Büyükbayram","doi":"10.5455/GMJ-30-39089","DOIUrl":"https://doi.org/10.5455/GMJ-30-39089","url":null,"abstract":"Primary thyroid lymphomas are lymphomas which are localized only to thyroid gland or thyroid gland and surrounding lymph nodes. Frequently these are mucosa associated lymphoid tissue (MALT) lymphomas with B cell origin. Our case is a 78-year-old female patient who admitted with a neck mass and had been operated with the suspicion of malignancy. Histopathological examination revealed diffuse proliferation of atypical lymphoid cells that consists of small to medium sized cells, with irregular nuclear contours, some of which resemble centrocytes. Immunohistochemical staining was positive for CD20, CD79alpha, CD43, and lambda while negative for CD3, CD30, CD15, bcl-2, bcl-6, CD68, CD138, CD5, and CD10. With these finding, the case was considered as “primary thyroid MALT lymphoma”. Thyroid MALT lymphomas are characterized with the presence of atypical lymphocytes that originate from the marginal zones of lymhoid follicles. Sixty seven patients with near total or total thyroidectomy were included in the study. Fifty patients did not have lymph node or distant metastases (Group 1), and 17 patients had distant metastases (Group 2). Tg and anti-Tg levels of these patients were measured before RAI treatment when TSH>30 mU/l. Serum Tg levels were found 8,5±10,6 ng/ml and 307±142 ng/ml in Group 1 and Group 2, respectively. Bayram","doi":"10.5455/GMJ-30-2011-44","DOIUrl":"https://doi.org/10.5455/GMJ-30-2011-44","url":null,"abstract":"Evaluating the performance of magnetic resonance imaging (MRI) and comparison with mammography, ultrasound (US) and histopathology results in cases of women with suspicious breast lesions. Forty nine cases on which histopathology was performed included in the study. All cases were applied mammography and US, and then MRI. Biopsy or post-operational results have been generated. Among the 49 cases, 27 (55%) benign and 22 (45%) were malign lesions. Using mammography, of the 49 cases, 24 (49%) were deemed to be malign. True positive cases were 20 (41%). In ultrasound, 25 (51%) were malign, 24 (49%) were benign. True positive cases were 21 (43%). Lesions were detected in all of the 49 cases using MRI. All of the 22 (45%) malign cases were diagnosed as malign lesion. Sensitivities of mammography, US and MRI in detecting lesions were 83%, 95% and 100%, and specificities were 85%, 85% and 92% respectively. In MRI, all cases were applied dynamic contrast sequences, for the cases with lesions detected, time signal intensity (SI) curves were drawn. 23 cases were detected as Type 1 (47%), 2 cases were Type 2 (4%), 24 cases were Type 3 (49%) SI curve. According to SI curves sensitivity in detecting malignities was 95% and specificity was 81%. MRI has been found superior to mammography and US in detecting masses especially with its characteristics of higher spatial resolution, less binding of dynamic properties evaluation on user. A 73-year-old women presented with purulent discharge from one of her previous port sites one year after she underwent LC. The latter revealed a round opaque mass in an abscess like cavity, and subsequently an ultrasonography showed a round echogenity with acoustic shadow posteriorly. Axial CT images verified the presence of a well-circumscribed hyperdensity which was really a gallstone in a cystic mass. To reduce this complication, excessive traction of the gallbladder should be avoided during dissection. Prior to extraction, gallbladder contents should be aspirated, and to extract larger stones. In terms of molecular biology, mouse hepatitis virus (MHV) is the best-studied CoVs, and it is consequently considered a model system for the family of Coronaviridae. TMDs are generally characterized by pain, joint sounds, and restricted mandibular movement. In addition to these symptoms pain, hearing loss, headache, vertigo, tinnitus, burning at tongue, nose and sinus can be associated with TMDs. Epidemiological studies have been reported that approximately 75% of the population present one sign of TMD and 25% present at least one symptom. However, 3-7% of the population has severe problems to look for treatment. TMD is a multifactorial disorder and noninvasive methods are the the first stage for the treatment of TMD. One of the noninvasive physical methods used for pain management in physical therapy is low level laser treatment (LLLT). LLLT has analgesic, antienflamatuar and regenerative effects. LLLT is applied easy and received result in a short period. They were diagnosed as cervical or lumbar disc herniation according to their clinical and radiological findings. The demographical features, medications, duration of each medications, concomitant diseases, routine test results were recorded. The severity of pain, health related quality of life was evaluated by Visual Analogue Scale (VAS), Short Form 36 (SF 36) in respectively. Transdermal fentanyl was applied as 12 μg/h patch after the first evaluation. Mean age of patients with low back pain and neck pain was measured as 43.6±10.5 and 48.7±10.6 years, respectively. There were no significant differences between two patients group in respect to age, gender, erythrocyte sedimentation rate (ESR) and c-reactive protein (CRP) levels (p>0.05).
