Little is published about the sequencing of engagement with consent and pre-measures in a research project. This article reports on nurse engagement in the first six weeks of the implementation of a new model of care in two correctional service health care settings. This article will outline a mental health units approach and experience of implementing collaborative practice development processes which identify values and beliefs and enable the development of a framework of action to effect positive change. A qualitative participatory action research design was used to engage participants in exploring and critiquing relevant workplace culture issues. The project was divided into four chronological stages: values clarification; claims, concerns and issues; barriers, enablers and actions; and action planning. The results of this project highlight a number of issues for practice development approaches. We believe that these processes are easily transferable to other organizations within health care. Hardy","doi":"10.1002/PDH.284","DOIUrl":"https://doi.org/10.1002/PDH.284","url":null,"abstract":"Work-based learning has the potential to transform health care services to improve patients' and users' experiences, support the implementation of evidence, provide value for money, improve productivity and achieve continued modernization. This article describes the findings from a literature-based concept analysis that explored a contemporary understanding of work-based learning relevant to modern health care practice and education. Many descriptions of work-based learning exist but it is difficult to define the concept in a way that practitioners can understand and relate to in their daily working lives. The article identifies the distinguishing attributes of work-based learning, together with its enabling factors and consequences. From this literature-based concept analysis, it is concluded that work-based learning is a process that concentrates on how learning takes place within the workplace. It is stimulated by workplace activities that engage the learner in discussion and debate with workplace colleagues. This critical dialogue, if facilitated and adequately resourced, can trigger a transformation of workplace culture into one that captures situated learning to enhance not only the individual, but also team and even organizational working practices. Within Sheffield Teaching Hospitals NHS Foundation Trust (STHFT), the geography across the city adds a unique dimension to bringing about changes in practice and using the best possible evidence. Specific strategies need to be put in place in order to enable clinicians to access specialist help within the Trust, using the available networks to their best advantage and thereby reducing repetition. The role of a practice development team situated within such a large organization can influence changes in practice and encourage newer innovative ways of working. However, getting ‘inside’ the prevailing practice culture can be challenging and complex and it can be difficult to determine the specific aspects of culture that need to be addressed in a development programme. One way of doing this, and a key process of practice development, is observation of practice. However, many of the tools available for the observation of practice are inconsistent with the philosophy and values underpinning emancipatory practice development, i.e. they tend towards ‘judgement’ of quality rather than the facilitation of engagement. The WCCAT has been developed as an observation tool consistent with the philosophy and values of emancipatory practice development and is a facilitative process. In this paper we set out the framework underpinning the WCCAT, the phases involved in undertaking an observation and examples of how to use the tool in practice. An initial evaluation of the use of the tool by practice development facilitators is also provided. It was divided into two phases, a pilot and full study. Aims agreed with local organisations included timely, targeted reporting to the NRLS in relation to selected medication errors, one of which was incidents involving anticoagulants. The present report outlines initial experience in the utilisation of data from the pilot phase of the study related to these drugs. \u0000 \u0000 \u0000 \u0000The total number of incidents was 55 in the pre-pilot period and 92 in the pilot phase, an increase of 67%. In the Primary Care sector there was a threefold increase in the pilot study while there was a 41% increase the Secondary Care sector. There was no overall reduction in the lag time of reporting incidents although in Secondary Care the median lag time in reporting incidents was 54 days in the pre-pilot phase compared to 38 days in the pilot phase (p = 0.008). Sixty five percent of incidents involved warfarin while low molecular weight heparin was involved in 20% of incidents. The most common type of incident related to blood testing (30%), particularly missing INR results, with communication failures being responsible for 20% of incidents. Prescription errors (16%) and errors in the amount of drug administered (14%) were other common causes of incidents. \u0000 \u0000 \u0000 \u0000Targeting increased the reporting rate to the NRLS in the selected area and reduced the lag time in submitting reports in the Secondary Care sector.
