Okul Öncesi Tekrarlayan Vizing ile İzlenen Hastalarda İmmünglobulin Düşüklüğü Sıklığı

Bu çalışmada okul öncesi vizingli hastalarda immünglobulin düşüklüğü sıklığını araştırmak amaçlandı.\u0000\u0000Gereç ve Yöntemler: Çalışmaya 01.01.2013 - 01.01.2016 tarihleri arasında T.C. Sağlık Bilimleri Üniversitesi Ankara Çocuk Sağlığı ve Hastalıkları Hematoloji Onkoloji Eğitim ve Araştırma Hastanesi Çocuk Allerji ve İmmünoloji Kliniği’nde 72 ay altında tekrarlayan vizing ataklarıyla en az bir yıldır takip ve tedavi edilen hastalar dahil edildi. Hastaların immünglobulin (G,A,M) değerleri geriye dönük olarak incelendi. İmmünglobulin düzeyleri yaş sınırlarına göre normal ve düşük olarak belirlendi. \u0000\u0000Bulgular: Çalışmada ortalama başvuru yaşı 26.9 ay olan 6 yaş altı 585 (%65.6 erkek, %34.4 kız) hasta dahil edilmiştir. Hastaların ortalama takip süresi 2.2 yıldır. Bu hastaların %33.7 sinde en az bir immünglobulinde düşüklük saptanmıştır. Bu hastaların hiçbirinde immün yetmezlik düşündürecek semptom ve bulgular saptanmamıştır. Tüm hastaların %21’inde immünglobulin A, %18’inde immünglobulin G, %7.5’inde immünglobulin M değerlerinde düşüklük olduğu saptanmıştır.\u0000\u0000Sonuç: Hastaların yaklaşık 1/3’ünde hipogamaglobulinemi saptandı. Bu hastalarda immün yetmezlik belirtileri yoktu. In this study, the frequency of pediatric home accidents in the second wave of the pandemic, where social isolation continues, was investigated. \u0000\u0000Material and Methods: This is a single center, retrospective study. We evaluated cases of patients under the age of 18 who were admitted to the hospital due to a home accident between November 1, 2020 and January 31, 2021 (the second wave of the pandemic). Results were compared with the patients who were admitted to the same emergency department within the same period of the previous year. Researchers examined home accidents in three groups: crash-fall-incision (1), intoxication-foreign body ingestion (2), and burns (3). The patients were divided into four age groups: 0-1 year, 2-5 years, 6-11 years, and 12-17 years.\u0000\u0000Results: The study was completed with 607 cases pre-Covid and 683 cases post-Covid. The median age was 44.4 month (17-57) from 2019-2020 and 49.1 month (18-64) from 2020-2021 (p=0.154). The outpatient and sequela-free discharges were more frequent in both periods (p=0.046). In the second wave of the pandemic, there was an increase in burns and fall related admissions compared to before the pandemic (p<0.001). A demographic variable questionnaire and The Turkish version of the Carolina Human Papilloma Virus Immunization Attitudes Scale (CHIAS) were assessed.\u0000\u0000Results: The Turkish version of the Carolina Human Papilloma Virus Immunization Attitudes Scale mean± SD scores of harms/barriers/effectiveness/uncertainty factors subgroup were calculated as 2.2±0.6, 2.5±0.7, 2.3±0.7, 2.9±0.8, respectively. 19.8% of parents were aware of Human Papilloma Virus and 22.5% of them declared to be willing to vaccinate their daughters against the infection. Human Papilloma Virus acceptance rate was still modest and lower than for vaccines in general during COVID-19 Pandemic.\u0000\u0000Conclusion: According to the result of this study, during COVID-19 Pandemic, the both female and male awareness and intention to vaccinate their daughters against Human Papilloma Virus is low. This is the first study analyzing parents’ current attitudes and beliefs using a standardised scale towards Human Papilloma Virus in Turkey and also is the first analyzing this subject during COVID-19 pandemic in the literature. Çalışmaya 1 ay ile 18 yaş arasında konjenital genitoüriner hastalık tanısı alan hastalar dahil edildi. Hastaların yaş, cinsiyet, başvuru tarihleri, takip süreleri, cerrahi ihtiyaçları ve yatış ihtiyaçları elektronik hasta dosyalarından kaydedildi.\u0000Bulgular: Konjenital genitoüriner hastalık tanısı almış toplam 288 Suriyeli mülteci çocuk (183 erkek/105 kız) çalışmaya dahil edildi. Iki yüz seksen sekiz Suriyeli mülteci çocuk arasında konjenital böbrek ve üriner sistem anomalileri (CAKUT) 175 hasta (%60,8) ile önde gelen konjenital ürogenital hastalık idi. Bunu 61 hasta ile (%21,2) nonspesifik hidronefroz, 47 hasta ile(%16,3) ürogenital anomali ve 5 hasta ile (%1,7) kistik böbrek hastalığı izledi.\u0000Sonuç: Sonuç olarak sağlık personeli, sosyal hizmet uzmanları ve tercümanlar arasındaki yakın işbirliği, hastaları uzun vadeli komplikasyonlardan koruyarak daha iyi bir yaşam kalitesi sağlamanın yanı sıra sağlık sistemini ek mali yükten de koruyacağını düşünüyoruz. Tedavi merkezlerinde kültürel duyarlılık ve mültecilerin özel sorunlarına yönelik disiplinler arası eğitimler oluşturulmalıdır. While pain is not usually caused by a serious cause, it can be of concern to the patient and their parents. Clinicians should inform the family of the benign nature of chest pain, unless they consider an important underlying cause. Routine referral of these patients to the cardiology department is not recommended, as it increases family and patient anxiety and may cause unnecessary evaluation. Chest pain can occur at any age, but its incidence increases after 10 years of age. The most frequent causes are costochondritis, chest wall muscle pain due to trauma or muscle strain and respiratory tract diseases. We evaluated the use of ES in hospitalized patients in the NICU of our hospital in terms of transfusion rate, indication, laboratory control, complications, and adherence to the guidelines.\u0000Material and Methods: Patients who were hospitalized at the University of Health Sciences of Turkey, Dr Sami Ulus Maternity and Children Research and Training Hospital, NICU, in 2016, and who received ES were included in this descriptive study.The demographic and clinical characteristics of the patients, the quantity of ES used, and the laboratory tests of the first three transfusions were recorded.The compliance of ES usage indications with the transfusion guidelines published in the Nelson Pediatric Textbook and the Turkish Neonatology (TND) Society Blood Products Transfusion Guidelines was assessed.\u0000Results: One hundred and ninety one of the 1538 admitted patients in the NICU received a total of 633 ES, for a 12.4% ES usage rate. Following an evaluation of the first three transfusions, it was determined that there was 66% compliance with the TND Blood Products Transfusion Guide and 64% compliance with the Nelson Pediatric Textbook transfusion protocol. It was significant that the frequency of retinopathy of prematurity (p=0.015) and intracranial hemorrhage (p=0.001) was high in premature infants who received more than one ES.\u0000Conclusion: Although transfusion is life-saving in crucial circumstances, there may be a cause-effect relationship between the detected morbidity and complications. Yalçin","doi":"10.12956/tchd.1285545","DOIUrl":"https://doi.org/10.12956/tchd.1285545","url":null,"abstract":"ABSTRACT\u0000Objective: This study aims to examine the factors that can cause postpartum depression (PPD) form others who gave birth at Şan Med Hospital in Şanlıurfa.\u0000Materials and Methods: The study was performed on volunteer mother-baby couples who were born in Şanlıurfa Şan Med Hospital and were admitted to Şan Med Hospital Pediatrics Polyclinic between September 2017 and December 2017. Participants were asked to provide some information about themselves and their babies, as well as their babies’ crying and sleeping patterns. The Edinburgh postpartum depression scale (EPDS) was administered to the mothers.\u0000Results: It was determined that the frequency of PPD increased by 2.73-folds for mothers whose babies cried excessively, and by 2.79-folds for the mothers whose babies had unconsolable crying/restlessness lasting 2-3 hours a day. The prognosis of MN is variable, ranging from spontaneous complete remission to end-stage renal disease (ESRD). The lack of large multicenter studies precludes the possibility of examining in detail the treatment options and clinical outcomes in these patients. The present study aimed to expand the literature on the clinical findings, treatment, and prognosis of MN in pediatric patients.\u0000Material and Methods: This single-center retrospective study included 13 patients with a diagnosis of primary and secondary membranous nephropathy. \u0000Results: The mean age of the sample was 12.29±3.67 years. Complete remission occurred in 7 (53.8%) patients (of which 1 case was spontaneous remission), and partial remission occurred in 4 (30.8%) patients. In long-term follow-ups; one patient had chronic kidney disease (CKD) and one patient had end-stage renal disease (ESRD). At the last-follow up, proteinuria was noted in 6 (46.2%) patients and microscopic hematuria was noted in 4 (30.8%) and 9 patients were still using low-dose steroids. \u0000Conclusion: The current findings have not identified any significant risk factors associated with the prognosis of MN in pediatric patients, but are thought to contribute to the limited data on pediatric MN. Most of the available data on the natural history, treatment options, and long-term outcomes of MN in the pediatric population consists of small, uncontrolled case series. Her bir hastalığın prevalansı nadirdir. Ancak bir kombinasyon olarak genel prevalans 1/784 kadar yüksektir. Çoğunluk (%72) 15 yaşında ve 1/3’ü ise 1 yaşında teşhis edilir. Nörometabolik hastalıklar genellikle doğum öncesi dönemden yetişkinliğe kadar belirli substratların eksikliği, toksinlerin veya metabolitlerin birikmesi ile tanımlanır. Klinik özellikler çoğunlukla ilerleyici nörodejeneratif bozukluklardır. Semptomlar genellikle kronik kötüleşme veya epizodik alevlenme ile ortaya çıkar. Çoğu hastalıkta genç yaşlarda ve sıklıkla birden fazla organ tutulumu ile ortaya çıkar. Anormal göz hareketleri ile birlikte hareket bozuklukları da sıklıkla görülür. Kalıtsal metabolik hastalıklarda göz tutulumu primer veya sekonder olabileceği gibi bulgular lokal veya sistemik olabilir. Patogenezden genellikle anormal metabolitlerin toksik etkisi veya normal metabolitlerin birikimi sorumludur. Tedavi edilebilir nörometabolik hastalıkların erken tanınması, hastanın tedavi sonucunu değiştirebileceği için önemlidir. Non-infectious uveitis is the most common extra-articular manifestation of JIA.The aim of this study is to evaluate the risk factors that play a role in occurrence and recurrence of uveitis and, to determine the relationship between arthritis and uveitis activity in patients with JIA.\u0000Material and Methods: This retrospective, cross sectional study included JIA patients with/without uveitis from a referral center in Turkey. The Juvenile Arthritis Disease Activity Score was used to evaluate the disease activity and calculated for arthritis and uveitis separately.\u0000Results: Uveitis was seen in 26 (13.3%) of 195 JIA patients. Of 26 JIA associated uveitis (JIA-U) patients, 19 (73%) had an oligoarticular subtype. The median age at diagnosis of JIA with uveitis was younger than without uveitis (p=0.015). Oligoarticular JIA was found to be associated with recurrence of uveitis (p=0.021). The occurrence age of arthritis and uveitis was significantly younger in patients with recurrent uveitis (p=0.041, p=0.002, respectively). The median JADAS27 score at the onset of uveitis was lower in the recurrent group (p=0.038). \u0000Conclusion: Early age is a significant risk factor for occurrence and recurrence of uveitis.
