In this study, it was aimed to compare central corneal thickness in terms of average keratometric values and axial length in control group and eyes with PE with or without cataracts. Files of the cases who were admitted to Gaziantep University, Medical Faculty, Ophthalmology Clinic between January 2004 and February 2011, and diagnosed as PE were analyzed retrospectively. All patients were performed a detailed ophthalmologic examination including ultrasonic pachtmetry, automatic keratorefractometry and ultrasonic biometry measurements. Study group was consisted of a total 159 cases with PE (115 cases with cataract and 44 cases with no cataract) and 60 healthy subjects (control group). Mean central corneal thickness value of the cases with PE was 537.20±5.24 microns, axial length values was 23.31±1.03 mm, and mean keratometric values was 44.06±0.24 diopter, while these values for control group were 556.23±13.21 micron, 23.12±0.98 mm, and 44.28±0.65 diopter, respectively. When PE cases and controls were compared, the difference was significant (p0.05). Mean central corneal thickness value, axial length, and mean keratometric value of the cases with PE cataract were 541.93±48.29 micron, 23.07±1.32 mm, and 44.59±1.29 diopter, respectively. These values were 540.28±4.34 micron, 23.31±1.03 mm, and 44.06±0.24 diopter for the cases without PE cataract. A significant difference was not detected between the cases with and without cataract in terms of average central corneal length, axial length, and keratometric values (p>0.05). Cornea was seen to be thinner in cases with pseudoexfoliation compared to control group. We present a case of a 24-year-old man who spontaneously developed lesions of unknown cause consistent with cutaneous lymphoid hyperplasia. {"title":"Admission appropriateness and profile of the patients attended to a state hospital emergency department","authors":"A. Köse, Beril Kose, M. Öncü, F. Adult and pediatric trauma patients admitted to the emergency department of Van State Hospital between March 01 and March 31, 2010, in the course of one month period, were retrospectively analyzed. Data of the 32800 patients admitted to the emergency department during this period were evaluated. Male and female ratio of these patients was 54.8% and 45.2%, respectively, and the most intense group with 77% was the age group of 17-65. The majority of the applications with 60.5% occurred between 8 a.m. and 17 p.m. An examination was asked for 50% of the patients. It was determined that emergency examinations and therapies were applied to 9.9% of the patients, consultation rates were 4.5%, and the highest consultations were taken from orthopedic clinics (16.1%). It was established that 1.4% of the patients were hospitalized, with the highest admission to the general surgery clinic (13.8%). In hospitalized patients, the most common diagnoses were abdominal pain (9.6%), and 88.4% of the patients were discharged from the emergency department. It was observed that the majority of patients admitted to the emergency department need not have been in emergency, and should have applied to primary health care services and policlinics. The study sample was comprised of 154 health care workers who nurses, lab technicians, emergency technicians and anaesthesia technicians. Participants were given a questionnaire consisting of 17 questions assessment of the training regarding hospital service quality system. This study presented that health care workers interest the service quality standards and the training increases hospital service quality standards. In conclusion, although increasing awareness and the settlement of concepts are still shortcomings, service quality standards under the provision of services and infrastructure that we continue to work at the university hospital, knowledge and the training of our employees in achieving the goal of this point can be considered. Donma","doi":"10.5455/GMJ-30-2013-164","DOIUrl":"https://doi.org/10.5455/GMJ-30-2013-164","url":null,"abstract":"The aim of our study was to assess whether endothelial function of the brachial artery is normal or impaired in children born at term with low birth weight (LBW) compared with their normal birth weight (NBW) peers, because there are still few data on this subject in children born at term with LBW. We compared brachial artery flow mediated dilatation (FMD) in children with LBW (n= 55, 30 male) and NBW (n= 45, 24 male) who were born at term. Age, gender, weight, height, waist and hip circumference, body mass index, body mass Z score, fasting glucose, total cholesterol, triglycerides, low density lipoprotein-cholesterol, high density lipoproteincholesterol, calcium, C-reactive protein, hemoglobin, serum insulin concentration, homeostatic model assessment index, creatinine values, brachial artery baseline diameter and brachial artery post dilatation diameter were similar in both groups (all p values >0.05). Compared with the NBW group, the LBW group had lower FMD (%) (6.68 ± 2.1 vs. 7.8 ± 1.9, p = 0.004). Brachial artery FMD was negatively correlated with waist circumferences (r = 0.521, p < 0.001). A general linear analysis model, FMD as the dependent variable revealed significant effect of waist circumference (β ± SE:0.07±0.02, p<0.001) and LBW controlling for age (β ± SE: -0.028±0.01, p = 0.004). LBW closely affects to FMD. Optimal adherence is the compliance to ART that achieves a sustained plasma drug concentration that suppresses viral replication. The objective of this study is to assess ART adherence and its determinants at Agaro Health Center ART Clinic, South Western Ethiopia. Facility-based cross sectional study was conducted from June 9-29, 2011. Patients were interviewed to collect adherence related information by using pretested semi-structured questionnaire. Subsequent charts of prescribed medications were reviewed using a pretested data collection format. A total of 209 respondents were included in the study, of which 127 (60.8%) were females and 82 (39.2%) were males. The average adherence rate was 98.1% among males and 99.5% among females. The overall average adherence rate among the respondents was 98.8%. One hundred sixty one (77.0%) respondents had a 100% adherence rate, 15 (7.2%) patients had a 93.3% adherence rate and the remaining 33 (15.8%) patients had adherence rate between 95 and 98% on average. The common reasons for missing of the prescribed doses were forgetting 27 (39.7%), being too busy 20 (29.4%), away from home 17 (25.0%) and being extremely ill 4 (5.9%). Of all the variables, sex, living alone, monthly income, use of stimulants and medications in addition to ART were significantly associated with non-adherence. Between November 2010- December 2011 patients applied Yahyali State Hospital Algology Clinic withlow back pain for at least 3 months was evaluated in a prospective study. Application of the lumbar epidural steroid injection was planned for 150 patients. visual analogue scale (VAS) were used for scoring pain of patients. Besides, patients with hand-finger floor distance and the degree of straight leg raising were evaluated before and after ESE injection. Ages of patients were between 27- 65 years old, of all patients 67 were men (44,7%), 83 were women (%55,3). Before and after treatment in VAS, direct leg increase, hand- finger ground distance of patients were found to be significantly different between measurements one by one (p=0,0037, p=0,0053, p=0,0062). The goal of this prospective study is to investigate the effects of per ioperative nutritional support on postoperative morbidity and mortality in severely malnourished gastrointestinal tract cancer patients. Seventyone patients with the diagnosis of gastrointestinal tract cancer were included in this study. Subjective Global Assessment index was applied for patients' nutritional state. The patients were divided into two groups, severely malnourished (Group 1) and nonseverely malnourished (Group 2: well-nourished and mildly malnourished). The daily energy requirement of 36 patients in Group 1 was calculated and total parenteral nutrition was applied 30-35 kcal/kg/day by average for preoperative 7 days and postoperative 3 days to these 36 patients. Perioperative total parenteral nutrition was not given to 35 patients in Group 2. Patient morbidity and mortality was followed during the hospitalization period. Total morbidity was defined as; major complications, minor complications and complications related to total parenteral nutrition. Major complications occurred in 7 patients (19.41%) in Group 1 and 6 patients (17.11%) in Group 2 (p=0.80). Total morbidity was in 16 patients (44.40%) in Group 1, and 17 patients (48.57%) in Group 2 (p=0.727). Mortality was seen as one patients in each group postoperatively. Twenty four years old man presented with visual impairment in our outpatients clinic. Bilateral anterior lenticonus were seen in ocular slit lamp examination. After clinical examinations, audiological tests and nephrological evaluation, the case was diagnosed as Alport's syndrome. Although males and females are affected equally in this disease, males have poorer prognosis than females owing to renal failure. It is acknowledged that the efficacy of antioxidant defense mechanism is reduced by the changes in trace element levels. These changes also increase the negative effects of free oxygen radicals on cell integrity. It is known that hs-CRP is a marker in predicting metabolic syndrome (MS) and its parameters. In this study we aim to find some other markers and compare the relation of Cu and Zn levels with MS and MS parameters.410 patients (90 men and 320 women) were included in this study. The measurement of waist and hip circumferences are performed. Fasting plasma glucose, postprandial plasma glucose, insulin, copper, zinc, lipid profile, hsCRP levels were examined. All the analyses were done according to diagnosis criteria of NCEP-ATP III. BMI, HDL and Cu levels were significantly higher in women when compared to men (P < 0.001). As hsCRP does, blood copper levels were found to predict MS and MS parameters. However, it is different from each other in that copper levels predicted different MS parameters at different sensitivity and specificity. We also observed that there was a more significant correlation between TG level, blood pressure and Cu, compared to hsCRP. When all these results are taken into consideration, Cu can be thought as an alternatýve marker in predicting MS and MS parameters.
