It mostly affects teenagers and, in most cases, clinical healing occurs as patients grow up. However, 10-40% of adults may have acne tarda. Due to the fact that face is one of the favorite locations of rashes, acne significantly worsens the quality of life, as a result of which patients develop social phobia, anxiety disorders, depression and even suicidal ideas, which requires a personalized approach to prescribing highly effective treatment. According to world recommendations, regardless of the severity of acne, almost all patients are shown the use of local remedies. In our pharmaceutical market, the choice of this group of drugs is quite limited. However, in September 2022, a new drug appeared in the arsenal of Russian dermatologists, which is a combination of benzoyl peroxide and clindamycin. Recent evidence suggests that it is necessary to take into account the individual characteristics of methotrexate pharmacokinetics, which are determined by the presence of polymorphisms of genes encoding methotrexate carrier proteins, to predict the risk of methotrexate-induced toxicity. \u0000Goal: \u0000To identify associations of ABCB1 rs1045642 polymorphism with methotrexate safety in patients with psoriasis. \u0000Materials and methods: \u0000The study included 75 patients diagnosed with psoriasis who received methotrexate. Data on adverse drug reactions were collected using a clinically structured questionnaire, complete and biochemical blood tests, and urinalysis. The severity of adverse drug reactions was assessed using visual analog scales and the CTCAE toxicity scale. The severity of gastrointestinal ADR was assessed using the GSRS questionnaire. Genotyping was carried out by real-time PCR. \u0000Results: \u0000Gastrointestinal toxicity was detected in 38 patients (50.67%). The mean GSRS score was 7.979.18. Analysis of differences in the incidence of adverse drug reactions showed the presence of statistically significant differences in the frequency of adverse drug reactions in the gastrointestinal tract: the toxic effect of methotrexate was more often observed in carriers of the T allele of the ABCB1 rs1045642 polymorphism (3435CT), (CC - 2 (14.3%), TC - 18 (52.9%), TT - 18 (66.7%), p=0.006). Binomial regression demonstrated the presence of a statistically significant effect of the rs1045642 polymorphism of the ABCB1 gene on the incidence of ADR from the gastrointestinal tract: estimation -2.16, OR = 8.64, 95% CI OR: 1.78 - 42.01, p -value = 0.008. \u0000Conclusion: \u0000An association of ABCB1 rs1045642 polymorphism with the safety of gastrointestinal methotrexate therapy in patients with moderate and severe forms of psoriasis was revealed. Basal cell skin cancer is the most common malignant epithelial skin neoplasm in fair-skinned people and accounts for two-thirds of all skin cancers in this population. Tumor lesions have a supporting vascular network that serves as a target for vascular selective lasers. \u0000Aims. To assess the effects of laser radiation at a wavelength of 585 and 1064 nm on the vasculature of various forms of basal cell skin cancer using dermatoscopy and in vivo confocal microscopy. \u0000Materials and methods. The study included 20 patients with 20 lesions of histologically confirmed basal cell skin cancer treated with a pulsed dye laser (wavelength 585 nm) and a long-pulse neodymium laser (wavelength 1064 nm) in the conditions of the consultative and diagnostic center of the Federal State Budgetary Institution GNTSDK of the Ministry of Health RF in the period from 2021 to 2022. The effectiveness of therapy was assessed using non-invasive methods for examining the skin. \u0000Results. According to the results of the two-year follow-up period, 100% effectiveness of therapy in both groups is noted, in 2 patients treated with a neodymium laser, the formation of hypertrophic scars in the treatment area is noted. \u0000Conclusions. Treatment of patients with basal cell skin cancer with selective vascular lasers may be an effective treatment option. {"title":"Periods of psoriatic arthritis onset and diagnosis in Russian patients with moderate to severe psoriasis (according to psoriasis patient registry)","authors":"E. Bogdanova, A. Early diagnosis and treatment initiation in patients with psoriatic arthritis decreases the risk of adverse outcomes and improves patients quality of life. \u0000Aims: to describe a period between psoriasis and psoriatic arthritis onset and a period between psoriatic arthritis onset and diagnosis in Russian patients with moderate to severe psoriasis. \u0000Materials and methods: analysis of data from the psoriasis patient registry. Patients aged 18 years and older with an established diagnosis of psoriasis and psoriatic arthritis were included in the analysis. Exclusion criteria: no data on timing of psoriatic arthritis onset or diagnosis. \u0000Results: sample size for the descriptive characteristic of period between psoriasis and psoriatic arthritis onset was 891 patients; sample size for the descriptive characteristic of period between psoriatic arthritis onset and diagnosis was 849 patients. \u0000Psoriatic arthritis onset preceeded cutaneous manifestation of psoriasis in 2.5% of patients. Concurrent onset of cutaneous psoriasis and psoriatic arthritis (within one year) had 10.7% of patients. In 86.8% of patients, cutaneous manifestation of psoriasis preceded manifestation of psoriatic arthritis. \u0000Exclusively of patients with psoriatic arthritis developed before or concurrently with cutaneous manifestation of psoriasis, the mean duration of period between psoriasis and psoriatic arthritis onset was 13.7 10.3 years, the median [IQR] 12 [6-19] years. \u0000In 24% of patients with psoriatic arthritis the was 1 year or more after its onset. On average, in this subgroup of patients, psoriatic arthritis was diagnosed 3.95.0 years after the signs and/or symptoms onset. Median [IQR] of period between psoriatic arthritis onset and diagnosis was 2 [1-4.5] years. \u0000Conclusions: a diagnostic delay in 24% Russian patients with psoriatic arthritis is one year or more. Ruksha","doi":"10.25208/vdv1327","DOIUrl":"https://doi.org/10.25208/vdv1327","url":null,"abstract":"Background: Acne vulgaris belongs to androgen-dependent diseases where androgens cause sebaceous gland hypertrophy with a subsequent enlargement of sebum production. \u0000Aims: Determination of the CYP450scc 3-hydroxysteroid dehydrogenase expression profile in normal skin and skin of patients with acne. \u0000Material and methods: Immunohistochemical assay was carried out to evaluate cytochrome P450 side chain cleavage and 3-hydroxysteroid dehydrogenase expression in skin of patients with acne vulgaris and in normal skin. \u0000Results: Cytochrome P450 side chain cleavage expression was elevated in epidermal keratinocytes and sebocytes whereas 3-hydroxysteroid dehydrogenase levels were increased in mature sebocytes of acne vulgaris patients. \u0000Conclusions: The revealed alterations in expression of steroid-converting enzymes in acne skin may indicate not only the conversion of steroids produced by external steroidogenesis organs but elevated steroids local production as well. In the early stages of mycosis fungoides, a treatment approach using external therapy and various ultraviolet irradiation spectra (UVB-311 nm and PUVA therapy) is preferable. With insufficient efficiency and / or short remissions in some patients, it is possible to use combined methods in the early stages: PUVA therapy or UVB-311 nm therapy. in combination with systemic therapy, including interferon (IFN) 2b preparations, methotrexate and retinoids. The results of original studies demonstrate an increase in the overall response rate when combining PUVA therapy with IFN- or retinoids in patients with stages IB-IIB. Russkikh","doi":"10.25208/vdv1379","DOIUrl":"https://doi.org/10.25208/vdv1379","url":null,"abstract":"Background: A comparative analysis of the results of a survey of young people (belonging to two different generations: millennials and zoomers) on sexual behavior and awareness of STIs and HIV infection allows us to assess the existing characteristics of the sexual behavior of young people and contribute to the further improvement of preventive measures among young people regarding STI/HIV infection. \u0000Aims: Comparative analysis of sexual behavior and assessment of the level of knowledge about STIs and HIV infection among young people (generations Y and Z). \u0000Materials and methods: For a comparative assessment of the level of awareness on the issues of prevention and risky sexual behavior in relation to STIs and HIV infection, data from earlier sociological surveys of young people in 20012002 and in 20172020 in the city of Novosibirsk were used. Among the respondents, a questionnaire was used, consisting of 23 questions, developed by the staff of the Department of Dermatovenereology of the St. acad. I.P. Pavlov and the Center for the Sociology of Deviance and Social Control of the Institute of Sociology of the Russian Academy of Sciences, adapted by the authors. Statistical analysis was carried out using Microsoft Excel and SPSS programs. The 2 test and the independent Student's t-test were evaluated. Statistical significance was set as p0.05 \u0000Results: A total of 772 young men took part in the study, of which 321 students were interviewed in 2001-2002. (Generation Y), and 451 students in 2017-2020. (Generation Z). The results show that generations of young people (millineals and zoomers) differ from each other in terms of the source of information about STIs, sexual intercourse with strangers, condom use, and the frequency of same-sex sexual intercourse. \u0000Conclusions: There is a positive trend in the sexual behavior of the younger generation. The clinical picture of the combined pathology consists of symptoms characteristic of both diseases, which makes diagnosis difficult. \u0000Management tactics of patients with combined pathology, first of all, involves the elimination of symptoms of rosacealike dermatitis, dominating the clinical picture, using metronidazole, calcineurin inhibitors, azelaic acid, in severe cases - low doses of systemic isotretinoin.
