[Neuritis of multiple cranial nerves in idiopathic focal pachymeningitis]. 特发性局灶性厚脑膜炎的多颅神经炎。

{"title":"[Neuritis of multiple cranial nerves in idiopathic focal pachymeningitis].","authors":"P Pedrazzi,&nbsp;J P Deruaz,&nbsp;F Regli","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A 76 years old patient suffering from insulin dependent diabetes presents a multiple, bilateral and asymmetric cranial neuropathy involving on the left side the cranial nerves VIII, IX with a participation of the efferent autonomic fibers of the nerves VII and IX (xerostomia) and the nerve XII, and involving on both sides the nerves VII and X. Like it is known from literature the nerve XI was spared, probably because of the more caudal emergence from the brainstem. The disease was preceded by a violent otalgia on the left side which was initially interpretated as a malignant external otitis. This affection was associated with an inflammatory syndrome that was easily monitored by the blood sedimentation rate. The course was favorable but marked by fluctuations of the neurological deficits. The MRI shows a pachymeningitis localised at the beginning in the medial fossa and on the cerebellar tentorium, later on the left parieto-occipital convexity. The diagnosis of a focal idiopathic pachymeningitis was confirmed by excluding different other affections like inflammatory, tumoral and infectious diseases, by using MRI examinations, CSF analysis and especially meningeal biopsy. It is a rare inflammatory disease of unknown origin with potentially persistent neurological deficits. The theory bases on three schizophrenic operations: The loss of thinking correlated to the absence of the psychical representative for the act of violence. The splitting of the ego by which the patient knows of his violence but does not remember it. The paradoxon of speaking and not speaking about violence in the same time. The active use of these psychotic mechanism in the therapy and technique is showed by casuistics. The annual incidence rate was found to be 0.92 per 100,000 population, while the prevalence rate was 3.88 per 100,000 population, with a preponderance for males. Strong yearly fluctuations in the number of newly detected cases average out over the whole period with no trends of increased incidence. Stimulant abuse in schizophrenics has apparently increased over the years, eventually alcoholic abuse, whereas no increase is evidenced for cannabis abuse. Patients with dual diagnosis have an increased risk for morbidity and mortality, for psychotic relapse and for social disintegration. No increased risk for schizophrenic psychosis is found for drug abusers. Preferential binding of neuroleptics to D2-receptors in the limbic system, as well as the combination of dopamine D2- and serotonin S2-antagonism proved to be effective. As obstacles are most of all mentioned the lack of motivation and the excessive acting out of these patients. Because of the development of the personality of dissocial people motivation in the ordinary sense can not be expected. But this does not mean that therapy is impossible. These patients are often rejected by therapists because they feel injured by the patients' rejection and by their devaluation of the therapeutic attention. We find the same dynamics concerning the excessive acting out or these patients. For both types of depression there was a significant decrease from day 1 to day 21 in the mean HAMD total score and in a subscore of cognitive and somatic items. The tension field of (social-)psychiatric treatment].","authors":"H Hoffmann","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>As generally in psychiatry the treatment concepts in social psychiatry base to a great extend on disease- and deficit models. These models, however, have their limits and run the risk to promote exactly that, what social psychiatry is fighting against: chronification. The question shall be discussed, if the systemic approach could be helpful to reduce (social) psychiatry's contribution on the process of chronification. Since ever psychiatric institutions have the mandate of social control and exclusion of social disturbing people from society. On the other hand, social support has become a cornerstone of rehabilitation in psychiatry. There the therapist becomes the manager of the disability. The (case-)manager subsequently takes over the responsibility for the future course. So he also gets power over the system to be treated. He decides, how much of social support and control is necessary. But then he gives up the aim of profoundly changing something in the sense that the patient can give up his symptoms and take more self-responsibility for his acting. The consequence is that the patient and psychiatry keep up each other: a coevolution develops. This leads to rigidity and makes changes difficult. By taking over responsibility the therapist becomes part of the system to be treated. So he looses his neutrality and also the possibility of therapeutical influence and change. The whole system quickly moves into a vicious circle of helplessness.
