Materials and Methods: The present study was observational, descriptive, cross-sectional and prospective, with a sample of 386 participants, conducted with a questionnaire validated by expert judgment and established reliability through a pilot test. It was structured in two sections, covering sociodemographic, economic and health characteristics of patients with chronic diseases during the COVID-19 pandemic and the four dimensions of accessibility (physical availability, affordability, geographic accessibility and acceptability). Results: 92.5% didn’t had full accessibility to medicines. 45.9%, 53.4%, 83.9% and 23.3% had physical availability, affordability, geographic accessibility and acceptability of medicines, respectively. When accessibility was related to associated factors, it was found that there was a statistically significant relationship (p<0.05) with age, number of people living in thehousehold, monthly income, number of medications and evolution of the disease. Conclusion: The majority of patients treated in health centers and hospitals in Chiclayo did not have full accessibility to their medications during the COVID-19 pandemic, and this was related to the number of people, monthly income, evolution of the disease, age and number of medications.","PeriodicalId":302638,"journal":{"name":"Revista Experiencia en Medicina del Hospital Regional Lambayeque","volume":"64 25","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141651659","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Nowadays, the lack of human connection is a challenge for the nursing professional, so the humanized care provided to patients living with HIV (PLWHA) should not only be practiced in the Sexually Transmitted Infections Strategy ( STI) - HIV/AIDS and Hepatitis B, but in all the services of a hospital where a patient with this reality is treated, a problem that is evident every day when visiting patients living with HIV, admitted to the services. of hospitalization or who go to the hospital to be treated on an outpatient basis, where the nursing professionals who work in these areas often do not demonstrate empathy with the patient, feeling even in this century, stigma and discrimination in their care, and everything is given by a backbone fact: ignorance. Therefore, it is concluded that undergraduate training in the professional nursing career should begin, not only strengthening knowledge about the prevention of sexually transmitted infections, but also the importance of providing comprehensive care to patients. patients living with HIV/AIDS, breaking prejudice, since the holistic and humanistic vision of care must be the foundation in the training of students, being the engine of change to mold the nursing professional by providing dignified, quality care, with knowledge, based on values, promoting a culture of respect for human rights with a gender focus, to eliminate the stigma and discrimination associated with the infection.","PeriodicalId":302638,"journal":{"name":"Revista Experiencia en Medicina del Hospital Regional Lambayeque","volume":"26 26","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141651014","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objetivo: Determinar si el uso de antibioticoterapia empírica se asocia con mortalidad a los 28 días en pacientes con sepsis por SARS-CoV-2 en el Servicio de Emergencias y Áreas Críticas del Hospital Regional de Lambayeque, Perú. Material y Métodos: El estudio de cohorte prospectivo de 102 pacientes con diagnóstico de sepsis por SARS-CoV-2, donde 52 recibieron antibioticoterapia empírica y 50 no recibieron antibioticoterapia empírica, de febrero a septiembre del 2021. Resultados: Se encontraron diferencias significativas con valores elevados de procalcitonina (p=0,03) y leucocitos (p=0,025), valores bajos de albúmina (p =0,01), y puntuaciones elevadas en la escala SOFA (p=0,022) en pacientes que recibieron antibioticoterapia empírica. El análisis de supervivencia de Kaplan Meier determinó que la antibioticoterapia empírica por sí sola no es una variable significativa para la mortalidad (p=0,89); sin embargo, el análisis de regresión multivariable de COX obtuvo que el uso de antibioticoterapia empírica (HR 1,9 IC 95% 1,02-3,91), PafiO2 (0,99 IC 95% 0,98-1,00), días libres de ventilación mecánica (HR 0,89 IC 95% 0,84-0,94) y la edad (HR 1,03 IC 95% 1,003-1,051) son predictores de mortalidad. Conclusiones: Los resultados de este estudio muestran discrepancias y no permiten concluir de manera definitiva si existen diferencias estadísticamente significativas en la mortalidad entre el grupo que recibió antibioticoterapia y el grupo que no la recibió. Se requieren investigaciones adicionales para obtener conclusiones más sólidas en relación con este aspecto. Sin embargo, las coinfecciones bacterianas fueron poco frecuentes, en pacientes que recibieron antibióticos. Además, la asociación encontrada entre el uso de antimicrobianos y la disminución de los niveles de albúmina, incremento de leucocitos y puntuaciones SOFA requieren un mayor estudio para comprender mejor sus implicaciones clínicas.","PeriodicalId":302638,"journal":{"name":"Revista Experiencia en Medicina del Hospital Regional Lambayeque","volume":"21 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140739139","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
