Reserving resources of the health care system to combat the spread of the new coronavirus infection makes it relevant to identify unused reserves for reducing mortality from diseases of the circulatory system through a more efficient use of the resources available to the regions to control cardiovascular diseases, as well as using resources of the neighboring regions through effective schemes of interregional routing of patients. Purpose. To assess performance and predictive accuracy of the developed information system \"Cardionet\" in ambulance service and, using this system, to analyze the existing routing schemes for patients with acute coronary syndrome (ACS) in the Kursk region to select the optimal routing schemes in terms of the time spent on transporting patients with ACS to cardiovascular centers, including cases of interregional cooperation. Material and methods. Performance and predictive accuracy of the \"Cardionet\" information system were analyzed on the basis of the ambulance service of the Kursk region. The digital twin of the Kursk region was used to study the time indicators of transportation of patients with acute coronary syndrome by ambulance teams to medical organizations on the basis of the current procedure versus routing with automatically determined optimal route, including during interregional cooperation with cardiovascular centers of the neighboring regions. Results. The difference between the estimated and actual time of transportation when using the “Cardionet” information system equaled to 1.17 ± 5.52 minutes on average (p=0.1534), which is statistically insignificant and indicates a high predictive accuracy of the software product for calculating the patient transportation time. Due to introduction of the interregional cooperation, the regional average transportation time for patients with acute coronary syndrome with ST segment elevation was reduced by 10.43 ± 5.39 minutes, with the estimated decrease in mortality from myocardial infarction equaling to 2.62%. In four districts of the region, the time saved ranged from 41.5 ± 19.99 to 95.4 ± 21.52 minutes, and the predicted decrease in mortality from myocardial infarction - from 10.43% to 23.91%. In six out of 28 districts of the Kursk region, the routing of patients with acute coronary syndrome without ST-segment elevation is suboptimal in terms of the transportation time. Improvement of the routing schemes using the “Cardionet” information system will save 11.63 ± 4.37 to 40.05 ± 13.65 minutes on average. Conclusion. The principles of lean manufacturing gained momentum in the priority project \"Creation of a new model of medical organization providing primary health care (2017-2022)\". Increasing patient satisfaction, access to services, elimination of the existing time, financial and other losses, as well as organization of safe and comfortable workplaces for employees improve performance effectiveness and efficiency of medical organizations. The purpose of the study. Scientific substantiation of the adoption of evidence-based management decisions for developing primary healthcare sector based on results of the independent comprehensive assessment of primary care quality and accessibility in the Stavropol Territory. Material and Methods. The present study assessed the quality and accessibility of primary health care in the Stavropol Territory over the period 2014-2020 according to the improved methodology of integral indicators and well-being index with adaptation to the structural transformations in health care. All medical organizations providing outpatient medical care (multidisciplinary hospitals and central city hospitals with attached population; individual polyclinics; district hospitals and central district hospital; local hospitals and outpatient clinics) were included in the study. The methodology has been adapted and improved to reflect changes in information sources in accordance with the adjustment of federal and sectoral statistical observation forms in 2019-2020. Conclusion. A comprehensive assessment of quality and accessibility of primary health care in the Stavropol Territory in 2014-2020 showed a multidirectional well-being index as an integral assessment criterion. In the period 2014-2019, all medical organizations that provide primary health care had a pronounced positive well-being index. In the conditions of the COVID-19 pandemic, the primary health care sector has been actively involved in providing diagnostics and treatment to patients with COVID-19, reflected by the decrease in the average well-being index in dynamics in comparing 2020 to 2014. However, the study results show that the decrease in the well-being index was not pronounced due to sustaining lean manufacturing principles in the primary care sector during the COVID-19 pandemic. Scope of application. The purpose of the study is to conduct a chronological description of the main models of health care systems in the world and an assessment of modern trends in medical care provision to the population Material and methods. To analyze scientific literature, results of modern research on implementing various models in health care systems of the highly developed countries were used. Results. The main characteristics of the models of health care systems in the world, which are fundamentally different from each other, have been outlined: the state-funded (budgetary, public system), which is most comprehensively and conspicuously realized in the UK; predominantly the insurance system (most fully and clearly implemented in Germany and Israel) and the private (paid) system (most typical of the U.S. healthcare). Conclusion. Analysis of the global trends in health care development shows that there is no ideal model of the health care system in the world. Emergence of such phenomenon as \"COVID nihilism\" and \"COVID dissidents\" can’t stop the COVID-19 pandemic. The purpose of the study is to review factors determining COVID nihilism in the fight against the COVID-19 pandemic in the world and in Russia. Material and methods. The information base of the study included publications in periodicals and the Internet, statistical materials of the Center for Systems Science and Engineering (CSSE) at Johns Hopkins University (JHU), YandexCoronavirus. Statistics and methods: general logical, analytical, statistical analysis and empirical research. Results. The main reasons for COVID nihilism and opposition to vaccine and mask mandates included imposition of freedom, independence of one's own body, belief in conspiracy theories, and concern about the side effects of masks and vaccines. The Internet activity to sabotage anti-COVID measures in the Russian Federation is rather pronounced, yet it failed to result in any riots and pogroms, unlike the EU countries. The share of people vaccinated against COVID-19 in the Russian Federation is twice lower compared to China, for example, due to citizens' concerns about the need for vaccination and fear due to the low level of virology literacy among the population. The main barriers to stop the pandemic are disbelief that the pathogen SARS-CoV-2 exists, disbelief that masks protect against infection and microdroplets containing viral particles, improper face mask wearing (inside out), violation of social distance, misinformation on the virus prevention and spread, anti-vaccine attitude due to misunderstanding of the process of vaccine development and action, social instability and mistrust in the authorities. Consequences of the ongoing pandemic include burden on healthcare system, excess mortality and post-COVID complications affecting quality of life of the entire population and duration of a healthy life. The purpose of the study was to analyze the main indicators of physical development and cardiovascular system in young people of the North who leave and do not leave the region for a summer vacation. Material and methods: the author has conducted a survey of 126 young residents of the Magadan region aged 16-18 (the average age 17.2±0.8 years), who were divided into the following two groups: young people traveling outside the Magadan region for a summer vacation on an annual basis or once in two years and young people who have not left the region for vacation for more than 3 years (on average 7±1.2 years)). The main indicators of physical development and cardiovascular system were determined in the survey participants. Results: The survey shows that 37% of young people travel outside the Magadan region for vacation on an annual basis, 40% leave the region every two years and 30% have not left the region for more than 3 years, while 3% of the surveyed sample have never left the Magadan region. According to the data obtained, in the group of people who do not leave the region for vacation, imbalances in the cardiovascular system have been registered, manifested by a significant increase in systolic blood pressure against the background of increased processes of mechanical activity of the heart, manifested in the increased myocardial oxygen demand and gas exchange in general. Conclusion: The study results expand knowledge about positive contribution of summer vacation to the health and well-being of the younger generation of the Northern region. It should be particularly noted that the beneficial effect of leaving the Magadan region for a summer vacation can be objectively measured by indicators of the cardiovascular system. Population health is the center of the paradigm, being the most important condition as well as the result of achieving targets of sustainable development in the interests of present and future generations. The purpose of the study is to assess Russia's progress in reducing mortality from chronic non-communicable diseases (NCD) in the context of changing lifestyle of the population and to determine the possibility of achieving targets of the Sustainable Development Goal 3 by 2030. Material and methods: calculation of standardized mortality rates was based on Rosstat data on deaths from major chronic NCD in the Russian Federation among people aged 30-69 in 2000-2020; calculation of predicted mortality values until 2030, with and without taking into account the impact of the pandemic, was based on the approximation of the indicators dynamics; secondary analysis of data from scientific publications, as well as results of the selected Rosstat studies on prevalence of risk factors and its changes in the period after 2000 was undertaken. Results. Since 2015, Russia has been successfully moving towards achieving the Sustainable Development Goal 3: mortality from chronic non-communicable diseases among people aged 30-69 has decreased by 10.3% by 2019. Positive shifts in mortality developed against the background of the improved lifestyle of the population. Noticeable progress has been made over the past 10 years in reducing tobacco and alcohol consumption. As far as physical activity and a healthy diet are concerned, the number of adherents to these components of a healthy lifestyle is yet to increase. The coronavirus pandemic has led to increased mortality from three of the four monitored diseases, in addition to neoplasms, since they are associated with COVID-19. In a pandemic situation, commitment to a healthy lifestyle has decreased, and consumption of alcohol, tobacco and drugs has increased as a response to stress. Opportunities for a healthy lifestyle have been limited due to economic hardship. Conclusion. Setting goals to reduce mortality from chronic non-communicable diseases in the framework of the Sustainable Development Goals in Russia fell on the period of sustainable positive trends. Significant results have been achieved in terms of reducing alcohol consumption and smoking. However, it was not possible to achieve higher physical activity and improvement in nutrition. However, linguistic determinants of health have received little scholarly attention. The present article highlights some challenges in the design of epidemiological studies that would credibly demonstrate the impact of linguistic factors on health outcomes on the example of transgender issues. The purpose of the study is to analyze the impact of linguistic factors on health outcomes of transgender individuals. Identify methodological challenges in the design of epidemiological studies that would credibly prove causation between linguistic factors and health outcomes. Material and methods. Content analysis of literature on the impact of hate speech, misgendering and deadnaming on health outcomes of transgender individuals. Results. Qualitative and quantitative data shows a negative impact of such linguistic practices as hate speech, misgendering and deadnaming on mental and physical health of transgender individuals. The purpose of the study: to conduct a dynamic analysis of the level and structure of mortality of patients with diabetes mellitus in the Samara region for the period from 2000 to 2017. Material and methods: the object of the study was a depersonalized database of the state register of diabetes mellitus in the Russian Federation and the Samara region and the death registration form of patients with diabetes mellitus of the annual report on diseases of the endocrine system. The mortality rates were calculated, the structure of mortality of patients with diabetes mellitus was analyzed according to the ICD-10 disease classes individually for diabetes mellitus type 1 and 2. Results: trends in the dynamics in mortality of patients with diabetes mellitus in the Samara region generally correspond to the Russian ones. Significant differences in the trends in mortality rates in patients with type 1 diabetes mellitus have been identified: the mortality rate in the Samara region increases against the background of the decreased indicator in the Russian Federation, and different vectors of the dynamics in mortality from atherosclerotic cardiovascular diseases are registered. Conclusion: the main reason for changes in the long-term decreasing trend in mortality from diabetes mellitus in the Samara region in 2010-2017 was changes introduced to the procedure for registering causes of death. When entering data into the state register of diabetes mellitus, if the cause of death is indicated as \"diabetes mellitus\", it is mandatory to indicate the direct cause of death.
