continuous journey, with personal or internal dimensions where the person/facilitator is part of the learning process. PDUs are reported as positive initiatives within health care. It is important to understand the process of becoming a PDU and the ongoing effects of the process to the staff working within the unit. This project engaged with multidisciplinary staff working on six PDU units throughout the north-west of England. Data collection was in two phases: a self-completion questionnaire distributed to all staff within the PDUs (n = 625, 28.2% response rate) followed by 17 semi-structured telephone interviews. The majority of questionnaire respondents (n = 114, 64%) indicated that they would recommend PDU accreditation to other units. The main themes arising from the project focus on accreditation having a positive influence on multidisciplinary team working, evidence-based practice and improving opportunities for professional development. Issues were raised during the interviews regarding the increased development of staff leading to high turnover, the high workload involved in leading the accreditation and the inequitable involvement of professions in the process. New graduates of nursing administer medicines as a large part of their role, and medicine administration error is well documented. A plan was developed to address these errors in one acute care setting; this included a review of the literature, analysis of reported error, a focus group, a new graduate survey and evaluation. Three supervised medicine rounds, a practice oral medicine assessment and checking of graduates' charts was implemented. Comparison data (2007 and 2008) revealed a decrease in the number of medicine errors. Future graduates will be included in all phases of this evolving practice development, which has increased the effectiveness of patient care by decreasing potential harm. The research team used Habermas' (1987) theory of communicative action to guide a process that enabled honest communication and equality within the group necessary to obtain sustained technical and emancipatory change. An action research method was used to make changes based on the best available evidence. Three criteria were used as a basis for change – the effectiveness of care, patient safety and patient-centeredness. Prior to the study, no patient had had complex day surgery at this hospital. During the study, 55 patients underwent complex day surgery in an effective, safe and patient-centred way. Improved communication and increased confidence enabled the research team to achieve significant technical outcomes. The group matured into autonomous, accountable and engaged clinicians that demonstrated emancipation.
