Kasikhina","doi":"10.36691/rja16906","DOIUrl":"https://doi.org/10.36691/rja16906","url":null,"abstract":"BACKGROUND: \u0000Cutaneous mastocytosis is a rare disease that primarily develops in childhood. \u0000AIMS: \u0000Thisstudyaimed to determine theclinical and epidemiological characteristicsand frequency of detection of cutaneous mastocytosis among children in Moscow. \u0000MATERIALS AND METHODS: \u0000A single-center epidemiological retrospective study was conducted. Data from Form №12 of the federal statistical observation “Information on the number of diseases registered in patients living in the service area of a medical organization” were used as a statistical source of information. To calculate the incidence rate in intensive indicators per 100 thousand population, data on the population of Moscow for the period 2019-2022 were used. \u0000RESULTS: \u0000Over the period 2019-2022, a gradual trend towards an increase in the number of newly identified cases of CM in children and adolescents in Moscow was revealed. Number of initial applications from children and adolescents (0-17 years old) for this period increased by 4.8 times. The share of CM in the structure of skin diseases and congenital anomalies (malformations) of the skin increases every year. In 2019, for every 1,834 pediatric visits (age category 0-17 years) regarding skin diseases, there was 1 case of mastocytosis; in 2020 the ratio was 1,863:1; in 2021 – 491:1; in 2022 – 376:1. Fassakhov","doi":"10.36691/rja16463","DOIUrl":"https://doi.org/10.36691/rja16463","url":null,"abstract":"AIMS: is a selective study of the quantity and avidity of IgG antibodies to SARS-CoV-2 over time among medical workers (HCWs) of a temporary infectious diseases hospital in Kazan - convalescents of COVID-19, during the period from July 2020 to July 2021. \u0000METHODS: Determination of IgG to the S antigen of SARS-CoV-2 by ELISA was carried out using the test system SARS-CoV-2-IgG quantitative-ELISA-BEST (JSC Vector-Best, Russia) and expressed in BAUml. Antibody avidity was determined using a 4.0 M urea solution and expressed as avidity indices (AI) 1, 4 and 7 months after COVID-19. \u0000RESULTS: IgG avidity to SARS-CoV-2 depended on the severity of COVID-19. The highest rates of IA were found in the group of those who had a moderate form of COVID-19. If in mild and asymptomatic forms there was a parallel decrease in AI and IgG titer, then in moderate forms an increase in antibody titer was accompanied by a decrease in their avidity 4 months after the infection. 7 months after seroconversion, the IgG level decreased almost twofold, both in mild, asymptomatic and moderate forms. In the group of HCWs who had COVID-19 repeatedly, the initially low levels of AI and antibody titers increased in parallel, while AI after 7 months did not decrease, but remained high. Differences in AI determined the subsequent formation of different trends in the development of the humoral immune response, which were mainly characterized by an uneven decrease in IgG and persistence of Ig M antibodies for more than 1 month. \u0000CONCLUSIONS: The research results expand the understanding of the mechanisms of formation of the humoral immune response and the avidity of IgG antibodies against SARS-CoV-2 in the risk group - MR. The level of humoral immunity decreases in the first six months and varies depending on the severity of COVID-19. Cross-reactive food allergy (CRFA) is an underestimated problem that complicates the course of pollinosis and requires further study. AIMS: To study the features of molecular sensitization profile in adult patients with birch pollen allergy and cross-reactive food allergy to PR-10 proteins. MATERIALS AND METHODS: A total of 25 adult patients with birch pollen allergy, with or without CRFA, were included in an open single-center interventional prospective controlled non-randomized study. Specific IgE to 112 allergens was determined using multiplex analysis ImmunoCAP ISAC. Patients with CRFA filled out a validated questionnaire on CRFA symptoms and causative allergens. The primary endpoint of the study was the assessment of sensitization profile to Bet v 1 and PR-10 proteins (Mal d 1, Ara h 8, Cor a 1.0101, Cor a 1.0401, Act d 8, Api g 1, Pru p 1, Gly m 4), as well as clinical characteristics of sensitized patients. The secondary endpoint was the comparison of patients with and without CRFA and the evaluation of how sensitization to Bet v 1 and PR-10 homologs can affect clinical manifestations of CRFA in patients. RESULTS: The study included 25 patients aged 18 to 55 years, from which two comparable groups were formed: Group 1 - 13 patients with CRFA symptoms, Group 2 (without CRFA) - 12 patients. When analyzing the sensitization profile to PR-10 allergens, we found similar patterns in both groups, but sIgE concentrations to all allergens were significantly higher in Group 1. We found a strong correlation between the number of triggering products and the total IgE level to PR-10 proteins (r=0.81; p=0.001). The severity of CRFA symptoms correlated with the total level of IgE to PR-10 proteins (r=0.97; p0.0001). The cut-off level for total sIgE to food PR-10 proteins, equal to 70 ISU-E, allowed to distinguish patients with only oral allergic syndrome from patients with systemic manifestations of CRFA. {"title":"THE SPECTRUM OF SENSITIZATION TO AEROALLERGENS IN ALLERGIC PATIENTS FROM UPPERCHIRCHIK DISTRICT OF THE REPUBLIC OF UZBEKISTAN","authors":"N. Dustbabaeva","doi":"10.36691/rja16464","DOIUrl":"https://doi.org/10.36691/rja16464","url":null,"abstract":"The purpose of the study: to study the spectrum of sensitization in people with allergic diseases in the Upperchirchik district of the Tashkent region, which is characterized by a long period of flowering of weeds (wormwood, saltwort, quinoa) - the end of August - the end of November, cereals (timothy, foxtail, rye, bluegrass) herbs - the end of February - the end of July; the duration of flowering is associated with the peculiarities of climatic and geographic conditions. Materials and methods: from January to May 2022, a clinical and allergological examination was carried out in 100 patients with allergic diseases: of these, 38 children aged 2-18 years: 20 children with AR, 12 with BA, 6 with AtD and 62 adults: 32 patients with AR, 13 with BA, 17 with AtD. \u0000Results: Among all examined patients the most frequent were intermittent and persistent forms of AR - in 52 (52 %), BA - in 25 (25 %), AtD - in 23 (23 %), combination of AR+BA - in 13 (13 %), AR+ATD - in 12 (12 %), AtD+BA - in 9 (9 %) patients.Combined sensitisation to pollen, household and food allergens was detected in 90% of patients, monosensitisation to allergens of cereal grasses (meadow timothy, meadow bluegrass) in 6%, and to allergens of weeds (wormwood, quinoa) in 4%.Sensitization to house dust allergen (h1) was detected in 29 children and 51 adult patients. Mistakes in biologicals choice lead to discontinuation and/or switching of the drug due to insufficient effectiveness of therapy. \u0000 \u0000AIMS: to determine reasons to stop targeted therapy and biologicals switching effectiveness in patients with SA in real clinical practice. \u0000 \u0000MATERIALS AND METHODS: analyzed patients with SA (n=116) from Sverdlovsk region register were divided into 3 groups (#1-continuous, #2-stoppers and #3-switched). Predictors of biologicals withdrawal and switching, reasons for the 1st biological stopping, switching schemes, therapy effectiveness after switching (according to ACT, FEV1, AQLQ, SNOT-22, the need for SGCS, achievement of SA remission) were determined. \u0000 \u0000RESULTS: There were 17.2% stopping and 12.1% switching patients out of 116 patients in the registry. Stoppers suffered from CRSwNP less often and had earlier asthma onsets. Switched patients had higher blood eosinophils level. Therapy was cancelled for personal reasons in 45% of patients. Therapys ineffectiveness in SA and/or in CRSwNP was the main reason for switching (92.8%) mostly from omalizumab and benralizumab. The switch drug of choice was dupilumab. Improved indicators: ACT (by 86.4%), FEV1 (by 21.2%), AQLQ (by 52.5%), SNOT-22 (by 48%), the need for SGCS decreased to 0 in 12 months after switching. Remission was achieved in 62.5% of patients (excluding FEV1) and 50% of patients (including FEV1). \u0000 \u0000CONCLUSIONS: Careful selection of targeted therapy patients allows to minimize the failures of the starting drug to 12.1%. The decrease of the effectiveness of antibacterial drugs aggravates the therapy of AD and actualizes the development of new antimicrobial agents. \u0000AIMS: synthesis and evaluation of the antibacterial activity of cationic peptides and an aqueous solution of fullerene C60 to create drugs based on them that will have a spectrum of biological activity, including anti-inflammatory, antiallergic and antibacterial. \u0000MATERIALS AND METHODS: the objects of the research were the developed linear and dendrimeric cationic peptides, whose structure was confirmed by mass spectrometry (MALDI-TOF). An aqueous solution of fullerene C60 was obtained using a unique developed and patented technology. Analysis of the antibacterial activity was carried out by the method of diffusion into agar using disks (screening), the method of serial dilution was used to determine the minimum bactericidal concentration of the studied compounds. \u0000RESULTS: as a result, 42 cationic peptides with various structures were developed and synthesized. The molecular weight of the peptides did not exceed 5000 Da. They contained from 7 to 25 amino acids with charges from +5 to +16. The screening revealed 15 peptides that showed activity against the strain of E. coli Dh5a. Thus, it was shown that the peptides AB14, AB17 and AB18 showed bactericidal activity relative to the bacterial strain E. coli Dh5a in concentrations of 0.03, 0.15 and 0.74mM, respectively, which exceeded that of ampicillin (0.74mM) several times. The analysis of an aqueous solution of fullerene C60 did not reveal its antibacterial activity. \u0000CONCLUSIONS: a number of cationic peptides with hydrophobic and positively charged sites in their composition was obtained. The structures of the most active peptides (AB-14 and AB-17) were close to -helical. The antibacterial activity of these peptide constructs was several times higher than that of ampicillin, which acted as a positive control drug. In recent decades, the incidence of bronchial asthma has been steadily increasing. In 2021, the prevalence of asthma among Russian children aged 014 and 1517 years was 0.89% and 1.87%, respectively, but it has been known for a long time that the prevalence of asthma according to official statistics is lower than the real data. Therefore, a significant proportion of patients do not receive proper treatment, which ultimately affects the quality of life of patients and their parents. Early and correct diagnosis of bronchial asthma and identification of children with an uncontrolled course of the disease will provide an opportunity to select/change therapy to achieve remission, preventing possible negative consequences of the disease. In Moscow, according to Order No 947n of the Ministry of Health of Russia, in order to improve the quality of medical services, including outpatient care, the Unified Medical Information and Analytical System (UMIAS) has been implemented, which has improved the availability and quality of medical services of public health care institutions in the city. However, data on correlations between the severity of atopic dermatitis, combined with and without food allergies (SCORAD index, etc.) with the level of eosinophils and interleukin-5, are scarce and contradictory. There are no results on the comparative frequency of eczematous reactions occurring at different time, or immediate allergic reactions in the above groups of patients. There are no or contradictory studies of the frequency of the combination of atopic dermatitis and food allergies, its various degrees of severity depending on these indicators, levels and relationships of the concentration of eosinophils, interleukin-5, including under conditions of psychosocial stress. \u0000AIMS: To study in the Donbass region, under conditions of psychosocial stress, the frequency and severity of the course, skin reactions in patients with atopic dermatitis, combined with and without food allergies, as well as the levels and correlations of interleukin-5 and eosinophils, depending on the above condition. \u0000MATERIALS AND METHODS: 165 patients with atopic dermatitis were examined. Questionnaires, anamnesis collection, general clinical and specific allergological examinations were carried out, the levels of interleukin-5, the absolute quantity and percentage of eosinophils in the peripheral blood were determined. There were 2 subgroups of patients with atopic dermatitis: 1) combined with food allergies; 2) had no combination with it. Persons living in areas with high and low intensity of shelling are also identified. Statistical data processing was carried out using the domestic program Medstat (Donetsk). \u0000RESULTS: It was found that in patients with atopic dermatitis, randomly selected from those who applied for an appointment with allergists-immunologists, in 81.2% of cases this disease is combined with food allergy. The severity of atopic dermatitis and eosinophil levels are significantly higher when it is combined with food allergies. Stable direct reliable correlations of varying strength have been established between the severity of atopic dermatitis (SCORAD index and its individual indicators) and the absolute quantity, percentage of eosinophils, interleukin-5 level, as well as between the above laboratory parameters. Differences were found in the frequency of recording eczematous, immediate skin reactions. {"title":"Severe anaphylaxis in children: definitions, etiology, approaches to prevention","authors":"N. Esakova, A. The course of each episode of anaphylaxis is the result of a complex interaction of the patient with a number of internal and external factors (concomitant diseases, profile and level of sensitization, features of allergen exposure, intercurrent infections, etc.). Currently, the clinical and biological features of severe anaphylaxis are not well characterized, however, arterial hypotension and/or loss of consciousness are usually observed in these patients, possibly without typical symptoms from the skin, while the outcome of the disease is associated with the possibility of timely emergency medical care. The lack of generally accepted reliable individual prognostic predictors for determining this group of patients is a serious problem. There is very little data on subfatal/fatal anaphylaxis in pediatrics and approaches to it are often based on clinical experience in other age groups. As a rule, an anaphylactic reaction develops to food, insect stings, and medicine drugs. Sometimes cofactors can modulate of anaphylaxis and significantly reduce the required dose of an allergen for the development of an anaphylactic reaction. In such cases anaphylaxis is considered as cofactor-dependent anaphylaxis. Alcohol consumption is one of the leading factors of cofactor-dependent anaphylaxis.
