In our study, the clinical, laboratory, genetic and radiological features of our UDD patients were evaluated. Method: In our study, the clinical, laboratory, genetic and radiological features of 12 patients with UDD were evaluated retrospectively. Results: Four (33%) patients presented with acute metabolic crisis in the neonatal period. One of the patients (citrullinemia type I) was diagnosed as intrauterine and his treatment was started as soon as he was born. The age at presentation of the patients ranged from 0 days to 12 years. The most common complaint in 4 patients with neonatal onset was sepsis-like clinical presentation, vomiting and coma occurring within the first 6 days after birth. Coma, mental retardation, gait delay, spasticity (arginineemia), growth retardation (LPI), avoidance of protein foods (LPI, OTC deficiency) were predominant in patients diagnosed outside the neonatal period. The age at presentation of late-onset patients ranged from 2 years to 12 years. Only one of the seven late-onset patients showed normal growth and mental development. Conclusion: UDD can be encountered not only in the neonatal period, but also in every period of life. In the presence of clinical suspicion, diagnostic tests should be planned quickly. Early diagnosis significantly affects mortality and morbidity.","PeriodicalId":30746,"journal":{"name":"Mustafa Kemal Universitesi Tip Dergisi","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2022-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67639957","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of our research is to share our experience of patients who underwent carotid endarterectomy under regional cervical block, and its effects on morbidity and mortality. Method: Between September 2020 and January 2020,a total of 61 patients who underwent carotid endarterectomy due to carotid stenosis, were examined retrospectively. The patients’ postoperative early results within the first 30 days are evaluated. The demographic characteristics, early operative data and early postoperative results (1 month) of the patients’ were analyzed. Age,sex,risk factors,surgical indications,diagnostic methods,the usage of shunt, duration of the operation, neurological events, morbidity and mortality were all evaluated. Results: The data of 61 patients were analyzed retrospectively. Fifteen of the patients were female and forty-six were male. The mean age of the patients was calculated as 61.6±9.1(48-88)years. Fifty-one of the patients were symptomatic. Ten of the patients were asymptomatic, however they had a carotid stenosis of 70 percent or above. In 11 of the 61 patients, carotid endarterectomy was performed bilaterally, so a total of 72 carotid endarterecomties were performed in 61 patients. The mean duration of the procedure was 77.6±19.7 minutes, and the average time of a clamp on the carotid artery was 29.4±11.5 minutes. Major neurological events, bleeding, infections or death, were seen in none of the discharged patients within 1 month postoperatively. Conclusion: With increasing experience, carotid endarterectomy can be performed safely under regional cervical block.","PeriodicalId":30746,"journal":{"name":"Mustafa Kemal Universitesi Tip Dergisi","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2022-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48962908","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Surgical treatment can be applied to a limited number of patients and the results are still not satisfactory. In this study, it is aimed to present the results of patients treated for anaplastic thyroid carcinoma in our clinic to the literature. Materials and Methods: The files of patients who were treated with the diagnosis of anaplastic thyroid carcinoma in our clinic between 2015-2020 were retrospectively analyzed. Ten patients over 18 years of age, who underwent surgical treatments in our clinic, were included in the study. Patients’ age, gender, complaints, ultrasonography, computed tomography, needle biopsy results, preoperative vocal cord examinations, surgical and oncological treatments, and survey information were recorded and analyzed. Results: 6 of the patients were male and 4 were female. The average age was 68.5 (53-86). All patients had shortness of breath due to compression. Computed tomography scans revealed lung metastasis in 5 patients and regional lymph node metastasis in 4 patients. Fine needle aspiration biopsy was performed in eight patients and the result was consistent with malignancy. In preoperative vocal cord examinations, 7 patients had unilateral paralysis. Surgical treatment was applied to all patients, chemoradiotherapy to 8 patients and radiotherapy to 2 patients. It was observed that all of the patients showed signs of postoperative progressive disease and died. Average surveillance was 150 (4-420) days. Conclusion: Anaplastic thyroid carcinoma is a disease that is difficult to diagnose and treat. Despite being managed with a multidisciplinary approach, it still has high mortality rates. It should be kept in mind that patients with a history of thyroid pathology may develop anaplastic carcinoma and should be evaluated carefully.","PeriodicalId":30746,"journal":{"name":"Mustafa Kemal Universitesi Tip Dergisi","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2022-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67638052","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Abstract Urinary Retention after Hemorrhoidectomy: Can we reduce risk? Objective: Hemorrhoidectomy is a common surgery and urinary retention (UR) after anorectal surgeries is an unwelcome complication. In this study we aimed to investigate risk of UR after hemorrhoidectomy and predict the factors associated with UR. Methods: Between 2016-2018 years, 180 patients who undergone hemorrhoidectomy by general anesthesia were included study. The patients with urological symptoms and urological surgery before were excluded. Results: Median age of patients was 45.5 (IQR: 40.0-55.0) years. There was 116 (64.4%) of male and 64 (35.6%) of woman. Urinary retention was seen significantly higher in men (p=0.03) but, visual pain scale (VPS) and the analgesic requirement were similar between men and women (p=0.39 and p=0.86, respectively). Regression analysis showed that male gender, operation time and analgesic requirement was not associated UR and the VPS is only predictive factor for UR (OR:0.224, p<0.001). Conclusion: The UR is an often and worrisome complication. The VPS is the important predictive factor for urinary retention. The sufficient analgesia could be the key to prevent this complication.","PeriodicalId":30746,"journal":{"name":"Mustafa Kemal Universitesi Tip Dergisi","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2022-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67638174","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Antiaggregant therapies have an important place in the secondary prevention of cardiovascular diseases. In this study, the effects of regular use of acetylsalicylic acid on TIMI flow grade and myocardial damage will be investigated in patients presenting with acute coronary syndrome. Methods: The patients who admitted with the clinic of acute coronary syndrome and underwent coronary angiography were included in the study, retrospectively. The patients were divided into two groups as those who used acetylsalicylic acid regularly (group 1) and those who never used acetylsalicylic acid (group 2). Demographic data, laboratory data, TIMI flow grade and troponin-I values as a marker of myocardial damage were compared between the groups. Results: 440 patients were included in the study. There were 122 patients in group 1(mean age 64.9±1.0 years; 25(20.5%) women) and 318 patients in group 2(mean age 61.0±0.7 years; 83(26.1%) women). Advanced age, hypertension, diabetes mellitus, hyperlipidemia and history of coronary artery disease were observed at a higher rate in group 1. TIMI-0 flow was observed at a significantly lower rate in patients in group 1(30.3% vs. 47.8%; p=0.004). Additionally, the maximum measured troponin values of the patients in group 1 were found significantly lower (median 5486 pg/ml vs. 15740 pg/ml; p<0.001). Conclusion: TIMI flow grades were higher and the amount of enzymatic myocardial damage was found less in patients who admitted with the clinic of acute coronary syndrome and used previously regular acetylsalicylic acid. Thus, the importance of secondary protection was emphasized once again in this study.","PeriodicalId":30746,"journal":{"name":"Mustafa Kemal Universitesi Tip Dergisi","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2022-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67640033","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
