Banteva","doi":"10.21045/2071-5021-2022-68-1-11","DOIUrl":"https://doi.org/10.21045/2071-5021-2022-68-1-11","url":null,"abstract":"Somatic morbidity in parturient and puerperant women, mainly diabetes mellitus and anemia remains an urgent problem of the modern obstetrics due to the multiple negative effect on both maternal and newborn health. Dynamic analysis of this morbidity makes it possible to identify priorities in preventive activities to ensure effective coordination of the multidisciplinary team of highly qualified specialists. Purpose. To study dynamics in somatic morbidity complicating labor in the Russian Federation. Material and methods. The analysis was carried out according to the form of statistical observation No. 32 «Information on medical care for pregnant, parturient and puerperant women» and on the basis of data from the collections «Main indicators of maternal and child health, performance of mother and child services in the Russian Federation» for 2016-2020, using methods of descriptive statistics, calculation of the increase and decrease rates of indicators, as well as prognostic options for diseases. Results. In 2016-2020 Russia registered increased morbidity among parturient and puerperant women with diabetes (+ 170.33%; 31.88 ‰ - 86.18 ‰), venous complications (+ 27.33%; 19.9 ‰ - 24.30 ‰) and anemia (+ 5.65%; 247.68 ‰ - 261.70 ‰). Diseases of the cardiovascular system (-15.76%; 57.11 ‰ - 48.10 ‰) and urinary tract infections (-7.44%; 51.13 ‰ - 47.30 ‰) tend to decrease. According to the calculated forecasts, a similar trend will remain until 2025. At the same time, there are pronounced regional differences in indicators with multidirectional nature of the dynamics. Conclusion. Dynamic analysis of morbidity makes it possible to identify priorities for preventive activities. Medical activity is a person's practice in the field of protecting one's own and public health, expressed in compliance with medical recommendations and hygiene rules, a complex of therapeutic and preventive measures for improving health, identifying and eliminating pathological changes, functional disorders, and preventing diseases. The basis of medical activity is awareness, which is formed through effective hygiene education and promotion of a healthy lifestyle. Therefore, the nature and intensity of the medical activity of an individual is influenced by the level of education and culture, and knowledge of health-related issues. The purpose of the study is to consider the main problems of medical activity, preservation and promotion of health in the context of the COVID-19 pandemic. Material and methods. The information base of the study was publications in periodicals and the Internet, statistical data of the Rosstat, WHO, “Yandex. Coronavirus. Statistics”, polls by the state-run VTsIOM (All-Russia Opinion Research Center). The study used general logical and analytical methods, methods of statistical analysis and empirical research. Results. Medical activity and lifestyle play a leading role in controlling the spread of COVID-19. Non-compliance with anti-epidemic measures and irresponsible hygienic behavior (ignoring the mask regimen, personal hygiene rules, social distancing, “anti-vaccination” attitude), irresponsible medical behavior (undertreatment and ignoring medical rehabilitation), unhealthy diet (unbalanced diet, insufficient intake of vitamins and microelements), inadequate physical activity (its insufficiency) and addictive behaviors (smoking, drinking alcoholic beverages) contribute to both rapid spread of the infection in the social environment, and prolongation of the complicated epidemiological situation over time, minimizing the entire complex mechanism of the anti-COVID actions of the health and administrative services, increasing the risk of severe course of the disease, post-COVID complications and deaths. Reasons of the reduced medical activity of the Russians include lack of confidence in a medical specialist, medical organizations, and the health care system in general; dsregard of one's health; low sanitary culture of the population due to insufficient education in health preservation; reduction in the number of medical institutions and personnel of the state medical institutions, etc. Among the objective reasons, the first and most acute problem is the economic one: low standard of living of the significant part of the population, which dramatically limits the ability to maintain their health. Tobacco smoking is one of the most important problems of public health. It is quite obvious that the prevention of tobacco consumption needs a comprehensive and systematic implementation of a wide range of multi-level and intersectoral measures, the essence of which is to manage tobacco smoking as one of the risk factors of chronic non-communicable diseases at the population and individual levels. The implementors of those preventive measures should include both healthcare and state and public organizations, employers, and mass media. The purpose of the study is to analyze the population strategy to quit tobacco in the Ryazan region and its impact on smoking cessation. Material and methods. The study was performed using a survey, statistical and analytical methods based on the evaluation of preventive antismoking measures implemented in the Ryazan region in the period from 2014 to 2019 within the framework of the annual interdepartmental medical and educational campaign “Summer without Tobacco Smoke”. The study information base is the results of an anonymous questionnaire survey of 1,604 registered (according to the terms of the campaign) participants of the anti-tobacco campaign aged 18-64 years. Analysis of the dynamics in performance indicators of medical offices regarding smoking secession was based of their reports during the 6-year period under study. Comparisons of tobacco smoking prevalence in these years were based on results of the adult population surveys on tobacco consumption in the region (Global Adult Tobacco Survey (GATS) conducted in the Russian Federation, as well as data of the Federal State Statistics Service. Results The annual anti-tobacco campaign “Summer without Tobacco Smoke” complies with the basic principles of highly effective public programs: duration (carried out for more than 5 years), focus on a specific risk factor (smoking) and regularity of such campaigns. The antismoking campaign forms flows of people who want to get rid of smoking on their own or through medical assistance in medical offices. Systematic nature of antismoking campaigns as a form of population-based tobacco smoking prevention, contributed to the formation of positive trends in reducing prevalence of tobacco consumption. Thus, in the period from 2014 to 2019, there was a 44.6% gradual decrease in the prevalence of tobacco smoking in the Ryazan region (by 12.2% among males and 37.6% among females). Conclusion. The obtained results evidence with confidence that the anti-tobacco campaign both forms groups of people who are aware of smoking-related hazards and of methods of smoking secession, and increases motivation for a healthy lifestyle, as well as positively affects utilization of medical offices for smoking secession. The pandemic of the new coronavirus infection Sars-CoV-2, (hereinafter referred to as NCI) affects all spheres of human life, and therefore it is relevant to study its impact on quality of life of military personnel. The purpose of the study: to give a comprehensive assessment of life quality of military personnel during the COVID-19 pandemic. Material and methods. In February-March 2022, analytical, cross-sectional study was conducted using individual data. A survey was conducted in a group of 374 military personnel serving under contract in Severodvinsk. The research tool was a short version of the WHO Quality of Life questionnaire (WHOQOL-BREF) with an additional block of questions associated with the COVID-19. To describe data, the median and interquartile range (25th and 75th percentiles) were calculated. To compare indicators between several groups of military personnel, analysis of variance using the Kraskel-Wallis criterion was used. The nonparametric Mann-Whitney criterion and Spearman correlation criterion were used to compare the variables. Assessment of impact of quality of life on social well-being was carried out by constructing a linear regression model with the inclusion as independent parameters of answers to the passport (personal details) part of the questionnaire and answers to questions related to previous diseases, by a step-by-step method of selecting criteria, using F-statistics as a selection method with an inclusion value of less than 0.05 and an exclusion value of more than 0.1. Differences were taken as reliable with a probability of error of the first type of less than 5% (p<0.05). Statistical data processing was performed using the IBM SPSS Statistics ver. 25 for Windows application software package. Results. The highest score was given to the sphere characterizing physical and mental well-being (82.8%), while the lowest score was given to the sphere characterizing social well-being (75.0%), the median of the overall assessment of quality of life equaled to 103.00, the interquartile range [96.00; 108.00], the share in the highest assessment equaled to 79.2%. A posteriori analysis showed statistically significant differences in the field of self-perception between groups of servicemen who are officially married and divorced (U=2475.5, p=0.004) and in the field of microsocial support (U=2408.5, p=0.001), and in the field of social well-being between groups of servicemen who indicated in the questionnaire 12 to 20 thousand rubles as income per family member per month and those with income of more than 30 thousand rubles (U=3070.5, p=0.003). Therefore, analysis of health status of population of the older retirement age and incidence rate of their outpatient visits is relevant and beyond any doubt. Purpose of the study: To analyze dynamics in health status of urban population of the older retirement age and incidence rate of their outpatient visits in 2011-2020 based on the data analysis on outpatient visits in 2011-2020. Material and methods. Materials of state statistics and summary form of the state statistical observation No. 12 for the city of Krasnoyarsk in 2011-2020 were used as sources of information. The analysis used absolute and relative values, ratio coefficients, dynamic series indicators and their growth rates (loss). Results. The analysis showed increased share of people of the older retirement age in the total adult population, resulting in their increased share in the structure of medical care seeking by adults. There is a decrease in the crude disease incidence by 21.1% and previously diagnosed diseases by 30.2% against the background of the decreased rate of disease chronicity from 4.4 to 2.6. The incidence rate of outpatient visits shows that the share of people of the older retirement age uptakes 1.5 times more outpatient services compared to people of working age. Rheumatic diseases belong to the group of the most socially significant diseases and have a negative impact not only on the patient and his family, but also on the State as a whole. According to the 10th Revision of the International Classification of Diseases, rheumatic diseases are mainly classified in Chapter XIII - \"Diseases of the musculoskeletal system and connective tissue\". Epidemiological studies are needed to assess the scale of the problem. The purpose of the study was to evaluate dynamics in crude incidence of diseases of the musculoskeletal system and connective tissue (in general and by individual nosological forms/groups) in the adult population of the Republic of Bashkortostan (Russian Federation) in 2015–2019. Material and Methods. A retrospective observational study was carried out. The object of the study included residents of the Republic of Bashkortostan aged over 18, and the subject was crude incidence of diseases of the musculoskeletal system and connective tissue (in general and by individual nosological forms/groups: osteoarthritis, rheumatoid arthritis, reactive arthropathies, dorsopathies (including spondylopathies and separately ankylosing spondylitis), osteopathies (including osteoporosis), systemic connective tissue disorders) in the adult population of the Republic of Bashkortostan in 2015–2019. Data on crude incidence were obtained from the statistical data of the Medical Information and Analytical Center of the Ministry of Health of the Republic of Bashkortostan, as well as annual statistical materials of the Ministry of Health of the Russian Federation. Results. The crude incidence of diseases of the musculoskeletal system and connective tissue (in general) in the adult population of the Republic of Bashkortostan increased over the study period 2015-2019. Analysis by individual nosological forms/groups shows that crude incidence of osteoarthritis, rheumatoid arthritis, reactive arthropathies, dorsopathies, including spondylopathies (including ankylosing spondylitis) in the Republic of Bashkortostan has also increased by 2019, while crude incidence of systemic connective tissue disorders and osteopathies, including osteoporosis has decreased. Furthermore, a number of differences have been revealed in the dynamics in the indicators under study in the Republic of Bashkortostan and in the Russian Federation as a whole for the period 2015–2019. Conclusion.
