One of the main tasks of medical examination of certain groups of the adult population is to identify chronic non-communicable diseases, including cancer, at early stages. The article presents data on the number of adults undergone the first stage of medical examination of certain groups of the adult population, information on newly detected neoplasms, including malignant ones, registered during medical examination in the Russian Federation, the Northwestern Federal District and its constituent entities in 2016-2019. Contribution of the adult medical examination to the detection of malignant neoplasms and neoplasms in situ in the subjects under study has been determined. Purpose: to analyze dynamics in the coverage of certain groups of the adult population with medical examination and the number of newly detected neoplasms, malignant neoplasms and in situ during medical examination, including at stage I and II in the subjects of the Northwestern Federal District. To evaluate contribution of medical examination of the adult population to the detection of malignant neoplasms and neoplasms in situ in the subjects under study. Material and methods. The analysis used data from federal and industry statistical reporting forms: «Information on malignant neoplasms» № 7 and «Information on the medical examination of certain groups of the adult population» № 131/o. For the content analysis of literary sources and normative legal acts, the method of dynamic series analysis and the Kendall rank correlation method were used. Results. In 2016-2019, the average annual increase in the number of adults undergone the first stage of medical examination in the Northwestern Federal District was significantly higher compared to the Russian Federation (15.8 and 4.8%, respectively). Among the top ten different classes of diseases detected during medical examination of certain groups of the adult population, neoplasms rank 10th. The average annual increase in the number of neoplasms, malignant neoplasms and in situ, as well as stages I-II detected during medical examination in the Northwestern Federal District is higher than in Russia. Implementation of the plan for the medical examination coverage is not a significant factor in primary detection of neoplasms. The contribution of medical examination to early detection of malignant neoplasms and neoplasms in situ in Russia and the Northwestern Federal District in general increased and equaled to 5.8 and 6.4% in 2019, respectively. Conclusions. However, the growth in the number of labor migrants in recent years in Russia increases the risk of the spread of tuberculosis in Russia, imported by migrants from countries with high TB prevalence. Further development of such situation may jeopardize achievement of the TB Control Program targets in the Russian regions with the highest inflow of labor migrants. Purpose: to analyze incidence of tuberculosis among labor migrants in the Kaluga region in dynamics for the period from 2010 to 2019. Material and methods: information from the Rosstat statistical surveillance forms No. 8 and No. 33, Rosstat data on the average annual population of the Kaluga region for 2001-2020, data from the Department of the Ministry of Internal Affairs in the Kaluga region on the number of labor migrants in 2017-2019. Pearson’s chi-square test (χ2) was used for comparative analysis. Results: The decline rate in the overall TB incidence in the Kaluga region over the past decade (2010-2019) equaled to 47%, which is 16.4% lower compared to the rate in the resident population of 56.2%. A slowdown in the decline in the overall TB incidence, which is the key indicator of the TB control program, may compromise achievement of the Program targets in the Kaluga region. A comparative analysis of TB incidence among resident population and labor migrants showed higher values among labor migrants compared to resident population: 7.2-fold in 2017, 4.9-fold in 2018 and 7.4-fold in 2019 (p<0.0000001). Conclusion. A significantly higher TB incidence among labor migrants poses a serious threat to its spread in resident population of the region. Over the last two decades, Russia has been regestering the increased prevalence of high blood pressure among adults. The relationship between blood pressure and mortality from diseases of the circulatory system has been proved. Reducing morbidity and mortality from diseases of the circulatory system calles for improved efficiency of the state health system to detect and control blood pressure at the population level. Purpose. To improve organization of measures aimed at detecting high blood pressure and enhancing control of arterial hypertension in order to reduce adult mortality from diseases of the circulatory system in Russia. Material and methods. The authors have conducted a content analysis of regulatory and legal documents regulating activities of individual structures of the public health system on medical examination and follow-up of patients with chronic non-communicable diseases, registration and reporting forms of the federal statistical surveillance (FSN No. 12) and industry statistical surveillance (No.131/o), including information on the number of adults with arterial hypertension residing within the catchment area of the healthcare facility in dynamics from 2010 to 2019, as well as information on the number of newly diagnosed cases of arterial hypertension during medical examination of certain groups of the Russian adult population in 2015-2019; and the number of patients taken under medical follow-up with the detected (including newly diagnosed) arterial hypertension. Results. During the period from 2010 to 2019, the number of registered patients with arterial hypertension in Russia increased by 5.4 million (46%), from 11.7 in 2010 to 17.1 million in 2019. The rate of growth in the detection of new cases of arterial hypertension actively increased with initiation of the state program on medical examination of certain groups of the adult population (state checkup for non-communicable diseases). Coverage with follow-up of patients with arterial hypertension increased from 905.7 thous. in 2015 (69.5%) to 1321.4 thous. (77.0%) in 2019, including among newly detected cases during medical examination from 74.9 to 83.4%. Decrease in mortality from arterial hypertension, diseases of the circulatory system and total mortality over the period under study (2010-2019) in Russia is primarily associated with the improved detection of arterial hypertension, increased effectiveness of drug treatment and control of arterial hypertension. Conclusion. Introduction of non-core subjects into the curriculum of pedagogical universities, in particular, “Fundamentals of Medical Knowledge” is fraught with greatest difficulties. On the one hand, this is a subject that does not develop any professional competences of students, it is of an introductory nature only. On the other hand, acquiring practical skills in first aid, students should know a clear algorithm of medical actions and remember it until the end of their studies at the university. The purpose of the study. To analyze the level of competence development in first aid provision. Маtеrial and methods. A survey of two groups of students was conducted: the 1st group – the first-year students who have completed “Fundamentals of Medical Knowledge” at the end of the academic year (2021), a total of 98 students; the 2nd group - graduate students (2021), who studied this subject also in the first year, i.e. 4 years ago, a total of 103 students. The method of random selection was used. All students answered 29 questions anonymously. The sampling error on the questionnaire questions did not exceed 5%. To analyze data of the sociological survey, an alternative analysis (extensive indicators) was used, and to identify cause-and-effect relationships a correlation analysis was undertaken. Reliability of the correlation coefficient was assessed using tabular standard values. Results. The majority of the respondents in both the 1st group (94.9%) and the 2nd group (96.1%) consider the non-core subject “Fundamentals of Medical Knowledge” as the need to use the acquired knowledge in future. According to the competence-based approach, the subject is aimed at developing knowledge and skills in managing conditions that can become fatal within a short period of time. Upon completion of the subject in the 1st year, signs of clinical death could be determined by 42.9%, while by the end of the studies at the university - only 8.7%. A similar situation is relevant to first aid for bleeding - 61.2% and 29.1%, respectively. The main reasons why students fail to master the threshold level of competence in first aid are both a feeling of fear, lack of experience, ethical side (vomit, blood), insufficient technical equipment during practicals, as well as the fact that this subject is taught by educators without medical background, which is confirmed by the results and correlation analysis. In addition, the respondents were faced with the approaches that do not correspond to the correct algorithm of actions: 8.2% - in the 1st group and 15.5% - in the 2nd. The level of competence development among students is decreasing, by the end of the university only 8.7% can implement “I know, can do, and master the skill”. Conclusion. The competence-based approach to non-core subjects does not produce the expected result. The regulatory and staffing problems in urological care delivery to adults and children in outpatient settings, combined with the high social significance of diseases of the genitourinary system for the population of different age, potentially threaten successful implementation of the priorities of the state demographic policy. Purpose: to identify problems in regulation and staffing in urological care delivery to adults and children on the outpatient basis in the Russian Federation in 2015-2019. Material and methods. The study used data of the Fedral statistical observation Form No. 30 \"Information about a medical organization\" for the period 2015-2019. Statistical processing of the study materials was carried out by methods of descriptive statistics. Results. The analysis has identified contradictions in the orders of the Ministry of Health of the Russian Federation, which currently regulate staffing in urological care. The number of full-time and occupied positions of urologists in 2015-2019 decreased, which, coupled with the increased number of individuals, resulted in the increased number of urologists (+3.1%), increased level of staffing of individuals up to 62.9%. At the same time, the part-time coefficient among urologists remained high adding up to to 1.3. In 2015-2019, the number of full-time and occupied positions and individual pediatric urologists-andrologists increased, against the background of extremely low staffing level of individuals equalling to 26.7% in 2019 without any significant dynamics over a 5-year period. The function of the urologist position remained unchanged (4072 appointments per position in 20194 and 4181.3 in 2015), while the function of the pediatric urologist-andrologist position increased from 4215 appointments per position in 2015 to 5342 in 2019 (+ 26,7%). The integral staffing coefficient had a negative trend especially in relation to pediatric urologists-andrologists. Conclusions. The growth in the number of individual urologists and pediatric urologists-andrologists in 2015-2019 did not result in any significant changes in the staffing levels in the urological care delivery to adults and children on the outpatient basis. It is necessary to update the staffing standards, objectify statistical reporting and take urgent measures to bridge the shortage of doctors in outpatient settings. Scope of application. A high level of coverage with preventive examinations of labor migrants arriving to work at enterprises in the Kaluga region makes it possible to detect tuberculosis (TB) at an early stage, which is characterized by an asymptomatic course, and with timely treatment it it possible to achieve its effective cure and thereby prevent the TB spread both among migrants and permanent population of the region. However, the majority of TB patients among labor migrant avoid treatment or interrupt the therapy due to lack of funds to pay for treatment. Undertreated patients continue to be sources of infection, often spreading resistant forms of tuberculosis with multidrug resistance of the pathogen. In addition, it is not possible to identify whereabouts of such patients, which fraught with TB spreading among the general population of the region. The purpose of the study: to analyze demographic and clinical factors influencing TB treatment outcomes in labor migrants. Material and methods. The study used information about 357 patients with tuberculosis, newly detected among the resident population of the Kaluga region, and 118 TB patients detected among labor migrants in 2019-2020. For the analysis, a questionnaire was developed, consisting of 9 demographic and clinical factors influencing outcomes of TB treatment. The analysis used information from the federal statistical surveillance forms No. 8 and No. 33 and Rosstat data on the average annual population of the Kaluga region in 2011-2020. To analyze TB treatment outcomes in labor migrants, data of the cohort analysis for 2016-2020 were used. Statistical analysis methods were used for the analysis. Results. In the Kaluga region in 2016-2020, 406 patients with tuberculosis were detected among 142,763 labor migrants examined by fluorography, or 0.3% of the total number. Analysis of a number of factors that affect TB treatment outcomes in labor migrants and the resident population of the Region in 2019-2020 showed that 90.7% of TB cases among labor migrants were detected at an early stage, which is asymptomatic, therefore in epidemic terms, they were much less dangerous compared to those detected among the resident population. However, only 123 patients were signed up for treatment, which accounted for 30.3% of the total number of TB patients among labor migrants, while the remaining 283 patients could not afford to pay for TB treatment and dropped off the TB service radar. Among patients with tuberculosis who started treatment, only 50% were effectively cured, while every third had to interrupt the treatment due to lack of funds to pay for the full course of chemotherapy. A significant part of the population has suffered a new coronavirus infection of varying severity and they need attention and follow-up at the outpatient level. Assessment and monitoring over functional status of the COVID-19 survivors is one of the important objectives of organizing medical rehabilitation in primary health and developing preventive measures. The purpose of the study is to analyze changes in functional status and assess rehabilitation potential of the COVID-19 survivors. Material and methods used include questionnaire survey, analytical and statistical analysis. Results. Based on the questionnaire, assessment of functional status of the COVID-19 survivors was carried out using a special scale, presence and degree of functional limitations were determined. According to the survey, 65.5% of the COVID-19 survivors did not feel any limitations in everyday life. Minor functional limitations were reported by 26.5% of the respondents, while an equal number of the respondents, 4% each, noted mild and moderate limitations of body functions. The COVID-19 survivors indicated the following symptoms: easy fatiguability - 33.3%, fatigue - 26%, shortness of breath on exertion (unusual) - 15%, “smell disorder” - 13%, and headache and joint pain - 12.4%; however, 36.7% of the respondents did not report any symptoms. After the disease, 50.8% of the respondents returned to their usual state, while 35% of the respondents chose the answer “rather yes than no”. 14.2% did not return to their usual state, choosing the answer “rather not” (8.5%) and “no” (5.7%). Rehabilitation measures in the first three months after the disease were carried out by 49.2% of the respondents (327 patients). Conclusion. Functional status of the COVID-19 survivors is largely determined by the initial state of somatic health, peculiar features of the biological and psychophysiological characteristics of a person, and a complex of external factors that allow the person to realize potential abilities to compensate for the existing functional limitations. Compliance with rehabilitation measures requires active participation of medical specialists in terms of quality control and correctness of their implementation. Conditions for integrating medical surveillance service for people engaged in physical activities and sports into the regional public health system have been already developed in a number of subjects of the Russian Federation. In some regions, medical and physical education departments and/or sports medicine centers are the main institutions for medical prevention. In the reporting documentation, information about activities of these institutions related to preventive work with the population is not detailed. The methodology, developed by the Federal Research Institute for Health Organization and Informatics of the Ministry of Health of the Russian Federation for generating information on activities of these institutions, makes it possible to supplement and specify information regarding measures aimed at maintaining and restoring public health. The purpose of the study was to analyze activities of the regional medical and physical education departments related to health promotion, prevention and interdepartmental interaction, in terms of its integration into the regional public health system. Material and methods. Regulatory documentation, the form №53 of the sectoral statistical observation, data of the explanatory notes to the form №53. Method: content analysis. Results. A comparative analysis of regulatory documents has revealed compliance, indicating the feasibility of integrating activities of sports medicine divisions into the regional public health system. The main work directions in the field of prevention, public health and interdepartmental interaction have been identified and analyzed. Conclusions. Sports medicine divisions are part of the prevention system of the region. To ensure integration into the regional public health system, it is necessary to establish a clear interaction between structures both within the preventive service and in the regional system of ministries and departments. The inconsistency of the reporting documentation and the lack of standard indicators make it difficult to assess effectiveness of the public health measures. The scope of application.
