The current European Сlinical Guideline (CG) have been initiated by the European Academy of Allergy and Clinical Immunology (EAACI), the Dermatology Section of the European Academy of Allergy and Clinical Immunology (EAACI), the Global Allergy and Asthma European Network (GALEN), Urticaria and Angioedema Centers of Reference and Excellence (UCAREs and ACAREs), the European Dermatology Forum (EDF; EuroGuiDerm), Asia Pacific Association of Allergy, Asthma and Clinical immunology (Asia Pacific Association of Allergy, Asthma and Clinical Immunology (APAAACI)). This guideline has been adopted by the European Union of Medical Professionals (UEMS). The purpose of the manual is to define and categorize urticaria (with the focus on chronic urticaria), causes, triggers, comorbidities, and burden on patients and society. Growing burden of bronchial asthma necessitates a search for the most effective methods of diagnostics and treatment of different asthma phenotypes, including relatively rare. \u0000From this point of view, glycomics appears to be one of the most interesting and perspective branch of medicine. This science section researches various carbohydrates complexes and their roles in the development of different diseases. \u0000In the context of bronchial asthma receptors for advanced glycation-end products (RAGE) and their soluble variants (sRAGE) are of interest to researches. Besides, sialic acid-binding immunoglobulin-type lectins (Siglecs) can play a key role in the principal new tactics of the treatment. Affecting on Siglecs can lead to decrease of proinflammatory activity of immunocompetent cells and bronchial walls protection. Finally, N- and O-glycans remain almost unresearched. For the first time, a clinical case of a combined immediate and delayed allergic reaction to eating lemon, which manifests itself in the form of oral allergic syndrome and gastrointestinal symptoms, in a 31-year-old woman was described. The patient was diagnosed with polysensitization to various food, household and pollen antigens, cross-allergy to fruits of the Rutaceae family (orange, tangerine, grapefruit). With the help of a prick+prick test and an enzyme-linked immunosorbent assay, she was diagnosed with allergy and sensitization to the allergens of lemon pulp. The presence of one or more thermolabile antigens in lemon pulp, causing immediate and delayed allergy symptoms, has been established. The ability of thermolabile allergens and/or individual antigenic determinants to induce delayed allergic reactions, despite the action of proteolytic enzymes of the gastrointestinal tract and hydrochloric acid, has been established. The urgency of the problem is associated with an increase in the number of patients and the fact that allergic rhinitis is considered to be a risk factor for the development of asthma. \u0000The clinical allergic rhinitis guideline aimed to optimize patient care, up-to-date information about the epidemiology, and disease etiology and pathogenesis. Herein, presented the actual data about allergic rhinitis classification, its clinical signs, modern diagnostics (clinical, laboratory, and instrumental), and differential diagnostics of allergic rhinitis. Studies reported allergic rhinitis treatment, rehabilitation, and prevention in the guideline. The authors describe in detail the existing healthcare options for patients with allergic rhinitis, diagnostics features and care in partial groups of population (children, pregnant women). Polymorphisms in the genes of cytokines involved in the regulation of the direction of the T-helper mediated immune response are risk factors for the development of the disease and the realization of various phenotypes of bronchial asthma. \u0000AIMS: Study of the structure and frequency of occurrence of single nucleotide polymorphisms of cytokine genes with an assessment of the risk of various phenotypes of bronchial asthma in children. \u0000MATERIALS AND METHODS: In this case control study, 250 children were examined, including 150 children with a verified diagnosis of bronchial asthma (including 75 children with virus-induced and 75 children with allergen-induced disease phenotypes) and 100 sexually comparable healthy peers. The children underwent a comprehensive general clinical and allergological examination, genotyping, analysis of the structure, frequency of occurrence of cytokine gene polymorphisms and calculation of the odds ratio of the risk of developing different bronchial asthma phenotypes. DNA samples isolated from peripheral venous blood were used as material for molecular genetic analysis. The following mutation points were selected: IFN (T-874 A), IL-4 (C-589 T), IL-6 (C-174 G), IL-17A (G-197 A), TNF (G-308 A). \u0000The work followed the ethical principles set forth by the Declaration of Helsinki by the World Medical Association and the Rules of Clinical Practice in the Russian Federation, the study design was approved by the Interdisciplinary Ethics Committee of the Federal State Budgetary Educational Institution of Higher Education Pacific State Medical University of the Ministry of Healthcare of the Russian Federation on April 27, 2015 (Protocol No. 8), informed voluntary consent was signed by the parents. \u0000When processing digital data, we used the methods of descriptive, parametric and nonparametric statistics of the Statistica 10 program, comparison of unrelated groups by qualitative characteristics, assessment of the correspondence of the distributions of genotypes to the expected values at the Hardy-Weinberg equilibrium. Khaitov","doi":"10.36691/rja1514","DOIUrl":"https://doi.org/10.36691/rja1514","url":null,"abstract":"The participation of an allergic reaction and its main constituent elements (IgE, mast cells / basophils, eosinophils) in maintaining various types of homeostatic function is now becoming more and more obvious. This is illustrated by the example of such evolutionary acquisitions as antiparasitic and antitumor types of resistance of the organism of highly organized animal species. All three participants in the allergic reaction are involved in the preservation of the mechanisms of these forms of homeostasis, from which follows the more and more often expressed fear that prolonged maintenance of the blockade of the activity of these participants, and even more so, their elimination may be accompanied by undesirable consequences in the form of suppression of specific homeostasis mechanisms, which should be taken into account when introducing into clinical practice of new groups of antiallergic drugs focused on long-term and even lifelong use. The appeal to the development and use of antiallergic agents that eliminate the participants in the allergic response, acquired in the course of evolution and performing homeostatic functions, rather stems from the hopelessness of the situation in the treatment of patients with a severe course of the disease who are resistant to other methods of antiallergic therapy. Understanding and acceptance of such a concept in the scientific community can serve as an impetus for the improvement of existing and the creation of fundamentally new and strategically justified methods of preventing not the very possibility, but the need for the development of an allergic response. Quality control of therapeutic forms of allergens is an essential condition for guaranteeing the clinical efficacy and safety of allergen-specific immunotherapy (ASIT). The technology for obtaining the granular form of the mixed allergen from house dust mites Dermatophagoides pteronyssinus (Der p) and Dermatophagoides farinae (Der f) for oral use was developed in the laboratory of allergens Mechnikov Research Institute of Vaccines and Sera. \u0000Aims: to determine the authenticity of the anphenolic water-salt extract of the mixed allergen from mites Der p and Der f, intended for the therapeutic form of the drug. \u0000Materials and methods: Standardization of the obtained allergen extract was carried out by protein nitrogen unit by the Nessler method. The protein concentration was determined by the Bradford method. Sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) was used to study the presence of protein composition in mixed allergen. The pH value was determined potentiometrically. The comparison of the specific activity of the standard allergen and the experimental mixed allergen was carried out by the method of reverse immunoassay (REAST). \u0000Results: SDS-PAGE showed the preservation of the protein profile of the water-salt extract of the mixed allergen Der p and Der f, corresponding to the protein profile of allergens of house dust mites. There were no statistically significant differences in the optical density of the studied allergen (four drug series) and the standard allergen with reference serums containing specific IgE to Der p or Der f when reacting with a Class 3 reference serum in REAST(р0,05). \u0000Conclusions: The above in accordance with the State Pharmacopoeia of the Russian Federation XIV edition is a confirmation of the authenticity of the drug. IgE- mediated form of food allergy remains a serious and growing problem worldwide. Its prevalence is steadily increasing, and is a severe psychosocial and economic burden for patients and their families. Cow's milk and products based on it are important components of a child's diet, which are introduced into the diet of children of the first year of life, but can cause allergic reactions. The traditional management of children with cow's milk allergy consists in prescribing an elimination dairy-free diet and a significant part of patients form tolerance to milk proteins by the age of 5 years. However, with persistent forms of allergy to cows milk proteins, the question is raised about the need for \"active\" tactics of patient management in order to form tolerance. Oral immunotherapy is a promising approach to the treatment of food allergies based on a gradual increase in the allergen taken, by analogy with standardized immunotherapy for respiratory allergens, until a maintenance dose is reached. Each stage of oral immunotherapy should be considered as a personalized therapy. One of the priorities of non-drug therapy of asthma is the fight against overweight. Preventive counseling allows doctors to teach patients the principles of a healthy lifestyle, including controlling body weight. There is no domestic population-based research reflects the coverage of overweighted persons with asthma of the counseling on weight loss in various socio-demographic groups. Thus our study is actual. \u0000Aims: Determine socio-demographic groups among overweighted patients with asthma that doesnt receive a counseling on weight loss. \u0000Materials and methods: This study based on cross-section population-based study \"Know Your Heart\" (2015-2018, Arkhangelsk, Novosibirsk, n=4504). For this research we selected overweighted patients with asthma (n=167). We applied the decision tree to identify socio-demographic groups that havent received a counseling on weight loss. \u0000Results: The probability of obtaining a counseling on weight loss increased by 1.39 times among obesity and pension status in women compared with the coverage level of counseling in the studied sample as a whole (61.7%). Among the men, the probability of obtaining a counseling on weight loss increased 1.27 times in the presence of obesity. Reducing the likelihood of getting counseling both among women (1.39 times compared with the general indicator) and among men (2.2 times compared with the general indicator) was noted if their weight corresponded to the category of excess body. \u0000Conclusion: 61.7% of the overweighted urban population of Russia with asthma received a counseling on weight loss. Groups of overweighted patients with asthma, which are statistically significantly less often obtaining counseling on weight loss: persons with BMI=25.0-29.9, regardless of gender, and non-retired women with BMI30.0. The decision tree developed by us will allow allergists-immunologists and other doctors to be wary of patients from the population groups identified in the study who are not covered by counseling. Thus, the prerequisites for the emergence of new concepts of pathogenesis and the search for the most effective therapeutic modalities arise. At the moment, atopic dermatitis is considered as the interaction of endogenous (impaired immune response, insufficient function of the epidermal barrier) and exogenous (exposure to allergens, chemical or physical irritants, microorganisms) factors. \u0000Environmental factors such as temperature and humidity, genetic makeup, antibiotic use, and good hygiene play a critical role in the maintenance and stability of the skin microbiome. Normally, the skin microbiota is mainly formed by bacteria of the genus Staphylococcus, Propionibacterium, Corynebacterium and Streptococcus. While in patients with AD, in 70% of cases, there is colonization of Staphylococcus aureus on the affected skin, in 39% on intact skin, which secondarily contributes to the development of immune imbalance and increased skin xerosis.
