Albanova","doi":"10.25208/vdv1396","DOIUrl":"https://doi.org/10.25208/vdv1396","url":null,"abstract":"Hereditary ichthyosis is a group of generalized hereditary keratinization disorders characterized by general dryness of the skin, peeling, hyperkeratosis and often erythroderma. These manifestations are caused by mutations in genes mainly involved in the formation of the skin barrier. Hereditary ichthyosis is divided into syndromic and non-syndromic. Nonsyndromic ichthyoses include: vulgar ichthyosis, recessive X-linked ichthyosis, autosomal recessive congenital ichthyosis, keratinopathic ichthyosis and other forms. At present, the best result for achieving clinical remission has been established with oral retinoids: retinol palmitate, isotretinoin (1st generation of retinoids) and acitretin (2nd generation of retinoids). The ability of retinol palmitate to regulate keratinization processes, strengthen the epidermal barrier and have an antioxidant effect is used in the treatment of generalized hereditary keratinization disorders. Medium and high therapeutic doses (2 000-10 000 IU / kg / day) are used in the treatment. The prescribed dose of retinol palmitate differs in various nosological forms of ichthyosis, and depends on the severity of the pathological process, the age and weight of the patient, which must be taken into account when prescribing therapy to obtain the best result. It should be noted that clinical manifestations mainly regress at doses that do not lead to the appearance of signs of toxicity of the drug. Despite advances in diagnostics, the identification of the primary focus of melanoma in some cases is still difficult. The natural course sometimes manifests only with the appearance of melanoma metastases, which can mimic other diseases. Patient S., 52 years old, was admitted to the FCBRN of FMBA of Russia with complaints on periodic systemic dizziness, headaches of a pressing nature, episodes of speech impairment over the past three months. According to the brain MRI-scan results, a volumetric formation of the left frontal lobe was revealed. Upon examination, two non-pigmented lesions were found on the skin of the scalp and forehead. Due to the presence of focal neurological symptoms, it was decided to remove the brain tumor using neurophysiological monitoring and the scalp skin lesion, with histological verification. Morphological diagnosis of the removed brain tumor was a metastasis of amelanotic epithelioid melanoma. The skin lesion was basal cell carcinoma. Thus, the patient had primarily multiple malignant tumors: metastatic melanoma and basal cell carcinoma. The primary focus of melanoma could not be identified by available noninvasive research methods. The patient was referred to an oncologist to decide on the tactics of further examination and treatment. Skin manifestations: erythroderma, xeroderma, excoriations, persistent white dermographism. The conclusion of the ophthalmologist: rhegmatogenous retinal detachment, complicated cataract, ophthalmohypertension of the right eye; hemophthalmos, complicated overmature cataract, terminal neovascular glaucoma of the left eye. A diagnosis of Andogsky's syndrome (SA) was made. He received treatment: prednisolone 60 mg per day parenterally for 14 days, methotrexate at a dose of 10 mg weekly, Reamberin, folic acid 5 mg weekly, asparcam, aevit, enterosorbents, ointments and creams with corticosteroids and moisturizers. As a result of treatment, hyperemia, infiltration, peeling and itching of the skin decreased. It is recommended to continue taking methotrexate, folic acid, prednisolone (5 mg daily) and asparkam on an outpatient basis; externally - moisturizing and softening creams and ointments. The reasons for the late appeal of patients for medical help are established - psychological states of fear and anxiety. \u0000The clinical picture of the presented observations was characterized by a slow long-term asymptomatic growth of solitary tumor-like nodes of a mushroom-shaped form of stagnant pink color with a bumpy surface, densely elastic consistency, soldered to the underlying soft tissues, sizes 9.5x7.0 cm and 5.0x9.0 cm, respectively. Giant basaliomas were localized on the scalp in a woman and on the skin of the trunk in a man. The clinical features of the tumors corresponded to those of a large conglomerated basal cell skin cancer. A description of the clinical and dermatoscopic picture of giant nodular basaliomas is presented. \u0000 The patients underwent radical surgical excision of tumors with pathomorphological examination of the postoperative material. The histological picture of giant basal cell carcinoma in both cases is represented by tumors of a complex structure - a solid adenoid type with invasion into the reticular dermis and subcutaneous fat. The global prevalence of syphilitic infection among people living with HIV exceeds population rates. Concomitant HIV infection can affect not just the clinical course of syphilis, but also the production of antibodies to Treponema pallidum. In the presence of severe immunodeficiency in patients with HIV infection associated with secondary syphilis, the results of non-treponemal and/or treponemal tests may be false-negative or may become positive at a later date. Such cases are known, they occur infrequently and cause some diagnostic difficulties. The article presents a clinical observation of delayed positivity of serological reactions in secondary syphilis in a 23-year-old HIV-positive man from the authors' practice. The development of atopic dermatitis involves environmental factors that violate the skin barrier, as well as genetic, immunological features of the patient.In modern science, the issue of pathogenetic treatment of atopic dermatitis remains not fully resolved. The first manifestations of the disease appeared after an injury in the region of the left shin, an abrasion appeared, which later transformed into a glossy spot that slowly increased in size. She first sought medical help in 2017 and was clinically diagnosed with necrobiosis lipoidica. The prescribed treatment did not lead to an improvement in the patients condition. In 2021, after vaccination against COVID-19, the patient notes a sharp deterioration in the process. No convincing data for diabetes mellitus received. She has a history of multinodular non-toxic goiter, for which in 1995 a complete resection of the left lobe of the thyroid gland was performed and in 2003 a partial resection of the right lobe of the thyroid gland. The patient has been receiving L-thyroxine replacement therapy since 2003. A biopsy of the affected skin was taken. The final diagnosis was established according to the pathological and anatomical study of the biopsy. The main genomic effects of the natural form of vitamin A (retinol palmitate) are associated with its active metabolite all-trans-retinoic acid and are compensated by several restrictive mechanisms. Numerous studies have proved that retinol stimulates the proliferation of keratinocytes of the basal layer of the epidermis and endothelial cells, and also activates dermal fibroblasts to synthesize proteins of the extracellular matrix of the dermis. As a result, the thickening the epidermis, increases the mechanical strength of the skin and the hydrating ability of the dermis, angiogenesis increase. The ability of retinol to enhance the adhesion of endothelial cells and leukocytes, regulate the processes of keratinization and sebum secretion was found. Vitamin A is also a powerful antioxidant. Retinol palmitate is used as the main or auxiliary drug for the treatment of a wide range of dermatoses. The principle of application is based on clinical studies and confirmed by existing experimental data. In the treatment, the following algorithm is followed. If retinol palmitate is necessary to improve epithelialization and strengthen the epidermal barrier, medium therapeutic doses should be used. For the treatment of disorders of keratinization processes, depending on the severity of the pathological condition, medium and high therapeutic doses of the drug are used. Violation of the processes of sebum secretion and severe hyperkeratosis respond better to treatment at high therapeutic doses. Maleev","doi":"10.25208/vdv1366","DOIUrl":"https://doi.org/10.25208/vdv1366","url":null,"abstract":"Aims: to study the prevalence rate and clinical features of extragenital STIs in both males and females. \u0000Materials and methods: a prospective observational clinical study included 57 patients with a history of unprotected orogenital and/or anogenital sexual contacts: group I (n=20) women practicing heterosexual sexual contacts, group II (n=9) men practicing heterosexual sexual contacts, group III (n=28) men who have sex with men (MSM). The identification of STIs was conducted by the polymerase chain reaction method and the ribosomal RNA NASBA detection method was used as an independent additional direct method. \u0000Results: a high level of detection of STIs of extragenital localization has been established: in heterosexual women in the rectum (10; 76.9%), in the oropharynx (9; 69.2%) and in the conjunctiva (2; 15.4%); in heterosexual men in the rectum (2; 40%), in the conjunctiva (2; 40%) and in the oropharynx (1; 20%); in MSM in the rectum (15; 75%), in the oropharynx (11; 55%) and in the conjunctiva (5; 25%); in 14 (36.8%) patients, STI infection of more than 2 localizations was found. In extragenital foci in group I patients chlamydia trachomatis infection was most frequently detected (5; 38.5%), in group II patients gonococcal infection (2; 40%), in group III patients syphilis (9; 45%), gonococcal infection (8; 40%) and chlamydia trachomatis infections (8; 40%). Before treatment and three months after start of methotrexate therapy, all patients underwent videodermatoscopy with dimension of density and size of dilated skin capillaries, ultrasonography of psoriatic plaques and nail bed of affected nails and measurement of doppler blood flow parameters and concentration of VEGF and En-1 in blood plasma. \u0000Results: a direct correlation was established between the average diameter of dilated skin capillaries (vascular glomeruli), the degree of increased blood flow in the doppler energy study of ​​psoriatic plaques skin area and the plasma concentration of VEGF and En-1 and values ​​of PASI, BSA, sPGA and DLQI indices, as well as the severity of doppler blood flow of the nail bed and NAPSI index.Under the influence of methotrexate treatment, a decrease in diameter and density of vascular glomeruli, the degree of blood flow in area of psoriatic plaques and the concentration of VEGF and En-1 was observed. \u0000Conclusions: studied indicators of angiogenesis can be used as additional criteria for assessing degree of activity and achieving clinical improvement/remission during systemic therapy in patients with moderate and severe psoriasis.","PeriodicalId":23618,"journal":{"name":"Vestnik dermatologii i venerologii","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44362466","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
